EMPLOYEE WAS MEASURING A BARGE DEPTH WHILE HE WAS WALKING ON THE BARGE BETWEEN CHECK POINTS HE BUMPED HIS KNEE ON A BULKHEAD
EMPLOYEE WAS FOUND UNCONSCIOUS IN THE CRUSHER BUILDING ON THE 2ND FLOOR NEAR THE STEPS GOING TO THE THIRD FLOOR. NO INJURIES WERE VISIBLE. DURING THE PAST NINE YEARS, HE HAS SUFFERED BLACKOUTS AND CONVULSIONS APPARENTLY AS A RESULT OF AN AUTOMOBILE ACCIDENT.

EE WAS USING A DRILL HOLE SAW ATTACHMENT. HE WAS DRILLING A 3 3/8 HOLE THRU INSULATED SIDING. AS HE DRILLED THRU THE SAW CAUGHT CAUSING THE DRILL TO TWIST. BEFORE HE COULD RELEASE THE TRIGGER THE DRILL TWISTED HIS RIGHT HAND BREAKING 2 BONES IN H IS HAND.

USING UTILITY KNIFE.KNIFE SLIPPED AND CUT LEFT HAND NEAR BASE OF THUMB.
DOZER OPERATOR WAS BACK FILLING A PIT ON RECLAMATION HE WAS PUSHING A FULLL BLADE OF DIRT WHEN A HYDRAULIC LINE BLEW IT APPARENTLY BLEW FLUID ON THE MANIFOLD CAUSING A FIRE IN THE BELLY PAN TH E OPERATOR COULD NOT PUT THE FIRE OUT WITH THE FIRE EXTINGUISHER A LOCAL FIRE DEPART WAS CALED IFIRE WAS CONFINED TO ENGINE COMPARTMENT AND DASH
WITNESS WAS CLEANING THE PAINT SPRAYER WITH ACETONE THE VICTIM CAME ALONG GRABBED THE SPRAY GUN AND PUT A NEW TIP ON IT IMPROPERLY THE VICTIM THE PRESSED THE TRIGGER SINCE THE TIP WAS NOT INS TALLED PROPERLY THE WITNESS HAD ADVISED THE VICTIM TO LEAVE THE PAINT SPRAYER ALONE
EMPLOYEE HAD EXITED THE DOZER AND WAS STANDING ON THE PUSH BAR WITH HIS LEFT HAND ON TRHE GRAB RAIL NEAR THE SIDE OF DOOR. THE WIND BLEW THE DOOR SHUT. THE DOOR CAUGHT THE EMPLOYEES GLOVE AND PULLED HIS LEFT INDEX FINGER INTO THE DOOR FRAME MASHING THE FINGER.
