ROLLING ROCK PIT Metal/Non-Metal

While trimming stone with a masons hammer a piece of the hammer head chipped off and embedded into the inside of the employees left wrist. The Dr says it will need to be removed and surgery is scheduled on 10/7/2025. Employee is allowed to work until then with restrictions

While lifting up a stone tread off of a stack of treads to see the bottom side, the tread slipped and fell onto the employees left middle finger pinching it between two treads. Employee suffered a smashed open fracture/crushing injury to the finger with nail damage.

While driving in a new pin on an excavator track a piece broke off and entered the top of EE left forearm. EE went to the Dr. and said to have it removed surgically. EE missed 1 day of work, the day of surgery. Dr. let EE come back to work the day after surgery to be able to work at a level comfortable for EE. No other restrictions were given.

While grinding an Excavator Bucket for repairs, a spark went under employee's safety glasses into EE's left eye. Dr didn't find anything in eye just prescribed a prescription antibiotic eye drop.

While moving a large stone on a conveyor belt EE smashed right index finger between the stone and the conveyor frame. Laceration on top and bottom of finger with swelling. Urgent Care cleaned and inspected wound for any broken bones & none were found. Returned to work with no restrictions. EE was completely discharged from their care after a 2 day follow up.

EE Was in the Wood Shop making palletizing Boxes using a pneumatic nail gun. EE felt discomfort in right shoulder at the end of shift. EE reported the discomfort the following day when the discomfort didn't seem to get better. EE was then sent occupational health care where the Dr. described it as Strain of Muscle and Tendons of the right shoulder Rotator Cuff.

Employee felt discomfort in the top of right forearm while sorting and repacking stone into crates. The pain developed over time and was not one specific incident. The Dr. says there is a strain of the extensor muscle, Facia and Tendon at the Forearm level of the Right Arm.

EE was in Building #18 operating Saw #4. While loading a larger stone on the feed conveyer they tried pushing the stone up against the stone in front of it on the conveyor belt. EE then heard a pop in left chest and felt some pain. The Doctor says they fractured a rib on the left side.

EE was sorting stone and was carrying a stone back to the splitter for a recut. EE stumbled and fell into machine with stone in hand, EE's finger was smashed between stone and machine. EE's post Drug and Alcohol test came back inconclusive and sent for further testing. Test came back 5 days later Positive. EE was Terminated due to Positive Drug and Alcohol Test.

Employee was using a folding utility knife to trim a piece of rubber window molding when knife slipped and cut the top of EE's left pinky finger resulting in 9 external sutures.

While pulling stone scrap pieces from underneath picking conveyor with a push broom EE felt discomfort in right shoulder. This is one of the job tasks associated with assisting in the stone sorting operations along with lifting and placing stones from conveyor into pans. EE could not pin point the exact moment injury occurred. Pain had worsened over time.

EE woke up 11/19/20 with pain in neck & worked all day. EE reported issue to mgt. 11/23/20. EE did not recall any specific incident causing pain. EE was pushing/pulling slabs on gravity roller conveyor leading to hyd. splitter. EE indicated EE did nothing at home to cause injury. Dr. report 11/23/20 indicates sprain of ligaments & strain of other muscles @ left shoulder upper arm.

While packaging stone EE attempted to hand a co-worker a rock which slipped out of EE's hand and landed on EE's left foot directly behind steel toe. X-ray of Left foot revealed a fracture at base of Left 5th toe. EE did not seek medical attention until pain and swelling developed on morning of 11/4/20.

EE had an umbilical hernia prior to employment with Rolling Rock. EE has no idea what EE did to aggravate it. Hernia is larger and needs repair. Due to aggravation and enlargement it is work related and does not fall under short term disability.

EE was bending a piece of steel which was inserted into a vice. During the process of bending the steel, it slipped out of the vice and snapped back into EE's face.

EE was trying to move a stone into a loader bucket. All was ground level. No lifting involved. When ee gave a last push - ee felt a pop in lower back. EE sprained back.

