Equipment Operator had been operating Caterpillar 627F Scraper to strip overburden. Operator had been operating scraper w/out using "cushion hitch" feature. At end of shift on 1/31/12, he spoke of sore neck to co-workers. On 2/1/12 he went to Doctor. Doctor's diagnosis is of a cervical strain and for him not to operate a scraper until some physical therapy could be performed.

We were in the process of repairing the loading ramp for the loading hopper. The employee was compacting around posts that hold the front retaining wall. While he was leaning over the wacker the wacker hit him in his chest resulting in his back hitting a steel beam, thus causing his back problems.

The employee was cleaning out feeder hopper with bar, he was lifting the bar up through the grizzly screen and pinched his thumb between the bottom of the screen and the bar.

Employee was kneeling on the lower end of the conveyor belt installing a material deflector. He lost his balance and fell about 32" off the end of the conveyor to the ground landing on his right side. He tried to break his fall with his left arm and injured it in the elbow area.

EE WAS HELPING PUT A NEW HOSE ON WATER PUMP. GOT IN PICKUP TO GO BACK TO HIS TEAM TO WORK,FELT SOMETHING IN HIS RIGHT EYE. WENT TO CONCENTRA MEDICAL CENTER TO GET CHECKED OUT. THE DOCTOR FOUND NOTHING IN HIS EYE. HE HAD A CORNEAL ABRASION OF THE RIGHT EYE. DOCTOR PUT EYE DROPS IN AND AN EYE PATCH. EE WENT HOME FOR THE DAY.

THE WIND CAUGHT DOOR SUBSEQUENTLY PUSHING IT INTO EE WITH ENOUGH FO RCE TO KNOCK HIM OFF THE STEPS AND TO THE GROUND WHERE HE LANDED ON HIS LEFT SHOULDER. NO TIME WAS LOST. SHOULDER WAS SORE AT THE END OF THE SHIFT (3:30PM). EE HAD IT CHECKED BY DR. AFTER WORK. NO RESTRICTIONS.
