Employee was performing maintenance and kneeled on the ground to perform work. When they stood, they felt a pop in their right knee. Employee had initial pain, but it went away. During the night after the shift, their knee began to hurt. The following day they reported the incident and were sent to the doctor to be checked out.

worker was performing a 2000 hr service on a dozer. When the worker was removing the reservoir bolts for the hydraulic ripper assembly the workers Knee slid in a small amount of grease. When the knee slid the worker reported feeling a pop and pain.

A large rock rolled away from the stacker discharge pile. The screening plant operator passed the discharge pile and struck the rock with the front right tire. The seat bottomed out and the employee suffered a lumbar strain and inflammation.

Employee was fueling a bulldozer and whilst dismounting slipped off the tracks and hit the ground. EE's head hit the bowl of an adjacent piece of equipment.

The Quarry operator was assigned to help the screening plant operator to clean up spillage in and around the screening plant. He was shoveling dirt into the bucket of the front end loader then he experienced pain in his lower back.

Employee is a scraper operator. For mining purpose, and respecting equipment design, scrapers are pushed out of the pit by dozers. Employee is complaining of neck pain due to this activity. Accident was declared on the 11/06/2015

Employee is quarry employee that operate most of the time a scrapper Employee informed his Doctor - While driving a scraper constant motion of neck and body due to rough roads neck area started to get really discomforting.

Employee was replacing the fan belt on the radial stacker of the screening plan. During the operation, he caught his right pant leg on a muffler clomp. He lost his balance and twisted his right knee when he landed.

Employee was operating a bulldozer in the mine and pushing a scraper. The bulldozer encountered a large rock and lost contact with the scraper. The bulldozer then slid off of the rock, and shift forward suddenly, hitting the scraper. This caused excessive force and resulted in a whiplash type of injury to both operators.

Employee was working on moving the screening plant from one section of the mine to another. He was lowering the auxiliary belt and pulled the pins. He was maneuvering the pins and pulling one while pushing on the other. As he pushed, he felt a sharp pain in his lower back. He was later diagnosed with a strained lower lumbar.

Miner was draining the wastewater from a truck/tanker. He was opening a ball valve when his right hand was caught between the frame of the tanker and the handle. This resulted in a fracture of his middle finger on his right hand.

Employee shoveling spillage under screening plant,as he bent down his hard hat fell off. Employee bent down picking hard hat up, stood up without putting hard hat back on,hitting head on cross beam, causing dizzy feeling. Dr wrote mild concussion/contusion.

A change in hearing sensitivity in 2012 has been evaluated by MSHA inspector as a reportable hearing lost during his 12/17/2014 inspection. However, according to our company Doctor, the lost is not reportable. In matter to close the citation 8786176, the hearing lost is reported in this form, but will be contested

Employee was tightening a bolt on a apron cylinder on a Cat 631 Scraper. This is a 5' cylinder. As the employee was pulling the wrench towards himself the 'bite' of the wrench slipped on the bolt, causing employee to fall backward about 5' onto the edge of an ecology (cement) block.

Employee Operating 631E Scraper approaching basalt stripping pad going uphill. Scraper hit a bump with one of the tires rocking machine causing employee to move sideways hitting head on door glass cracking glass.

Employee was operating 631E Scraper to load basalt rock. Person operating Dozer was pushing scraper when a large basalt rock caught under the 631S Scrapers bowl causing a jolt to machine injuring employee.

Scraper operator had filled bowl with basalt and started to pull away, as he was moving forward, the scraper slid on snow. Scraper operator slowed down and stopped abruptly as dozer operator approached from behind to push scraper, hitting stinger (push pad) on scraper causing injury to scraper operator.

Employee in 631E Scraper hauling crude from pit to 113 stockpile on level packed haul road that was recently watered.Employee applied retarder causing scraper to slide, causing trailer to come around to right.Machine continued to slide sideways until right edge of bowl caught on ground, causing scraper to tip on side.Factors:Recently watered road,speed and operational error.

Employee in 631E scraper was hauling crude from mining pit to crude pile in rotation with other scrapers, which were being pushed with a D-9N dozer. Accident occurred when employee had turned in seat to see where dozer was, when D-9N dozer made contact with the 631E scraper, causing neck strain injury to employee.