Repetitive movement when handling stones. CTS - Carpal Tunnel Syndrome. Employee was on light duty since the date of injury until surgery date. Absence started effective 9/1/16.

Cutting, hammering & chiseling on sidewalk to place a stone ramp. When trying to stand up straight, lower back popped. Injury was not caused by a tool or another person, it was simply getting up from a low position.

Sweeping floor when employee felt a sharp burning pain in right groin area. NOTE: Due to annual company shutdown which stats 12/24/15 to 1/3/16 - work resulted 1/4/16, Jan 4th 2016 became the 8th day for reporting this injury which includes the date of injury.

EE was taking stone from bucket to palletize. Stone slipped out of hands and landed on left foot.

Trimming stone w/mason hammer (also driving truck at day of injury reporting) Right forearm, wrist, hand tendonitis with pain going up the elbow.

Employee was trimming stone with brick hammer when either a chip from hammer or stone penetrated clothing into upper right arm causing small puncture wound. Required X-ray exam & 2 sutures.

Employee was lifting and twisting in the normal process of stacking and sorting building stone when he noticed pain in his left groin area.

Picking a building stone from a hopper the top part of the stone broke free, employee lost his grip and the broken piece fell on left forearm causing a cut.

After completing a cut with hydraulic stone splitter employee pushed cut stone thru cutting head to off side conveyor & caught left forearm on sharp edge of stone. Equipment did not directly cause injury. Injury required couple sutures

Employee was hand trimming & packing stone at #3 saw conveyor at thin stone bldg. Employee experienced pain in right elbow running down to middle two fingers. Injury was originally reported on 8/17/10 & employee continued to work until 2/28/11. At this time due to current Dr's restrictions, employee is not working and is awaiting elbow surgery scheduled on 3/17/11.

EE reported that he nailed 2 pieces of wood together w/nail gun. He laid down the gun on his right side & proceeded to pick up a piece of wood when he claims the gun fired. He claims he was not touching the gun & it was 18" away when it misfired & shot him through the top of left hand going completely through but remaining exposed on both sides. Accident under investigation.

Stepping off loader he overtread his left ankle.

EE came into office at end of the day complaining about being hot and said that he needed a break. EE arrived at work on 06/11/08 and reported not feeling well, after some observation we decided to call for an ambulance.

Trimming stone with trimming hammer. Handle of trimming hammer hit left thumb causing a cut of the thumb.

A stone fell from the bucket and EE stepped to the side to avoid it and sprained his right ankle.

EE was torquing down the lug nuts on a truck when torque wrench ratchet slipped. Wrench came down on leg & broke leg/ankle.

EE came into office on 3/6/06 to say he felt pain in his left knee while crating sone on 3/3/06 at 3:30 pm.

Employee was handling stones in a wooden crate when he caught a sharp edge and cut his right hand between the index finger and thumb. Injury required 6 sutures. No lost time.

Moving a flagstone tread and pinched 2 fingers.

EE WAS PICKING STONE OFF CONVEYOR BELT AT THE STONE SPLITTER. AS HE TURNED TO LOSS STONE IN A HOPPER ANOTHER STONE FELL OFF THE CONVEYOR ON EE RIGHT FOOT. STONE PINCHED EE S RT SMALL TOE DIREC TLY BEHIND STEEL TOE.

EE WAS PICKING A STONE FROM A CONVEYOR INTO A HOPPER WHEN HE PULLED HIS LITTLE FINGER ON LEFT HAND BETWEEN THE STONE AND A SAFETY HANDRAIL. POST ACCIDENT DRUG TEST REVEALED POSITIVE RESULT . EE WAS TERMINATED AS A RESULT OF DRUG TEST.

