Employee was assisting in changing out a screen. While pulling on the screen to slide it up, they felt a small pop in right shoulder. Diagnosed with shoulder strain and placed on restricted duty of no push/pull greater than 5lbs and no overhead work.

On April 3, 2023, employee was on fire-watch for a small brush pile. Employee was stomping out embers and kicking dirt around, when they felt a strain in their right knee. Employee did not request medical attention until two months later (June 5, 2023). No medical treatment was necessary until July 12, 2023 when EE was given a corticosteroid injection & sent to physical therapy.

Employee states that right foot became stuck in the mud on Monday, January 3, 2022. When employee attempted to remove foot from the mud, employee twisted right knee. This has been a first aid only injury until 03-01-2022. On this date, an orthopedic surgeon placed employee on restricted duty, changing injury status.

Employee fell down on EE's right side and caused a sprain to EE's right wrist. The incident became recordable on 02-15-2022 due to physical therapy. Prior to this date, it has been a first aid only incident (hence the date of accident as 12/01/2021)
Employee was proceeding down the stairs, at the primary, to clock out for the evening. Employee placed left foot/leg on the first step. When employee went to bear weight on the leg, employee felt a pop in knee. Employee was diagnosed with a torn meniscus and sent the physical therapy beginning 11-02-2021. Employee is working full duty.

Drill operator overturned equipment. This resulted in a laceration to the back of head requiring medical treatment beyond first aid.

Employee / miner has been diagnosed with silicosis. This diagnosis was confirmed today, May 23, 2013, by an occupational specialist. Employee has worked at the mine site since January 11, 1993. Since that time, he has had consistent annual chest exams / x-rays with no significant findings, until this date. He has been released to full duty, effective today.

Employee was using a 6 lb. sledge hammer to remove a stuck tooth from a trackhoe bucket. When the employee struck the tooth with the hammer, a small shard of metal broke off and embedded into employee's left forearm. Employee was taken to a clinic to have the shard removed. Two (2) stitches were used to close the wound.
