Simplot Mill & Pit #1 Metal/Non-Metal

Employee walked up five steps to take a sample from a belt and once the employee had the sample they turned around and walked back down the steps reaching the bottom step, they reported that they stepped bad and their right knee began to hurt.

Employee was ascending stairs at load out and knee popped. The employee did not report this until 04/19/2024. When the employee reported this, EE did not want to go to the doctor and was just sore. On Monday 04/22/2024 EE requested to see a doctor and was put on restricted duty.

Employee was using a 1 inch impact to remove bolts from a sprocket segment on a D11 dozer. EE stood and stretched after removing the bolts. EE then felt EE's back crack, causing a strain in EE's upper back. EE was treated with First Aid measures until 8/29/2022 when EE was prescribed physical therapy.

Employee was changing valve positions at a booster pump station when he stepped in a hole and twisted his ankle.

Employee was removing a tire from a backhoe, his pry bar broke and he fell down and twisted his left ankle.

Employee was cleaning the firebox at the dryer and had his fire boots on. Once cleaning was completed and employee removed his fire protective boots he noticed a blister on bottom of right foot.

Employee was using a hand-held 9" grinder to reduce thickness of a metal plate. During the activity, the grinder "kicked" back, w/ the rotating wheel striking the employee on top of the left knee. Employee suffered a laceration/cut approximately 3" long and approximately 0.25" deep. Employee was taken to Urgent Care facility where wound was treated and closed w/ seven stitches.

Working with another EE on pump. EE placed a bar to hold sheave and the other EE asked him to move the bar so he could check the threads on the shaft. When the other EE tried to screw on the impeller the EE had not replaced the bar and tried to hold the sheave by the belts. EE's fingers were caught between the drive belt and sheave.

Dark, windy night. Employee was washing sand from dryer pad with pressure hose. Hose became kinked and the employee tried to move hose around to get the kink out. Hose was full of pressure and he lost control. The nozzle portion came up and struck employee in the face.

EE slipped on dirt berm with loose rock and lost footing. Normal stairs with handrails used to connect mill floor level to water truck access level were not used; Concrete on mill floor was wet. EE perceived that water on concrete would create a greater hazard.

Employee was running to open by pass gate on dryer discharge conveyor and tripped on a pile of sand.

A vendor truck driver was using the hatch opening station to open the hatches on his trailer when the access platform failed and the truck driver fell to the ground. The truck driver received a laceration the right side of his scalp. The fall protection unit was compromised earlier in the shift by another vendor truck driver.

Unknown. EE finished his shift, went home and later found both hands swollen. EE was fine and in no pain while at work. EE had been painting and cleaning parts with WD-40 while at work.

Employee went to check water in radiator. Hot water spewed out, burning him.

While removing caustic line that goes to the seal tank some caustic got into his boot. While removing protective equipment EE failed to wash his gloves before removing his boots or suit & goggles. Caused chemical burn on right foot. EE told supervisor @ 8:00PM that this happened. Super visor told him to remove boot.EE told supervisor not a problem only small amount.

EMPLOYEE WAS WALKING BACK TO LOADER WHEN MAINTENANCE EMPLOYEE ASKED HIM A QUESTION. EMPLOYEE TURNED TO ANSWER WHEN PORTABLE LIGHT PLANT FELL ONTO HIS NECK/SHOULDER. EMPLOYEE WORKED A FULL DAY ON 3/17/06 AND 6 1/2 HRS. ON 3/18/06 BEFORE GOING TO DOCTOR.

Employee had hood up on company truck to check engine oil when shocks failed and hood fell onto employee's back. Employee worked his regular shift on 2-8-06 and 2-9-06. He worked 3 hours on 2-10-06 and then went to the doctor.

EMPLOYEE WAS BACKING DOWN WASTE DUMP ROAD AND HIT SOME BUMPS IN THE ROAD. (We took him to the doctor and did find out he had some sort of disc problem. Classify this as an occupational illness.)

Employee was operating loader when his eye became irritated.

Ee was dumping a load of waste & on his way back to pick up a load of sand he hit a pot hole in road. Ee originally went to Dr on 9/15/05 for exam on this injury & since no observations were made, we considered this as diagnostic only - no lost time or medical treatment. This is a reoccurance of the same injury. EE is now losing time as of 3-13-06 for this injury.

While operating loader, employee complained of numbness in leg and arm because the seat in the loader was bouncing too much.

