While working on A/C unit for a mixer truck, a shop air hose crimp failed and shot out the air fitting. Hitting employee in the head, causing a head injury. Did not break the skin, but did cause swelling causing EE to go to ER.

Employee was checking for oversize rock around crusher fine stacker, wind caused dust or particles to enter left eye causing abrasions.

Miner was crossing a small ditch when miner slid on the mud and fell. Causing pain to left knee, left hip, and right shoulder.
Right thigh was struck by rock while operating a skid steer loader.

Walking to the trash bin, lost footing and fell. Released after medical examination to return to work.
While checking the settling pond in windy conditions with proper eye safety wear in place, foreign material unexpectedly entered the right eye. This later caused an infection which required medical attention.
A mechanic in a truck bay was tightening a tag axle on a mixer truck when a flake of debris fell behind the mechanic's safety glasses and into right eye. Employee sought medical care to remove the debris from right eye and took one day off to recuperate from conjunctivitis.

During driveshaft removal on mixer truck, a mechanics hand was pinched between the driveshaft and pusher crossmember. Right hand pinky finger required sutures due to a cut.

A groundman was shoveling material on a catwalk when they felt a pop and pain in back and subsequently abdomen pain. Shoveling excess material on the catwalks is required at the end of each crushing shift. The employee was in a hurry to get the job done and move to the next catwalk.

Employee was shoveling underneath the 7x20 screen prior to starting up, EE stood up too close to the screen and hit pelvis on bottom of 7x20 screen causing a bruise and pain. Employee was prescribed pain medication and allowed to come back to work after one full day off.

While removing a pulley from a jaw crusher pulley shaft suspended by forklift chain 4-5" off the ground in the maintenance bay the shaft came free and hit the tip of an employee's left index finger, breaking the bone. The supervisor had instructed the employee to hold onto the outside of the pulley, but at some point the employee put his finger inside the pulley where it was hit.

Employee complained of pain in lower right side of back after changing oil on jaw feeder and cone, he was taken to the ER, doctor there said he could return to work at full capacity when pain free and cleared by a physician. Employee later told supervisor he previously broke his back in a high school car accident. Employee did not tell us he had any physical limitations when hired.
Employee saw a doctor for discomfort in his left eye on 7/26/14, employee thought it was from dirt getting in his eye on 7/23/14. He was diagnosed with ultraviolet keratitis (eye sunburn) in left eye. After investigating we believe that it was caused by torch cutting without shaded eye protection on 7/20/14. Doctor restricted employee from welding for one week.

Employee was working under the Jaw belt under windy conditions and the wind blew something into his eye. He rubbed it causing a scratch.
Employee was running the excavator and needed to use the restroom. He climbed off excavator using three point contact. When he reached the ground he turned and took a step and fell because of the uneven ground below him, hurting his shoulder as he landed on it.

Employee believes he pulled his groin after shoveling material or pulling a belt, didn't feel any pain until several hours later while riding a bobcat. Unsure of what caused the injury and when it occurred. Doctor recommended no heavy lifting for 10 days.
