Employee was roof bolting when the sleeve of his coveralls got caught on drill steel thus wrapping right arm around drill steel.

While performing his normal duties as a Roof Bolter/MBC Operator - employee experienced pain in right hand. Employee continued working until October 14, 2010 at which time he started missing work making this reportable. This document supersedes #1305920.

Employee was performing his normal duties as a belt cleaner when he experienced pain in right forearm. Employee continued working until August 12, 2010, at which time he went for a medical evaluation and was diagnosed as tendenitis and has since started missing work, now making this reportable.

Employee was removing a block wall that was used to store mud behind it in order to be dried and later removed from the mine via Scoop. As he started to remove blocks some mud came loose thus pushing employee's leg against the blade of the Scoop.

Employee was installing pin on a bridge when he caught finger under the pin. Employee did not miss any work however he did go to a medical facility to get an x-ray done and it was determined that it was fractured therefore making this accident a reportable.

Employee jumped out of the bed of the company pick up truck near the mine office at the completion of his shift when he experienced pain in right knee. Employee continued working until January 7, 2010 at which time he started missing work making this accident reportable.

Employee was pulling rock down with bar when he experienced pain in right shoulder. Employee continued working until he was placed on restricted duty effective 08/19/2009 now making this reportable.

Employee was attempting to pull a piece of rock off of the continuous miner when his hand slipped and he caught his left wrist on a sharp edge of the rock sustaining a laceration. He did not miss any work however he did receive eight (8) sutures thus making this report a medical.

Employee was roof bolting, when a piece of rock dislodged from the roof, striking employee on the back of the head, neck and between shoulders.

Employee was pulling plastic water line when he lost his footing on uneven bottom slipping and landing chest first on bottom.

While employee was attempting to use the drill head of the roof bolter to move coal laying in the bottom in order to obtain clearance for the second set of drill steels, the canopy swung, thus catching the webbing of his left hand between the canopy and drill steels that he was holding on to.

Employee was tramming continuous miner to the right side of cut when he caught his foot under pan of miner.

An unplanned roof fall occurred in the #3 belt entry of Southwest Mains just outby the face. Fall was approximately 7' high, 18' wide and 45' long. The affected area was bolted with 6' tension roof bolts according to the approved roof control plan. Did not affect any ventilation or equipment. Fall will be cleaned up.

Employee was removing rock from the #1 bridge when he experienced pain in his lower back. Employee continued working until December 3, 2007 at which time he started missing work due to this injury, therefore, now making this a lost time accident.

While employee was exiting the steps of the IT 28 (forklift) he inadvertently stepped on a piece of rock with his heel permitting the right ankle to twist. Employee did not miss any work however he was placed on restricted duty effective May 1, 2007 thus making this a reportable.

While employee was performing his normal duties as a beltman/mine examiner, he experienced numbness in both hands - particularly the left hand. Employee continued to work until June 4, 2007 at which time he started missing work to have surgery done.

Employee was pulling MBC cable, when he experienced pain in lower back.

While the employee was hanging the bolter cable the bolter was being trammed and the cable apparently got caught in the reel thus pulling the cable tight and caught his left leg twisting the left knee.

While employee was cleaning nut coal belt he slipped on ice thus experiencing pain in left shoulder, back and neck. Employee has continued working however on 03/01/2007 he started receiving physical therapy thus making this now a medical report.

Employee was shoveling belt from the kneeling position and as he stood back up he apparently stepped on irregular bottom when he felt his left knee pop. Employee continued working until 02/27/2007 at which time he went off to have surgery performed.

Employee was riding on a personnel carrier directly behind the operators compartment when apparently his cap lamp cord caught a roof bolt thus pulling his hard hat off his head. As he went to duck down his left shoulder struck the roof and rolled his body back over the fender of the personnel carrier. Employee did not miss any work however he had been placed on restricted duty.

The working supervisor was observing the Continuous Miner Operator in the No. 2 entry, when a piece of rock dislodged between the roof bolt and rib line, striking employee on the head forcing his hard hat off thus causing a head laceration and jamming neck. Employee required staples to close laceration.

Employee had arrived in x-cut #3 to #4 just as loading crew was backing out of cut when he was struck by bridge.

Employee was setting 8' timber-while standing on cribs to reach roof the cribs went out from under him causing him to fall landing on right shoulder. He continued working until 1/16/06 at which time this became a lost time accident.

While employee was walking by the #1 bridge a piece of rock fell off striking employee's left foot.

EMPLOYEE WAS USING A PRY BAR TO ASSIST IN CHANGING OUT A ROLLER ON THE STACKER BELT WHEN BAR SLIPPED STRIKING HIM ON THE RIGHT FOOT

WHILE EE WAS CUTTING CLIP OFF BELT SPLICE TO PULL PIN, THE KNIFE SLIPPED OFF, LACERATING HIS LEFT LEG. EE DID NOT MISS ANY WORK. HOWEVER, HE DID RECEIVE SUTURES, THUS MAKING THIS REPORT A MEDICAL.

EMPLOYEE WAS CHANGING OUT MINER BITS WHEN A SPRING CLIP THAT ATTACHES THE BIT TO THE BLOCK SPRANG BACK, STRIKING THE EMPLOYEE IN LEFT EYE. EE WENT TO THE HOSPITAL & WAS TREATED FOR A CORNEA ABRASION THUS MAKING THIS A MEDICAL.

WHILE DROPPING THE HEAD ON THE ROOF BOLTER, THE HEAD CAUGHT ON THE KNEE PAD HYPEREXTENDING THE LEFT KNEE. EMPLOYEE CONTINUED WORKING UNTIL 10/8 AT WHICH TIME HE STARTED MISSING WORK MAKING THIS A LOST TIME ACCIDENT.

EMployee dropped the roof bolter head down on his left knee.

While ee was drilling with a roof bolter he experienced pain in shoulder area.

WHILE EE WAS WALKING IN MUDDY CONDTIONS DOING MINE EXAMINATIONS, EE EXPERIENCED PAIN IN BACK.

WHILE OPERATING MBC EMPLOYEE TURNED TO LOOK BACK WHEN HE EXPERIENCED PAIN IN NECK.

WHILE EE WAS REMOVING ROCK OFF OF CABLE TENDER, HE CAUGHT LITTLE FINGER BETWEEN THE ROCK AND CABLE TENDER.

Bending roof bolts, EE had pain in shoulder.

EMPLOYEE WAS ROOF BOLTING WHEN WRENCH SLIPPED OFF ROOF BOLT STRIKING HIM IN THE MOUTH. HE RECEIVED A LACERATION THAT REQUIRED STITCHES MAKING THIS REPORT A MEDICAL.
