emp was cleaning mud from dozer tracks with shovel at end of shift and struck a bolt causing jarring motion in arm/elbow

the affected employee was in the process of hooking up a water hose. While hooking up the hose one cam lock would not fully seat. The affected employee then turned to the left to pick up a rock to use as a hammer to seat the cam, when they stepped to the left their head came into contact with an elevated beam on the front of head. Hard hat was not knocked off their head.

Miner was assisting in changing a sheave on a gearbox with the shaft pointed vertically. The miner held the sheave from rotating using their hands while another miner attempted to free the bushing. The sheave unexpectedly broke loose from the busing and shaft and fell down to the gearbox, pinching the miner's finger between the sheave and the gearbox housing. Left, middle finger.

While operating an articulating haul truck, the employee drove through a rut caused by water drainage from recent rains. When the rut was struck, the cab was shaken around. After these events, the employee complained of pain in neck, back, shoulder and knee on the left side of the body. Events did not become occupational injury until medical treatment on 3/2/2023

While tramming on ledge 2 after finishing boulder drilling, the drill got stuck. EE tried to tram and the drill went up on a rock giving a tipping feeling. EE moved back off the rock and got out of the cab to move it out of the way of the track. EE lifted and rolled the rock. It moved unexpectedly and pinched EE's left index finger cutting it between the first two joints.

After loading material from the floor of the bench, the excavator operator was leaving their working pad to load the material from the pad itself when some material including rock and mud fell from the wall nearby, landing on the counterweight of the excavator damaging it and rendering it inoperable

Employee 1 was standing beside a screen deck attempting to remove bolts from the screen rails. Simultaneously employee 2 was standing on top of the screen deck prying on the screen rail with a prybar in order to apply pressure to the bolts so they could be removed more easily. The prybar slipped from it's pry-point and the handle struck employee 1 in the head.

Employee was cutting a plastic pipe when the sprung weight caused the pipe to hit employee in the face

Employee was assisting in changing screens in the plant when one screen fell on the top of the right foot of the employee. Employee was wearing safety-toe boots.

Sales Manager who is not a miner was taking a tour of the quarry with an experienced Area Manager and several customers. The Sales Manager slipped while walking over a small mound of looses rocks and dirt and fell to the ground resulting in an unspecified left shoulder injury possible rotator cuff tear.

EE dismounted front end loader. As EE started to step away from machine EE caught toe on a rock & felt a pop in right knee. EE continued working & didn't request to see a doctor until 04/18/18. Diagnosed with a sprained knee & allowed to continue to work regular job. Report submitted within 10 working days initial doctor visit. *EE began losing time on 6/26/18 for surgery.

Employee performing pre-shift inspection on Hitachi excavator. Employee raised engine shroud to check engine oil level and sprained right shoulder.

Performing maintenance on primary crusher. Employee using sledge hammer to remove crusher wear item from carrier. A small piece of metal broke off and struck employee below left knee.

Employee was assisting experience miner with installing v-belt onto screen #9. Employee's finger became caught between sheave and v-belt resulting in injury.

On 7/17/2014 employee was terminated for excessive absenteeism. Later in the day on 7/17/14 after termination, company was informed employee is alleging he suffered from a hernia. Hernia or incident was not reported to company prior to contact after termination.

On 6/27/14 while removing gears from feeder shaft, employee pinched right wrist. Employee continued work without issues until 7/2/14. While washing screen decks on 7/2/14 with water hose, employee felt "pop" in right wrist. Employee was seen by doctor on 7/3/14 for first time.

Employee was turning around, tripped and fell on flat ground, resulting in abrasions to left leg and fracture to right ankle

While flipping steel plate, plate kicked back, struck employee leg. Abrasion and Contusion to Right leg

Employee while working from a manlift 20ft above the ground he injured his right shoulder during the lowering of a counter weight pulley using a chain jack. The manlift basket suddenly dropped 2ft from the original working position causing an over extension of his right arm.

employee was hitting on a gear to remove it from shaft when a small chip came off and stuck him in the forearm it was less than 3/16" in diameter he reported the injury on 6/23/12

Employee while conducting repairs on a conveyor injured his right eye from foreign material (dust/debris). This injury occurred due high winds in the work area. Employee was wearing proper PPE (safety glasses)

Tamrock DX800 Drill operator during the repositioning/alignment of a T51 steel on the carriage rack, strained a muscle in his neck.