EE was Driving on Rough Terrain, Hauling Crude using a S2 - Caterpillar Scraper. The Scraper hit a bump and the Seat lost Air Pressure, causing EE to Jar his Back, resulting in a strain to the Back.

10/26/09 Employee opening jammed utility tool box. Box popped open forcibly causing his right arm/shoulder to fully extend, causing stabbing pain in top of shoulder. Employee did not want to go to doctor. Accident report turned in by working foreman on 12/18/09 saying employees shoulder wasn't getting any better and Gary would be going to doctor. Went to doctor on 12/24/09.

Employee was driving scraper up to topsoil pile and hit several ruts that he didn't notice. This jarred the scraper and strained employee's back. Employee was put on Light Duty for 5 duty and then returned to previous job as Truck Driver.

Accident report written2/5/08, employee believed injury was minor, did not go to Dr. Report kept on file if he wanted to see Dr. 4/1/08 He went to Dr., injury not getting better. Loading Scrapers w/Overburden, pushing w/cat. Cat hung up on Stinger Wear Patch, pressure increased, machines separated causing Scraper Operator to be jarred hard, causing strained neck.

Employee was replacing seals-bearings on Equalizer bar of Cat Dozer D9R. Space is limited; he crawled underneath the dozer using a hammer to beat out the seals. While trying to remove seals he hit his left hand three times with the hammer causing bruising - swelling of his left hand. *Note: employee has vacation Sept. 29-Oct. 5, 2005. Will return full capacity Oct. 6, 2005.

CONDITIONS WERE ICY AND MUDDY, EE WAS GETTING OUT OF BULLDOZER TO SPEAK WITH LEAD FOREMAN REGARDING MECHANICAL PROBLEM. AS HE WAS GETTING OUT HE SLIPPED AND FELL AGAINST BULLDOZER KNOCKING THE WIND OUT OF HIM AND HURTING RIBS.

LAST COMPANY PHYSICAL- DR. RECOMMENDED THAT EE GET HEARING AIDS.

MOVING RADIAL STACKER TO DIFFERENT LOCATION REQUIRES WHEELS MUST BE ROTATED. A CHAIN IS ATTACHED TO LIFT STACKER ON ONE SIDE SO WHEEL CAN BE ROTATED. AS THE STACKER WAS BEING LOWERED, EMPLOYEE HAD HOLD OF CHAIN TO DISENGAGE. THE STACKER STARTED TO ROLL SO CO-WORKER LIFTED STACKER CAUSING THE CHAIN TO TIGHTEN AND CATCH EMPLOYEE'S HAND BETWEEN CHAIN AND STACKER.

EE HAS BEEN EXPERIENCING PAIN IN ARM & SHOULDER & NUMBNESS WITH CARPAL TUNNEL SYNDROME IN HAND, WRIST & FINGERS. HE WAS DIAGNOSED WITH CARPAL TUNNEL IN MID APRIL. EE THOUGHT THE INJURY/ILLNESS WAS RELATED TO AN ACCIDENT WHICH OCCURED IN SEPT. 2002. HOWEVER, THE DR. TREATING EE DETERMINED THE PAINS WERE NOT RELATED.

EE WAS OPERATING SCRAPER, STRIPPING BASALT IN WEST PIT, OTHER EE WAS OPERATING PUSH CAT. AS EE IN SCRAPER WAS DROPPING BOWL, OTHER EE BUMPED INTO HIM HARD FROM BEHIND, CAUSING SCRAPER TO GO OVER A LARGE PIECE OF BASALT, JARRING EE A SECOND TIME, STRAINING HIS NECK AND BRUISING HIS RIBS AND HIPS.

WHILE REMOVING AN OPEN CONTAINER OF WELDING ROD FROM THE BED OF A SERVICE TRUCK, CUT LEFT HAND BETWEEN THUMB AND INDEX FINGER ON THE SHARP OPENING OF THE CONTAINER.

STARTED THE DIESEL ENGINE ON WATER TANKER, WHEN THE ENGINE STARTED, THE THROTTLE STUCK AND WAS OVER-REVVING THE ENGINE. TO SLOW THE ENGINE, I REACHED TO PULL BACK ON THE FOOT THROTTLE, AND CUT ONE FINGER ON A SHARP EDGE OF THE PEDAL.
