Employees were removing the screen frame. It became dislodged quickly and struck an employee in the chin causing a laceration.
Employee's hand slipped while swinging a hammer. The hammer struck EE's face causing tooth damage.

The employee was cutting urethane chute liner when the utility knife slipped out of the cut. This resulted in a laceration below the right thumb. He received 9 stitches.

EMPLOYEE REPORTED TO MANAGEMENT ON 1-19-03 THAT HE THOUGHT HE HURT HIS BACK ON THE PREVIOUS DAY SHIFT WHILE POSITIONING A 10" BY 20" PIECE OF PIPE.