Employee exiting truck lost footing/balance letting go of ladder with right hand leaving left hand on railing briefly supporting body weight. Employee taken directly to local medical center. Injuries recieved; straining of three fingers on left hand, left wrist, and left shoulder. Employee released back to work on 5/05/2010 under limited duty, with therapy.

Employee lacerated his right index finger during the removal of a worn chair cushion. Employee was taken to the hospital, received 6 sutures and released the same day back to full duty.

Employee was injured during the completion of a walk around pre-shift inspection of a CAT 966 wheel loader. At inspection of the coolant level, lost balance and fell forward, bruising left shoulder on the engine compartment. Employee taken to hospital and released back to work on modified duty on the same day. Employee has not been yet released back to full duty.

Employee while grinding a handrail over his head, a small piece of metal fell into his eye. The employee was wearing safety glasses while using the grinder. The next day (10/03/08) he was seen by a physician who removed a small particle. He was treated and released to return to full duty with no further treatment.

Employee was trying to catch a wedge he was driving out of a horizontal impact crusher. The wedge slid through his hand and the edge cut the palm of his left hand.

EMPLOYEE USING UTILITY KNIFE TO CUT SCRAPER WHEN KNIFE SLIPPED RESULTING IN A LACERATION TO HIS LEFT WRIST. REQUIRED STITCHES.

While shoveling, digging out holes, the shovel entered loose material and it stopped suddenly jarring left shoulder causing pain.

Dismounted vibrating screen improperly resulting in impaction of left ankle.

EE was descending into the quarry pit when he had break in the bulk truck that he was driving. The truck traveled about 1070 feet and ended up crashing into a pile of rip rap.

EMPLOYEE WAS LOADING BOULDERS OFF OF A LADDER AT SEMO STONE. HE WAS BETWEEN 5-6 FEET OFF THE GROUND WHEN HE LOST HIS BALANCE AND FELL. HIS LEG WENT THROUGH THE LADDER, CAUSING HIM TO STRAIN MUSCLE IN HIS LEG AND BACK.

THE EMPLOYEE ATTEMPTED TO STRAIGHTEN 4 PIECE OF BOLT SHEET METAL WITHOUT PROTECTIVE GLOVES AND CUT HIS FINGER.

EMPLOYEE WAS INSTALLING A DRIVE SHAFT ON A EUCLID R 50. THE PARKING BRAKE WAS RELEASED BEFORE ENGINE WAS SHUT OFF WHICH ALLOWED THE DRIVE SHAFT TO TURN SMASHING AND FRACTURING EMPLOYEE'S THUMB .

EE WAS PRYING ON PIECE OF HEAD PULLY LAGGING THAT HAD COME LOOSE WITH CROW BAR WHEN THE BAR SLIPPED & SMASHED HIS THUMB

A TOOL BOX LID WAS OPENED WHEN THE EE LOOKED THEOTHER DIRECTION THE LID FELL & SMASHED HIS WRISTTHE LID WEIGHTED APPROX 15 LBS

EMPLOYEE WAS STANDING AT THE DESK IN THE SCALE HOUSE WEIGHING A TRUCK WHEN HIGH WINDS BLEW THE ROOF OFF OF THE BUILDING AND BLEW OUT ALL OF THW WINDOWS IN THE BUILDING. EMPLOYEE SUSTAINED A CUT ON THE HAND WHICH REQUIRED STITCHES.

WHILE CHANGING THE HAMMERS IN THE PRIMARY IMPACTER, A PIECE OF STEEL CAME CAME OFF OF THE EXISTING HAMMER THAT WAS BEING HIT WITH A SLEDGE HAMMER AND PIERCED HIS RIGHT FOREARM.

