At approximately 12:35 PM, a fire occurred at the TR Bolinder Crusher. Prior to the incident, maintenance activities were being performed on the guarding for the cone feed, during which a torch was used to clear bolt holes for installation of the guard. Upon completion of the task, three bolinder employees left the work area and returned to their trailer for a scheduled break.

THE EMPLOYEE WAS USING A CABLE WENCH TO LOWER A PREP SCREEN. THE PRAWL LOCK RELEASED, JERKING THE WENCH HANDLE OUT OF THE EMPLOYEES HAND, ROTATING COUNTER CLOCKWIDE AND SLAPPING THE BACK OF HI S LEFT HAND IN BACK OF THE KNUCKLES.
LEFT INQUINAL HERNIA AND RIGHT INGUINAL BULGE. HE WAS PULLING UP ON A 5 FT. PRYBAR, TRYING TO HELP THE HYDRAULIC LIFTING RAM TO LIFT THE LID OFF OF A BARMAC CRUSHER. THE HYDRUALIC LIFTING CYLI NDER WAS NOT WORKING PROPERLY.

EE AND OTHER EMPLOYEE WERE LIFTING A DECK SCREEN OUT OF THE FRONT END LOADER BUCKET, THE SCREEN WAS CAUGHT ON A METAL FLANGE ATTACHED TO THE EL-JAY SCREENING PLANT AND EE JERKED ON IT. WHEN TH E SCREEN CAME LOOSE, EE LOST HIS GRIP AND THE SCREEN FELL ON HIS LEFT FOOT. EE HAS A LACERATION AND A FRACTURE TO THE THIRD DIGIT OF HIS LEFT FOOT.
THE EMPLOYEE WAS RELOCATING A 4/4 UNDER A CONCRETE BARRIER FORM.THE BOTTOM OF THE FORM HAD TO BE UNPINNED.THE BOTTOM PLATE FELL SMASHING THE RIGHT INDEX FINGER.
EMPLOYEE WAS HAMMERING ON A METAL CONCRETE FORM TO BREAK LOSE THE FORM.A FOREIGN BODY HIT HIS LEFT EYE.EMPLOYEE WAS NOT WEARING HIS EYE PROTECTION.