WHILE SWINGING SNOUT ON LOADOUT THE GUARD AROUND SPROCKET CAUGHT ON THE CHAIN DRIVE, WHEN IT CAME LOOSE, IT SLAPPED HIM ON THE SIDE OF FACE

Employee was servicing equipment, while walking across the yard he stepped in a hole and twisted his ankle.

EE WAS OPERATING LOADER. SAND PILES ON ROAD. ALSO SAID HE HIT HIS HEAD WHEN HE WENT TO GET OUT OF THE CAB.

EE was draining oil from equipment. When he removed oil plug, hot oil, sprayed onto his face causing oil to go into eyes.

WHILE ATTEMPTING TO REPLACE THE DRIVE BELT ON THE ASH SCREW BY ROLLING THE NEW BELTS, EMPLOYEE'S FINGERS GOT CAUGHT BETWEEN THE BELTS & THE SHEAVE CUTTING HIS MIDDLE & RING FINGER ON RIGHT HAND.

On 3-16-04 while climbing onto a D9L dozer to service machine, the ee alleged that he pulled a muscle in his groin. This was later diagnosed as a hernia on 3-19-04. EE has not returned to work as of this report. LAST DAY WORKED WAS 3-23-04.

EE was operating a 988 cat loader. The seat was loose causing him to rock back and forth. he injured his back. EE went to see the Dr on 3-9-04. he continued to work until 3-19-04. On 3-22-04 he was taken to the hospital and has not returned as of this report.

WHILE SHOVELING OUT FIREBOX, EMPLOYEE SWUNG BACK TO GET ANOTHER SHOVELFULL WHEN HE HIT THE SIDE OF THE FIREBOX ON A SHARP EDGE CUTTING HIS LEFT HAND.

EE WAS REPLACING STARTER IN HAUL TRUCK. THOUGHT HE HAD BOLT STARTED WHEN STARTER FELL AND SMASHED HIS FINGER.

EMPLOYEE WAS WORKING A LOT OF THE TIME IN STANDING WATER AND DEEP MUD. AFTER SEVERAL DAYS OF THIS, HE DEVELOPED PAIN IN HIS RIGHT HIP. IT BECAME WORSE OVER THE WEEKEND.

WORKING ON ROD DECK COUNTER WEIGHT CROSS MEMBER HAD SHARP EDGE AND CUT LEFT WRIST.

EMPLOYEE PINCHED RIGHT HAND WHILE WORKING ON FORK ON FORKLIFT MAST.

EMPLOYEE WAS CHANGING COVERALLS AND HIS HARD HATOFF WHEN HE STOOD UP HE HIT HIS HEAD ON TRACK ADJUSTER.

WIND WAS BLOWING AND BLEW DUST IN EYE.

EE HAD FINISHED LOADING RAILCAR AND WAS CLIMBING DOWN LADDER FROMT HE PLATFORM OVER THE LOADOUT BELT. HE WAS ON THE RUNG ABOVE THE CATWALK WHEN HE FAINTED. EE DOES NOT REMEMBER LAYING ON CATWA LK. EE STARTED VACATION ON 7-13-00.

LIFTING A GEAR BOX FROM THE GROUND TO A PICK-UP BED.

EE WAS CUTTING SCRAP STEEL. A PIECE OF STEEL PLATE THAT HE HAD CUT LOOSE FELL ONTO HIS RIGHT FOOT.

EE WAS CHANGING SWITCH ON RAILROAD TRACKS. HE STOPPED OVER TO RAISE HANDLE ON SWITCH AND AS HE WAS MOVING SWITCH HIS BACK STARTED TO HURT.

EE WAS STEAM CLEANING CRANE AND A COVER WAS LEANING UP AGAINST CRANE, WHEN HE TOUCHED COVER, THE BOTTOM WAS HINGED AND FELL ON HIS LEFT SHIN.

DESCENDING LADDER OF 988 LOADER AND STEPPED DOWN ON ENEVEN GROUND.

SERVICING EQUIPMENT EE STEPPED OFF A 988B CAT LOADER ONTO A MEDIUM SIZE LOOSE ROCK AND HIS FOOT SLIPPED OUTWARD. HE TRIED TO BRING LEG UP AND FELT A POP AND BURNING PAIN IN CALF AND KNEE.

