The employee was taking a busted bag off the conveyor belt and as EE tossed this bag in a wheel barrel the material in the wheel barrel blew up into EE's face getting under EE's safety glasses and causing and eye injury.

Employee was walking the plant all day collecting vibration data with a iPad and collapsed unexpectedly. 1st responders performed CPR, used an AED, & EE was transported to hospital. As of 9/30, remains in the hospital with double pneumonia, diabetes, & 3 blocked arteries to requires surgery. MSHA investigate the same day. No environmental or work related cause was found.
employee was going checking air hoses on a truck. when EE went to step up onto the platform, the step broke and the employee fell into the trailer injuring EE's right arm.

EE was going up to the top of the upper shelf lime silo to unclog and get it to flow when EE opened the smaller round door to probe the lime or rodding if you will, it broke loose and came out of there like water getting all over EE and into EE's right eye.
EE was straightening out the hose on the grease gun to the drum. When EE was walking toward the steel portable silo legs, EE tripped over the steel legs on the ground and hurt left hip. The employee fell to the ground and said that EE couldn't get up and we called for an ambulance to assist.
Employee was removing a broken pallet from the turntable on our production line. The employee slipped on the rollers on the turntable, and strained lower back.

Poor engineering. Employee's finger was smashed by swinging door.
Employee was cleaning up cement that was on the floor under the cement line palletizer and was bent over using a come along to pull out the cement that was under the cement line palletizer when EE stood and turned and fell and twisted EE's knee on EE's left leg.

Employee was palletizing cement when a bag busted and some cement got into the employees eye
I received a phone call at 8:04 of emergency at Sand line. I arrived at 8:06 found an EE holding the head of IE to keep IE's airway open. EE said IE had a fast hard pulse. IE Was laying in the chain belt beside the packer chair. Three paramedics tended to IE at 8:09 and transported to ambulance by 8:12. Narcan was administered. No equipment malfuncions found.

Operator was running the related line, running the packer chair. The other operator who was there running the the machine said that EE stood up. Then EE fell, like EE passed out. EE was unconscious, had a pulse and was breathing. We had 911 called. CPR & AED was used. EMT worked on EE. EE left in ambulance with a pulse to hospital.
Maintenance employee was walking into the maintenance parts shop room when something got into EE's left eye. Eye wash would not remove it. Had to seek outside medical to get it out. Return to work full duty same day as injury.
Operator slipped going up steps and hit left shin on the second step. Then said while they got up they hit their back on the railing and bruised their back. This was not reported until 2/18/21

EE was helping change motor on sand line palletizer. The crew went to tighten the motor but had an alignment issues with the motor. When they tried to adjust the motor it started to fall to the ground. EE took the motor slowly to the ground and in the process bruised left forearm.
Operator was hand adding concentrate for the batching operation when air from fan blew some dust or dirt in left eye causing irritation.
EE was assisting forklift operator moving trash. EE squatted to ground level where to straighten the trash on the forks so the trash could be raised up. As EE squatted, EE felt a sharp pain in the lower middle back.

Operator had just adjusted packer. Needed to adjust again, instead of turning off machine and completing the adjustment, operator tried to adjust machine with machine still on. Operator should have followed procedure. When adjusting machine from front side, operator's head hit switch that made cylinder move and caught operator's arm. Only bruise. X-Rays negative.
Operator was running palletizer for the bagging operation. Bag started to fall from the feed conveyor and operator reached to catch bag. In the process the operator cut right elbow.
Employee said they were coming down stairs and turned on platform twisting left knee.
Employee was walking to maintenance shop to get a forklift for the start of the shift. There are two steps that EE must go up to get to the maintenance shop. EE missed or caught the top step causing EE to fall to knees. Bruised left knee. Was not reported until 2/19/19.

