EE was climbing truck ladder to enter truck. EE had left foot on the first chain rung and was putting right foot onto the rung and both feet slipped off. EE stayed holding onto the handrails and didn't fall, but felt a pull and pain in the back of right shoulder.
Employee was welding a pipe to patch a hole. EE stopped to chip slag from the weld bead. EE raised the welding hood to chip and a piece of slag flew into EE's left eye.
Employee and coworker were replacing roller of conveyor belt. To remove roller, it had to be twisted and laid on its side due to belt tension. When the roller was lowered, employee was guiding with left hand and had right hand on the bottom of the belt. The roller was lowered and employee's right thumb was pinched.

Employee was loading pipes onto a trailer. EE noticed that one the pipes was sticking over the bed of the trailer and tried to push it onto the truck. EE heard a pop in their left shoulder, followed by a burn.
A concrete slab was being placed onto a structure by crane, and the employee was using a shovel to help guide it into place. As the crane operator set down the slab onto the structure, the slab broke apart. A piece of the slab struck the employee on EE's upper back knocking EE to the ground.

Employee was using a loader to dig out big rocks. One rock was halfway out of the ground and EE was digging and ramming it with the loader's bucket, when EE felt pinching/pulling in EE's back. As the day went on, EE felt pain getting progressively worse.
Employee was walking through weld/maintenance shop and walked by a pallet. A piece of cut metal banding/strapping was sticking out a few inches from the pallet and right foot came into contact with it and it punctured through the boot, sock, and the foot.

EE was helping to lift an empty water pipe, which was braced on a wooden block with a prybar between the pipe & block. As EE lifted the pipe, another employee kicked out the wooden block, causing the prybar to fall. EE stumbled and fell while trying to catch the falling prybar.

Employee was cutting a piece of wood with a table saw. The wood shifted suddenly and pushed the employee's finger into the saw.

Employee was breaking up frozen stone with a crowbar. The crowbar broke thru the stones and the employee struck finger on a metal plate attached to the frame.

Employee felt pain in his left hip and groin area as he was trying to dislodge a rock that was stuck in a chute. The pain grew worse as the day progressed, becoming significant after a few hours.
Employee was working inside the crusher underneath of the jaws. After exiting the crusher and shutting its door, he turned and slipped on the wet conveyor belt. He fell on the belt on his right side, injuring his right shoulder and arm.
Ice from a water pump hose broke loose and hit employee's right shin.
Employee was standing on an I-beam of a drill rig. He was working on the rig when his slipped and got wedged between the radiator and intake tube of the compressor. His was holding onto to something above his head, and when he slipped it wrenched his right arm and shoulder which were in an awkward position.

Employee was grinding a steel plate. Despite wearing safety glasses, the employee felt something hit him in the eye. He tried to flush the eye but it continued to be irritated. He had to get medical treatment. He suffered a corneal abrasion. He probably had something embedded which the flushing removed before he was seen by Dr.
Employee was loosening a screen's spring pad with a wrench. While using the wrench, he felt pain in his right elbow which got progressively worse.
Employee was jumping over a conduit ditch in order to help shovel concrete. He slipped and his right foot went into the ditch between the conduits.

Employee was stepping down from platform to lower platform on truck washing station. His boot got caught on the grate edge of the platform step and he lost his balance and fell onto the lower platform. When he fell, his left elbow jammed into his ribcage and he landed on his left side.
Employee was getting samples from a belt feed. When the employee stepped down from a platform onto a concrete slab, he stepped onto the conduit that runs across the slab and overturned his right ankle.

Employee was climbing down a ladder of a drill rig he was working on and scraped against the carrousel of the rig which caused a brush burn on his back. A few weeks later the abrasion became infected. The injury was initially denied by worker's comp and then the denial was overturned.
EE was laying on his back changing screens. About an hour later he was walking across the yard and felt severe pain in his lower back.
EE was placing chocks behind the wheels of his truck. When he bent down to place them, he felt a pain in his neck.

EE got out of his truck to check the tires because he thought one blew. He didn't see anything wrong, but when he was climbing the ladder to get in the cab, he felt a pain across his shoulders and in his neck. He thought it was a minor issue that would go away. On 12/21/11, he sought treatment because the pain had not resolved.
Employee was part of a crew moving water pipes. He was holding a pipe valve and another crew member was moving a different pipe when the employee's right pinky finger got pinched between the two pipes.
A team of employees was changing screens that are part of the crushing operation. Employee was taking a screen that was being handed to him from another employee and twisted and over extended his left wrist and arm.
Employee was getting a flashlight out of a loader cab to check the oil in another loader. As he was leaving the cab, he slipped on the wet steps and fell down the remaining steps, landing on his knees on the platform/deck of the loader.
Employee was getting ready to exit the crusher. He lifted a ladder through the chute to guide it to the platform below. The ladder slipped out of his hand bending his right thumb back.

Employee was loosening bolt on a bucket loader. He noticed the bolt slipping and reached under the bucket with his left hand to feel if the nut was moving. He cut his left middle finger on the nut.
Employee was working on a skid loader all day, while on his knees. The next day his knee swelled and stayed swollen for a week.

Employee was cleaning up around the crusher. Employee's hard hat blew off his head. It was a windy day and he had a sweatshirt hood on under his hard hat so it wasn't properly secured. The employee's hat rolled under a conveyor and when he retrieved it and was exiting from underneath the conveyor, he hit his head on the metal frame of the conveyor.

Employee was moving a large rock on a ramp by manually pushing it and felt pain in his lower back and leg, with numbness in his foot.
While lowering the hood of a truck, felt sharp pain in right shoulder.
Employee was using a hand drill on a steel plate of the beetle ground drill. The drill bit locked in the plate, causing the hand drill to be pulled forcefully out of his hand; then it came back and struck the top of the employee's hand.

Employee was on top of crusher, reaching down while leaning forward, guiding a lifting device of a crane. The crane was placing a piece of steel on the crusher.
Employee was closing the door on a crusher tub for removal and lost his footing on the step plate, and fell 3.5 ft. onto the walkway below. There was loose gravel all around due to a crusher breakdown.

Employee was on the back of a truck getting tools. The employee stepped over some cans of oil, lost his balance and fell, landing on casings and oil cans. The employee attributed strong winds to be a part of the cause for loss of balance.