ASSISTING IN REPAIRS WHEN A 2" HOSE RELEASED FROM HOOK ABOVE AND FELL ON HAND. THIS WAS HER LAST DAY AT WORK.

EMPLOYEE WAS SWINGING A HAMMER AND INJURED HIS RIGHT ELBOW. INJURY NOT CLASSIFIED.

EMPLOYEE WAS REMOVING THE INNER RACE OF TRUNION BEARING FORM THE TRUNNION SHAFT. HE HAD SPLIT THE RACE WITH A TORCH AND WAS USING A 3LB HAMMER TO KNOCK THE RACE LOOSE FROM THE SHAFT. A FRAGMEN T BROKE OFF THE RACE AND STRUCK HIS RIGHT LEG.

EE WAS OPENING THE TOP DOORS ON RAILCARS. THE CUFF ON HIS GLOVE CAUGHT ON THE EDGE OF THE DOOR AS IT WAS WALLING OPEN. THE PALM OF HIS HAND WAS PINCED BETWEEN THE DOOR & THE TOP OF THE CAR.

EE WAS HELPING TO SPIKE A RAILROAD TRACK. A SPIKE FLEW BACK AND HIT BETWEEN THE EYES.

EMPLOYEE WAS WASHING RAILCARS. HE CLIMBED DOWN INSIDE CAR TO RINSE OFF THE ROOF. HIS FOOT SLIPPED OFF THE RUNG OF THE LADDER AND HE STRETCHED HIS SHOULDER MUSCLE WHEN HE HELD ON WITH ONE ARM.

CLOSING TOP DOOR ON RAIL CAR, DOOR BOWED UP AND WOULD NOT CLOSE. STRAINED LOWER BACK.

WHILE MOVING DRISCO PIPE FROM #26 PICK UP TRUCK TO #23 CREW CAB PICK UP TRUCK. HE STRAINED HIS LOWER BACK.

INJURED WAS OPENING A RAILROAD CAR DOOR TO CLEANINSIDE OF CAR. THE BAR THAT WAS IN THE KNUCKLE ON DOOR SLIPPED OUT OF KNUCKLE, PINCHING THUMB BETWEEN BAR AND RAILROAD TIE.

LIFTING DRISCO PIPE. THEY WRAPPED A CHAIN AROUND PIPE TO SET IT IN FUSING MACHINE.

HE WAS WORKING UNDER A HOPPER WHILE LACING A FEED BELT, HE HIT HIS HARD HAT ON A BEAM, JAMMING HIS NECK.

EE & EE WERE CHANGING A TRACK ROLLER ON A D9 DOZER EE WAS ON H IS KNEES UNDER THE DOZER LIFTING ON ONE END OF THE ROLLER TO MOVE IT INTO POSITION WHEN HE HURT HIS BACK

HE WAS PERFORMING NORMAL MAINTENANCE, HANDLING METAL AND WORKING ON A SHORT BELT. HE GOT A PIECE OF METAL IN HIS EYE.

HE WAS GETTING OUT OF A HAUL TRUCK & HIS HARD HAT HIT THE TOP OF THE DOOR FRAME & JAMMED HIS NECK DOWN

HE WAS USING A 6" GRINDER TO GRIND A WELD OFF OF AN "I" BEAM. THE GRINDING WHEEL CAUGHT AND TWISTED THE GRINDER OUT OF HIS ELFT HAND. THE WHEEL CUT THE UNDER SIDE OF HIS LEFT WRIST.

HE WAS GREASING A 988 LOADER. HE STEPPED FROM THE BUCKET TO THE LEFT FRONT TIRE. HIS FOOT SLIPPED OFF THE EDGE OF THE TIRE. WHEN HE FELL HIS BACK HIT THE TIRE.

HE HAD THE GREASE HOSE ON THE LUBE TRUCK PULLED OUT GREASING A LOADER. THE CATCH ON THE HOSE REEL CAME LOOSE WINDING THE HOSE UP. IT GAVE HIS RIGHT ARM A JERK, TWISTING HIS ELBOW.

HE WAS INSIDE OF A 10 CYCLONE TUB PATCHING SOME HOLES IN THE BOTTOM WITH SILICONE. THE SILICONE REACTED WITHW ATER RELEASING FUMES WHICH BOTHERED HIS LUNGS.