Conveyor belt stopped on sand line. Operator reached past guard to pull belt trying to free operation. Operator did not follow lockout/tagout procedure. Operator got right hand caught between conveyor belt and head pulley. Right pinky received 8 stitches and ring finger has fracture on bone at tip of finger.
Operator said they got right shoulder strain from performing hand add for the batch operation. Operator did not report the condition until 3-19-19 and said it occurred back in October 2018.
Operator was complaining of rash on back, cause could be from lime in contact with skin causing the issue.
EE said EE strained left shoulder on Friday while operating the main line palletizer. EE did not know what time it occurred. EE did not report the injury until Monday 6-18-18
Operator was pulling bag off the conveyor belt. When operator started to step down from the step at the side of the conveyor operator missed/slide at step falling about 16 inches. Operator twisted or sprained left knee.

Operator was hand stacking tube sand bags and said ee pulled a muscle in right shoulder.
Operator was driving forklift and felt ill. Operator got off forklift and went down to ground and puked. Was not sure if due to something eaten or something else. Operator had been drinking fluids. Operator got a cut above left eye brow while on the ground puking.
Operator was cleaning material from floor from broken bags. Operator got cement/lime powder on leg and the hot weather caused sweat to move lime/cement down into boot. One leg had on short socks and did not stop sweat. Operator experienced a lime burn and needed treatment. Burn was not reported until 7-21-17 and then medical treatment followed.
Operator was handing bulk bags, when removing bags from hooks EE got hand caught between strap and bag. Was not following proper communication with forklift driver and other operator.
Operator was hand stacking tube sand and said EE hurt back/bulging disc while performing the job.

Operator was operating moffett forklift when they extended the forks to pickup a load. Employee then proceed to back up with the load on the extended forks off the trailer, the weight caused the forklift to tip forward due to the extended forks. Operator hit head and neck when machine tipped forward.
Miner was running the main line palletizer and felt a pain in EE's left shoulder. The miner could not reach above EE's head without feeling pain in shoulder.
Operator was in mixing room adding hand adds to batch system and then noticed swelling a couple days later at home. Accident was then not reported to plant until 2-13-17. Operator says suffered a hernia on right side.
EE was working on the main line packer and cleaned the feed spout that caused ee to cut hand. There is a cut on left thumb and looks like it needs stitches.
Dryer Operator was near end of shift and was sitting down and talking to replacement for transition. Dryer operator started to slur speech while talking and then proceeded to pass out. Appears EE might have had a stroke. When EE lost consciousness EE fell from chair and hit head. EE was unconscious on the floor and an ambulance was called to take EE to the hospital.
Control room operator was up on platform by main line packer, was stepping down from one platform to another, with one step and somehow got foot caught that caused EE to sprain right ankle.
Operator was outside picking up trash when EE reported they stepped on a nail that went through EE's boot. It was EE's left foot. Nail was probably from a broken wooden pallet.
Operator was walking down stairs and slipped on the 4th stair from bottom causing EE to fall, hitting EE's side in the rib cage area causing soreness. The stairs are grated, but still some minimal sand on steps could have aided to the slip.

Employee pinched thumb between strap on bulk bag and hook that holds bulk bag when second operator lowered pallet holding bulk bag. Operator suffered bruising and did not seek medical attention until 6:30 on 9/2/2015
On 5/5/2015 at approximately 8:00 am Linda Gentry notified plant management she had tripped going into the dispatch office and hit the front of her legs on the door threshold. She was sent to Concentra Medical for evaluation and then sent to St Anthony Medical Center for further evaluation. She was kept overnight for observation at St Anthony on 5/5/2015.
A production operator was bent down to pick up some tubes and hit his right knee on some empty pallets.

A bag of portland cement was travel up an incline belt on its side. Miner pushed bag with broom handle to position bag. Broom handle pierced bag and portland cement went around safety glasses and got in employee's right eye. Cornea of right eye is scratched.
Miner's left ring finger was pinned between a piece of equipment being moved and a stationary pole. A fracture of the finger resulted.
