THREE MEN WERE BEGINNING TO INSTALL A ROLL OF CONVEYOR BELTING WHICH WAS SUSPENDED FROM A CRANE FOR UNROLLING. THE CHAIN SUPPORTING THE ROLL SLIPPED THROUGH A CLEVIS ALLOWING THE CHAIN TO SLID E OFF A PIPE THROUGH THE CENTER OF THE ROLL. THE ROLL FELL STRIKING THE VICTIM ON THE HEAD. THE SEVERITY OF THE ACCIDENT WAS INCREASED BY THE FACT THAT THE ROLL WAS HOISTED HIGHER THAN NECESS

While standing, individual attempted to lift a 4x8' sheet of plywood from chest height to overhead position. Plywood had dirt on it making it heavier than anticipated. Extra exertion caused a pulled muscle under left scapula around to under left pectoral muscle. Co-worker immediately checked individual for any sign of back injury before returning to the office to report incident.

Employee was checking battery cable connections on track hoe that would not start. EE lifted the battery box lid with EE's right hand. the box lid slammed shut onto EE's left thumb, just below the thumb nail.

Contractor claims that a 300 pound steel sheet was being lowered with a crane. Got hung up and then dropped a foot while EE was holding it, caused pain in EE's left shoulder because EE caught the sheet.
Employee shoveling under #2 crusher system. Employee alleges EE hit a bolt head or solid object with the shovel and the shovel stopped abruptly and EE's right shoulder popped.

Employee was bending over a table/workbench pulling a large gear toward self. Employee felt pain in lower back and leg. Pain increased with time. The employee used a sledge hammer after feeling pain.
Employee was descending dozer. EE stepped from dozer to stepladder. The ladder went sideways. Employee tried to catch self by grabbing hose on the dozer's lift cylinder, but EE's hand slipped, causing EE to hit the left side of head against the dozer filter and fell to the ground.

6'1", 430lb employee was performing morning mobile equipment servicing duties. He was dismounting passenger-side ladder of 988 endloader. As he applied his weight to the last rung, the ladder gave way. He did not fall. He was able to maintain 3 pt contact w/left leg extended. Emp. claimed he heard a "pop" in his left knee. Medical exam determined sprain/tissue injury to left knee.

EE preparing to air tires on another co. vehicle. F350 was parked on 3-5% grade w/parking brake set & trans. in neutral. Employee was out of his truck starting to air tires, he saw his F350 rolling backwards. He attempted to get in his F350 to stop the rolling, fell under. Front drivers' s side tire rolled over his left leg. Fractured left femur. Minor cut to back of head.
Employee was changing hydraulic cylinder on the quarry dump truck. As the cylinder was remove, it caught the employees right pinky finger between cylinder and the truck's fuel tank.
Employee was welding (maintenance) @ riprap plant when experienced irritation in his rt eye. Employee went to exam eye, flushed it with saline solution, found small particle on cornea. After an unsuccessful attempt at removing the particle, management sent employee to an eye care professional for removal. The particle was small piece of rust embedded in the cornea.

Empl.was using grinder to clean pin from the brakes of a quarry machine. His right index finger was pulled into the wire wheel causing an abrasion. The outer layer of skin was removed for 2/3 the dia. of the finger from the tip to primary knuckle. The abrasion was approx 2.5 in. After first aid, the empl.refused medical treatment,returned to work. No gloves or holding tool.

Employee had a lit cigarette in his mouth. He was wearing a work vest. He was attempting to stop a slowly rolling pickup truck. Upon entering the truck, the vest flipped the burning cigarette up into his right eye.
Employee was exiting #3 loadout system, going down stairs, slipped on step and hit left shoulder on railing.

Employee was stepping down from an upper roll top barge lid to a lower barge lid approximately 14". He slipped and sprained right ankle. The injury was the result of the employee not being adequately aware of his environment and incorrect foot placement. No equipment was involved, employee was wearing required PPE. Employee was familiar with job being performed.
Employee phoned in 5/1/06 a.m. with eye problem. He indicated that he may have gotten something in it Thurs. (4/27/06) or Fri. (4/28/06) or may have "pink eye". He was told to go to a clinic. He was not able to see a Doctor until 5/2/06. Diagnosis - inflamation of eyelid.

EE WAS TRYING TO LOWER A CONVEYOR BELT BY CUTTING THE SUPPORT BEAM OFF, SUPPORTING THIS BEAM WITH A PROP & THEN REMOVING THE PROP TO ALLOW THE SUPPORT BEAM TO FALL 6' TO THE GROUND. AS HE WAS REMOVING THE PROP, THE BEAM & PROP BOTH FELL TO THE GROUND & ONE OF THEM STRUCK HIM IN THE LEFT HIP AREA.

EMPLOYEE WAS LYING IN A PRONE POSITION WORKING ON REPLACING TWO SIZING SCREENS WHEN HIS LEFT FOOT BECAME CAUGHT BETWEEN TWO SCREEN SUPPORTS. IN THE PROCESS OF TRYIN GTO FREE HIS BOOT, HE TWI STIGED HIS LEFT KNEE OUT OF JOINT. EMPLOYEE HAD SURGERY TO REPAIR LEFT KNEE IN 1990 AND STATED THA KNEE HAS DISLOCATED SEVERAL TIMES SINCE AT HOME.

WHILE SPREADING WATER ON ROADS, EE ENTRED A ONE WAY ROAD TRAVELING THE WRONG DIRECTION AND MET AHAUL TRUCK. IN TURNING TO THE RIGHT, THE LOADER BUCKET STRUCK A TREE AND TRAVELED UP THE TREE CAUSING THE LOADER TO TOP ON ITS LEFT SIDE