WHEN HE WAS STEPPING DOWN FROM A TRUCK HE WAS WORKING ON HIS FOOT LANDED IN SOME COOLANT ON THE FLOOR HIS FOOT SLIPPED & TWISTED HIS LEFT KNEE

WHILE FUSING SCRUBBER LINE PIPE, HE HIT HIS LEFT HAND WITH CHIPPING HAMMER BETWEEN THUMB FOREFINGER.

68935 OPERATING A LOADER, THE LOADER SEAT GAVE HIM A PAIN IN THE BACK.

WHILE HE WAS WIPING OFF A TRUNION ON A LOADER HE GOT A METAL SLIVER IN HIS CENTER FINGER ON HIS RIGHT HAND.

HE WAS CLIMBING UNDER GRIZZLEY AND HE BUMPED HIS LEFT KNEE ON A CROSS MEMBER.

HE WAS CHANGING ROLLERS ON A CONVEYOR. WHEN HE WENT TO THROW THE OLD FRAME DOWN, IT HIT AN ANGLE BRACE ON THE STRUCTURE AND HIS FINGERS WERE BETWEEN THEM. THIS CUT HIS INDEX AND MIDDLE FINGERS ON HIS RIGHT HAND. HE ALSO GOT A PIECE OF METAL IN HIS FINGER.

HE WAS LIFTING A PUMP OUT OF A BARREL AND PULLED A MUSCLE IN LOWER BACK.

GRINDING A WELD ON NEW HAND RAIL WHEN PARTICLE OF METAL WENT IN RIGHT EYE.

WHEN CLEANING ASH BUILD UP ON 5" PIPE WITH HAMMER AND CHISEL A PIECE BROKE OFF THE END OF THE CHISEL, PENETRATING RIGHT SIDE OF LEFT WRIST.

INSTALLING V-BELTS ON PULLEY ,RING & LITTLE FINGERS GOT CAUGHT BETWEEN BELT AND PULLEY RESULTING IN LACERATIONS AND BROKEN NAILS.

WHEN REMOVING V BELTS FROM WET SCREEN MOTOR SWINING MOTOR BASE SUPPORT BROKE OFF ALOWING MOTOR TO FALL STRIKING EMPL'S LEFT FOOT.

EMPLOYEE WAS USING A PRY BAR TO CLOSE BOTTOM DOORS ON RAIL CAR WHEN BAR SLIPPED CAUSING BACK STRAIN

EMPLOYEE WAS LOOKING IN FLAME PEEP HOLE IN FRONT OF FURNACE. IGNITION TOOK PLACE AS HE LOOKED IN, BACKFIRING INTO EMPLOYEE'S FACE.

WHILE RAISING THE LOADOUT SNOUT THE HANDLE OF THE WINCH SLIPPED OUT OF HIS HAND AND SPUN AROUND AND HIT THE BACK OF HIS RIGHT HAND.

WALKING OVER TO #70 STACKER BELT SLIPPED ON WET CONCRETE.

AGGRIVATED A PRE-EXISTING HERNIA CONDITION WHILE OPENING DOOR ON RAILROAD CARS.

CHANGING BEARING ON SCREEN FEED CONVEYOR GOT SMALL PIECE OF METAL IN EYE.

WHILE SHOVELING SAND INCURRED PAIN IN BOTH HANDS.

HE WAS CLOSING A BOTTOM RAILROAD CAR DOOR BY HITTING IT WITH A BIG BAR HIS INDEX FINGER ONHIS L HAND HIT A VACUUM ADAPTER CAP AND CUT AND BRUISED HIS FINGER

NARROW ROAD, CURVES, PICKUP TRAVELING TOO FAST FOR SAFE OPERATION AND CONTROL OF VEHICLE DRIVER KEPT BRAKES ON SKIDDED TO LEFT, LOSING STEERING CONTROL, SLID UNDER EUCLID WATER TRUCK, LEFT FRO NT WHEEL OF EUCLID TRUCK WENT OVER RIGHT FRONT FENDER DIAGONALLY TO THE RIGHT TOTALING PICKUP.

EMPLOYEE WAS REMOVING A COLLAR FROM PULLEY SHAFT. ANOTHER EMPLOYEE PUT A TENSION ON PULLEY WITH COME ALONGS SHAFT PULLED THROUGH PULLEY FROM TENSION APPLIED, CATCHING J LEFT MIDDLE FINGER BETW EEN COLLAR & BEARING CUTTING OFF TIP OF FINGER BEHIND NAIL.
