Heavy equipment technician was removing rubber step material from a Caterpillar 992 loader with a razor knife. The knife slipped and lacerated EE's left palm and pinky finger. EE was treated at the local clinic and received stitches and was released back to duty the same shift.

An employee was using an angle grinder to debur a socket. The socket was placed in a vice, and the employee was holding the grinder with their right hand. The stopped the task, and moved the grinder to their left hand. While doing so, the grinding disc was slowing down but still spinning and contacted the back of their right hand resulting in a laceration.

A loader operator was loading haul trucks, and a large rock was placed into the bed, causing the truck to shake. This caused discomfort to the haul truck driver's neck and back.

Employee was performing housekeeping duties in the fire assay lab when a foreign body entered the left eye while sweeping off the ovens.
The employee was attempting to cut a piece of skirting for a feed chute. The knife slipped while cutting, struck the right leg causing a small laceration.
Employee was loading trash into the bucket of the loader in the maintenance yard at 11:00 pm. They lifted the front of a pallet box over the lip of the loader bucket, then repositioned themselves to push the pallet box into the bucket. As they pushed, they felt a pop in their lower back, followed by a sharp pain.
Two Drill operators were changing the sub on a drill. At approximately 16:15, the second operator positioned the J wrench on the sub to break it loose from the drill steel. The drill operator inside Cab went through neutral on the controls and engaged reverse rotation of the drill steel. As a result, the J wrench swung back and pushed the operator's foot beneath the carousel.

While performing a 360-degree walk around of a haul truck, an employee slipped on ice which resulted in pain to their right knee.
The injured employee was cutting a plastic cable tie with a knife. As the knife cut the tie, the downward pressure caused the knife to move across the employee's leg causing a laceration.
While tripping out of the hole IP (Injured Person) started slowing down, driller told IP to take a break and clean up pad. While picking up pad the helper noticed a behavioral change in IP. IP then said might be injured. Driller then took IP to the truck called work care and drove IP back to town.
A mechanic was positioning a wheeled jack stand under a haul truck to make a repair when the worker's middle finger on their left hand was caught between the jack's handle and the belly pan, causing a laceration and a fracture.

Employee was dismounting HTK 82146, 777F. On the last rung Operator stepped wrong on left foot and sprained the ankle.
Employee's finger was caught between the lid and dust bin after emptying.
Employee was performing clean-up duties with a skid steer loader and encountered rough terrain causing pain in the lower back.
Employee was standing on a short 3:1 slope and was prepared to pull a piece of plastic up slope. When employee raised the plastic sheet above the ground, a gust of wind pulled the plastic which was being held by employee. Employee was pulled forward down the slope, fell unrestricted head first down the slope landing with head and right shoulder hitting the ground first.
Employee was fixing a lifting strap on one of the forks of the forklift when the strap on the other fork slid and caused the load to shift on the fork the employee was working with. The fork moved back pinching the edge of EE's hand between the fork and carriage.

777F Haul Truck 82197 was moving material from the Dynamite Pit to the Soft Ore Stockpile using the A Road. Truck lost control, loaded, down hill. During corrective maneuver the truck overturned onto the left side. The Operator safely dismounted the vehicle with minor discomfort in lower right leg.
Employee was moving a drum of dirt and felt a pain in the lower back. Employee did not want medical evaluation at the time of incident. No follow up needed. Requested medical in November, no issues for three months. Dr requested MRI and have had no follow up with Workmans Comp following initial visit.
The employee was staking out ore control when a gust of wind blew dirt and dust under the safety glasses and entered the right eye. The Dirt and dust caused minor scratches and abrasions to the eye.
At 5:40am an employee was dismounting the water truck at the end of night shift. While climbing down the steps and maintaining three points of contact, EE reached for the equipment shut off master switch. Footing slipped from the bottom step and they fell backwards from a height of 18", to the ground resulting in injuries to the head and arm.

Technician was using an impact driver to remove a pin from the articulation cylinder. The pin suddenly came loose, crushing the technician's left index finger between the back of the driver and the engine compartment door. Left index finger had been previously amputated to the first knuckle. The crush caused an open fracture to the second finger bone.

At approximately 1:45 pm, a Ledcor employee notified a coworker of the sudden onset of chest pain. The employee was operating a Caterpillar 777F Haul Truck. No violations of health and safety standards were observed or found to contribute to the employee's condition. Employee received medical treatment and was released the same day with no diagnosis.

While washing the cab window on a DM45 Rock Drill an employee stepped into an open deck sampling hole with his left leg. The employees right leg slid out from underneath him causing injury to the employees right leg. The employee was taken to the ER and assessed for injury. Employee was released back to modified work with a request for the employee to seek care from a orthopedist.

Mechanic cut fingertip using razor blade. While first aid was being given he passed out due to the sight of his own blood and struck his head as he collapsed. Was revived and transported to local clinic. They couldn't treat him and he was flown to UMC Trauma in Las Vegas. Cat scan was performed and read in Las Vegas. He was then released to return to work at 2:15 pm that day.
Mechanic was repairing seal on hydraulic ram block. He disconnected hose and pushed it out of the way. The hose then slipped and struck the mechanic in the tooth.
