Emp was working with two other emp installing erosion control matting and grass seed on a dirt slope. While placing rock and staples for holding the matting the emp right foot slid into a washout causing them to fall. Their foot was wedged in the washout and remained stationary as they fell resulting in twisting to their ankle.

Emp was holding chisel while trying to loosen bolts on blow bar locking rounds on crusher rotor and striking it with a sledgehammer. The hammer ricocheted off the chisel and struck emp left thumb.

emp was cleaning mud from dozer tracks with shovel at end of shift and struck a bolt causing jarring motion in arm/elbow
the affected employee was in the process of hooking up a water hose. While hooking up the hose one cam lock would not fully seat. The affected employee then turned to the left to pick up a rock to use as a hammer to seat the cam, when they stepped to the left their head came into contact with an elevated beam on the front of head. Hard hat was not knocked off their head.

Miner was assisting in changing a sheave on a gearbox with the shaft pointed vertically. The miner held the sheave from rotating using their hands while another miner attempted to free the bushing. The sheave unexpectedly broke loose from the busing and shaft and fell down to the gearbox, pinching the miner's finger between the sheave and the gearbox housing. Left, middle finger.

While operating an articulating haul truck, the employee drove through a rut caused by water drainage from recent rains. When the rut was struck, the cab was shaken around. After these events, the employee complained of pain in neck, back, shoulder and knee on the left side of the body. Events did not become occupational injury until medical treatment on 3/2/2023

While tramming on ledge 2 after finishing boulder drilling, the drill got stuck. EE tried to tram and the drill went up on a rock giving a tipping feeling. EE moved back off the rock and got out of the cab to move it out of the way of the track. EE lifted and rolled the rock. It moved unexpectedly and pinched EE's left index finger cutting it between the first two joints.

After loading material from the floor of the bench, the excavator operator was leaving their working pad to load the material from the pad itself when some material including rock and mud fell from the wall nearby, landing on the counterweight of the excavator damaging it and rendering it inoperable
Employee 1 was standing beside a screen deck attempting to remove bolts from the screen rails. Simultaneously employee 2 was standing on top of the screen deck prying on the screen rail with a prybar in order to apply pressure to the bolts so they could be removed more easily. The prybar slipped from it's pry-point and the handle struck employee 1 in the head.

Sales Manager who is not a miner was taking a tour of the quarry with an experienced Area Manager and several customers. The Sales Manager slipped while walking over a small mound of looses rocks and dirt and fell to the ground resulting in an unspecified left shoulder injury possible rotator cuff tear.
EE dismounted front end loader. As EE started to step away from machine EE caught toe on a rock & felt a pop in right knee. EE continued working & didn't request to see a doctor until 04/18/18. Diagnosed with a sprained knee & allowed to continue to work regular job. Report submitted within 10 working days initial doctor visit. *EE began losing time on 6/26/18 for surgery.

Employee performing pre-shift inspection on Hitachi excavator. Employee raised engine shroud to check engine oil level and sprained right shoulder.
Performing maintenance on primary crusher. Employee using sledge hammer to remove crusher wear item from carrier. A small piece of metal broke off and struck employee below left knee.
Employee was assisting experience miner with installing v-belt onto screen #9. Employee's finger became caught between sheave and v-belt resulting in injury.

On 7/17/2014 employee was terminated for excessive absenteeism. Later in the day on 7/17/14 after termination, company was informed employee is alleging he suffered from a hernia. Hernia or incident was not reported to company prior to contact after termination.
On 6/27/14 while removing gears from feeder shaft, employee pinched right wrist. Employee continued work without issues until 7/2/14. While washing screen decks on 7/2/14 with water hose, employee felt "pop" in right wrist. Employee was seen by doctor on 7/3/14 for first time.

Employee lacerated his right index finger during the removal of a worn chair cushion. Employee was taken to the hospital, received 6 sutures and released the same day back to full duty.
Employee was injured during the completion of a walk around pre-shift inspection of a CAT 966 wheel loader. At inspection of the coolant level, lost balance and fell forward, bruising left shoulder on the engine compartment. Employee taken to hospital and released back to work on modified duty on the same day. Employee has not been yet released back to full duty.

Employee while grinding a handrail over his head, a small piece of metal fell into his eye. The employee was wearing safety glasses while using the grinder. The next day (10/03/08) he was seen by a physician who removed a small particle. He was treated and released to return to full duty with no further treatment.

A SHAKER HOPPER WAS BEING INSTALLED AT A LIMESTONE QUARRY. DURING THE CONSTRUCTION A 20# TORQUE WRENCH WAS USED TO BOLT CONNECTIONS. THE INJURED EE HURT HIS LOWER BACK WHILE USING THE WRENCH T O TIGHTEN A BOLT IN AN AWKWARD POSITION.
EE WELDED IN IMACT CRUSHER. THE NEXT MORNING EYE WAS BURINING. EE THOUGHT IT WAS FLASHBURN. VISIT TO DR WHO REMOVED SMALL FOREIGN PARTICLE.