THE EMPLOYEE PINCHED HIS LEFT PINKY FINGER BETWEEN A ROLLER AND A STONE ON A GRAVITY ROLLED CONVEYOR. A CO-WORKER WAS PUSHING THE STONE ON THE GRAVITY FEED CONVEYOR AT THE TIME OF THE INCIDENT AND THE INJURED EMPLOYEE WAS NOT ABLE TO REMOVE HIS HAND IN TIME.

REOCCURRANCE OF PAIN ON 3/25/03. DOCTOR HAS HIM OFF WORK UNTIL HE SEES HIM ON 4/1/03.

EMPLOYEE WAS USING A SEARS BRAND RATCHET WRENCH. THE RATCHET SWIVEL SOCKET BROKE AND WHEN IT BROKE THE EMPLOYEE HIS HIS ELBOW ON A PART OF A DUMP TRAILER.

ROCK FELL OFF CONVEYOR AND HIT HIM ON LEFT KNEE.

TWISTED RIGHT KNEE GETTING OFF LOADER.

TWISTED BACK WHILE PUTTING STONE INTO BASKET.

LABORER FELL & HIT LET HIP AFTER SIGNING IN AT TIME CLOCK LOCATED IN SCALE HOUSE.

LABORER WAS BENDING OVER TO PICK UP A CHAIN TO ATTACH TO A CONVEYOR WHEN HE TWISTED AND POPPED HIS KNEE.

EMPLOYEE SPLITTING SANDSTONE, CHIPS FROM STONE HIT LEFT FOREARM, CAUSING LACERATION OF FOREARM . 2 SUTURES WERE REQUIRED.

STEPPED OFF A STONE PICKING CONVEYER. WHEN STEPPING OFF, STEPPED ONTO A BOULDER AND LOST HIS BALANCE.

EMPLOYE HEATING UP A TRANSMISSION MOUNT BOLT. RUBBER MOUNTING CUSION MELTED DOWN FROM TRANSMISSION ON EMPLOYEES LEFT HAND.

REOCCURRANCE OF NECK STRAIN (ORIGINAL NECK STRAIN REPORTED ON ACCIDENT REPORT #17 1-9-98).

DURING HORMAL HANDLING 9F STONE, EE CUT HIS WRIST OPN A SHARP PIECE OF STONE WHEN TOSSING STONE INTO WASTE BIN. (SUTURES REQUIRED)

EMPLOYEE WAS CUTTING A STONE WITH A SLEDGE HAMMER WHEN A PIECE OF THE HAMMER BROKE OFF. THE STRIKING PART OF THE HAMMER ENTERED THE FOREARM OF THE EMPLOYEE. A PUNCTURE WOUND WAS CAUSED TO FORE ARM BY PENETRATION OF METAL FRAGMENT.

THE DRIVER WAS EXITING HIS TRUCK AND MISSED HIS STEP & FELL TO GROUND FROM CAB OF TRUCK. THE INCIDENT OCCURRED ON PUBLIC HIGHWAY IN FRONT OF THECOMPANY OFFICE

EE WAS ROLLING A STONE INTO POSITION ON TOP OF ADUMP TRUCK TO SECURE LOOSE LOAD OF STONE

PICKED UP STONE TO PUT INTO LOADER BUCKET.

EMPLOYEE WAS PICKING STONE OFF THE PICKING CONVEYOR. HE TURNED TO THROW A STONE INTO AN ADJACENT TRUCK BODY AND IN THE PROCESS BUMPED HIS HEAD INTO AN OVERHEAD BRACE WHICH SUPPORTS THE ROOF OV ER TOP OF THE CONVEYOR. THE EMPLOYEE WAS WEARINGA HARD HAT AT TIME.

CONDITIONS UNKNOWN EE INDICATED THAT HE NOTICED PAIN IN RIGHT SHOULDER AS HE WAS STEERING TRUCK WHILE BACKING TO STOCK PILE. HE WAS NOT AWARE OF A SPECIFIC INJURY THAT CAUSED THE PAIN.

