Employees were putting on the hoist cables for the dredge bucket while pulling cable up. Employee was feeding the cable over the drum to other employees. The cable had pinched employee's hand/finger into the hoist drum cutting through the gloves and cutting their right index finger.

Employee was removing cable clips between a 40 mm and 3/4 cable the 3/4 cable was broke and was frayed {had cable wires sticking out} while removing cable clips it allowed the 3/4 cable to slide down and cut their wrist.

Individual was cleaning a catwalk by oversized collector on plant# 2. When EE stood back up, EE stepped on a rock and rolled ankle falling to the ground. Sought medical treatment and therapy on own. Reported occupational injury on 2/6/24. Had therapy and requires surgery on 5/29/24. Will have restrictions post surgery.

A piece of steel with a lifting eye welded on top of it was sitting in the upright position for lifting. The employee attempted to lay the piece of steel over to keep it from possibly falling over and mashing someone's foot. When EE did this the glove became caught on the lifting lug and EE was unable to get hand away.

At quitting time the employee was exiting EE's service vehicle and twisted EE's left ankle and fell to the ground.

Miner was conducting crusher maintenance - moving a piece of the crusher to a different location - while setting the piece down, crushed left ring fingertip.

EMPLOYEE WAS WELDING IN A CRUSHER. A HELPER HANDED HIM A PIECE OF STEEL. IT SLIPPED OUT OF HIS HAND AND HE MOVED SUDDENLY TO AVOID THE STEEL. ATTHE TIME, HE DID NOT NOTICE A PROBLEM, BUT THE N EXT MORNING, HE COULD NOT GET OUT OF BED.

WORKER WAS SHOVELING UNDER THE TAIL PULLEY OF A CONVEYOR WHEN HIS HAND HIT THE GUARD CAUSING A LACERATION BETWEEN HIS THUMB AND FOREFINGER.

WHILE REPLACIGN V-BELTS ON DRIVE FOR WATER PUMP MAINTENANCE PERSONNEL ATTEMPTED TO TAKE WEIGHT OFF OF DRIVE PULLEY WITH PRY BAR UNDER MTOOR BASE AND DID NOT USE PROPER LIFTING TECHNIQUES AND I NJRUED LOWER BACK.

EMPLOYEE WAS ROTATING VBELT DRIVE BY HAND TO ALIGN SCREW SHAFT FOR REMOVE OF BEARING HIS RT 5TH & PINKY FINGER GOT CAUGHT IN DRIVE UNIT

EMPLOYEE WAS CUTTING SOME CONVEYOR FRAME TO BE RE-USED WHEN THE PIECE HE WAS CUTTING FELL LOOSE & STRUCK HIM BEHIND THE STEEL TOE OF HIS BOOT. THE INJURY WAS A HAIR LINE FRACTURE TO HIS LEFT F OOT BEHIND THE TOE AREA.

EMPL STEPPED OUT OF SERVICE TRUCK & TURNED ANKLE.

OPERATOR WAS CH9CKING ON PROBLEM WITH THE JAW CRUSHER AND GRIZZLIE FEEDER. AFTER INSPECTION, OPERATOR JUMPED TO GROUND. RING ON THIRD FINGER OF LEFT HAND CAUGHT ON HAND RAIL CAUSING THE FINGER TO BE TORN FRO4 HAND.