EE WAS SHUTTING OFF THE WATER PUMP. HE STEPPED OVER THE TONGUE OF THE TRAILER AND SLIPPED OFF THE WET TONGUE. HE FELL ON HIS BACK AND COMPLAINED OF SORENESS IN THE GROIN AREA, THE EE NEVER SOU GHT MEDICAL TREATMENT UNTIL NOV. HE HAS NOT MISSED ANY TIME FROM WORK TO DATE.

ROCK ROLLED DOWN BOOM OF 400 KOMATSU EXCAVATOR BROKE GLASS HIT LEG BRUISED & 8 STITCHES

A SHAKER HOPPER WAS BEING INSTALLED AT A LIMESTONE QUARRY. DURING THE CONSTRUCTION A 20# TORQUE WRENCH WAS USED TO BOLT CONNECTIONS. THE INJURED EE HURT HIS LOWER BACK WHILE USING THE WRENCH T O TIGHTEN A BOLT IN AN AWKWARD POSITION.

EE WELDED IN IMACT CRUSHER. THE NEXT MORNING EYE WAS BURINING. EE THOUGHT IT WAS FLASHBURN. VISIT TO DR WHO REMOVED SMALL FOREIGN PARTICLE.

EMPLOYEE GOT FOREIGN OBJECT IN EYE NECESSITATINGREMOVAL BY A DR. AT THE END OF SHIFT.

WHILE ATTEMPTING TO MOVE 55 GAL DRUM IN MAINTENANCE SHOP EE SLIPPED & SPRAINED WRIST.

WHILE BREAKING THE COUPLING ON DRILL STEEL A SLIVER BROKE LOOSE AND EMBEDDED IN DRILLER S ARM RESULTING IN A SMALL WOUND REQUIRING A TETANUS SHOT AS A PRECAUTION.

WHILE ATTEMPTING TO REMOVE BROKEN TROUGHING ROLLER ON CONVEYOR BELT, EE'S HAND SLIPPED, CAUSING HIM TO CUT 3 FINGERS ON JAGGED EDGE. ONE CUT REQUIRING 5 STITCHES.

WHILE CHANGING STEEKS IB DRUKKM STRUJER BAR BROKE PUSHING SPLINTER OF STEEL INTO EE'S LEG.

OPERATOR WAS STANDING ON REAR WHEEL OF FRONT-END LOADER, CHECKING HIS AIR FILTERS. HIS FOOT SLIPPED CAUSING A FALL TO THE GROUND. OPERATOR HA DSLIGHT ABRASION ON HIS FOREHEAD. AFTER CLEANIN G ABRASION OEPRATOR STATED HE WA FINE, NEEDED NO MEDICAL ATTENTION, RETURNED TO WORK. ON 9-23-93 HE STATED HE HAS SOME PAIN IN HIS SIDE. X-RAYS REVEALED THREE FRACTURED RIBS. HE CONTINUES T

ON 11/9/91 EMPLOYEE WENT TO EMERGENCY ROOM WITH SWOLLEN EYE AND BLURRED VISION. DR EXTRACTED A PIECE OF STEEL AND CLEANED EYE. EMPLOYEE DOES NOT KNOW EXACTLY WHEN OR WHERE ACCIDENT OCCURRED, B UT ASSUME IT HAPPENED LAST DAY OF WORK, 11/7/91, WHEN HE MAY HAVE RUBBED EYE WITH HIS GLOVE ON.

WHILE PULLING WIRE,THE ROPE BROKE AND WRAPPED AROUND HEAD AREA.

UPON REENTERING CAB OF THE LOADER, OPERATOR REACHED TO CLOSE THE DOOR. WHILE REACHING FOR HANDLE, THE WIND CAUGHT THE DOOR BLOWING IT SHUT AND THE OPERATOR'S HAND SLIPPED, CATCHING HIS THUMB B ETWEEN THE BACK EDGE OF THE DOOR AND METAL PLATE, CUTTING HIS THUMB.

FOGGY OUTSIDE RAN INTO STOCKPILE WITH PICKUP TRUCK AND UPPER LIP HIT STEERING WHEEL, WHILE DRIVING FROM HIS WORK STATION TO CLOCK ROOM. THIS WAS THE END OF HIS SHIFT.