EE WAS PICKING STONE OFF CONVEYOR BELT. AS HE PICKED A STONE OFF THE BELT AND THREW STONE ONTO TRUCK BEHIND HIM HE FELT INSTANT PAIN LEFT SIDE OF NECK.

EMP WAS LIFTING A HEAVY STONE. WHENHE HAD THE STONE ELEVATED TO WAIST LEVEL HE TWISTED TO THROW STONE AND HE GOT PAIN IN LOWER LEFT SIDE OF BACKAREA.

HERNIA OF UNKNOWN CAUSE WAS FOUND WHEN EMPLOYEE WENT TO DR. FOR EMPLOYEE PHYSICIAL. IT WAS JUST RECENTLY DETERMINED THAT IT WAS A WORK RELATED INJURY, ALTHOUGH THE EMPLOYEE HAD NO KNOWLEDGE OF WHEN THE INJURY MAY HAVE OCCURRED. THIS DETERMINATION IS PURELY OF SPECULATION SINCE NO ACCIDENT OR INJURY WAS REPORTED BY EMPLOYEE.

EMP IS NOT SURE IT HAPPENED AT WORK. BREATHING WAS A PROBLEM AND WENT TO EMERGENCY ROOM AT HOSPITAL. HOSPITAL DIAGNOSED AS A PULLED MUSCLE.

FELLOW EE WAS CUTTING AND TRIMMING STONE WITH STONE MASON HAMMER, WHEN A STONE CHIP FLEW AND LACERATED EE'S LEFT SIDE OF FACE.

PATIENT NOT SURE IF CAUSE DUE TO WORK RELATED INJURY OR CONDITION OR NOT. SHE CLAIMS SHE EXPERIENCED PAIN AFTER MOVING A PORTABLE PRESSURE WASHING UNIT.

STARTED AT 1100 AM EE WAS HAND THROWING STONES OFF CONVEYOR BELT. IN THE PROCESS OF THROWING A STONE OFF BELT HE PINCHED HIS LEFT PINKY FINGER BETWEEN THE STONE AND PART OF THE CONVEYOR FRAME.

EE THINK INJURY MAY HAVE OCCURED WHILE THROWING STONES OFF A CONVEYOR BELT THERE WAS NO INJURY CAUSING ANY IMMEDICATE PAIN OR DISCOMFORT. DR HAD NOT DECIDED IF PAIN OR INJURY IS WORK RELATED O R NOT.''

NO DIRECT INJURY CAN BE AHRIBUTED TO PAIN & DISCOMFORT EXPERIENCED BY DOC HAS NOT DECIDED AT TIME OF THIS REPORT IF THE PAIN OR INJURY IS WORK RELATED OR NOT

EMPLOYEE WAS RAISING DUMP BODY ON TRUCK TO UNLOAD STONE WHEN HYDRAULIC CYLINDER BROKE EJECTING PART OF THE LIFT CYLINDER INTO REAR OF TRUCK CAB, CAUSING WINDOW TO BREAK AND GLASS CUTTING HIS R IGHT SHOULDER.

EMPLOYEE WAS MANUALLY TURNING A SONE WHEN IT FELL AGAINST HIS RIGHT FOOT AND LEG CAUSING THE KNEE TO BE SPRAINED.

EMPLOYEE WAS SPLITTING STONES WITH SPLITTING HAMMER WHEN THE HANDLE BROKE. FELT SOME PAIN BUT DIDN'T THINK ANYTHING OF IT. NEXT DAY LEFT HAND STARTED TO SWELL.

EMPLOYEE WAS HANDLING STONES WHICH INVOLVED PHYSICALLY TURNING, LIFTING AND MOVING OF INDIV IDUAL PIECES. ONE OF THESE PIECES DURING ITS MOVEMENT CAUSED A STRAIN TO THE EMPLOYEES UPPER BACK AREA. AS OF THE DATE OF THIS REPORT IT IS UNSURE WHEN EMPLOYEE WILL RETURN TO WORK.
