EMPLOYEE WAS HELPING CHANGE STEMS ON A DRILL AND WHILE LOWERING THE STEM THROUGH THE DECK, THE STEM CAUGHT THE EDGE OF THE DECK AND KICKED OUT PINCHING HIS HAND BETWEEN TWO STEMS.
Employee was climbing up the ladder of a Komatsu 500 loader with mud on his boots and slipped off of the 4th step on the loader. Injured landed on his back and shoulder area.
EE CAME TO A STOP ON AN INCLINE AT A ROAD CROSSING. WHEN HE PROCEDED FORWARD, EE STALLED THE TRUCK. WHILE ATTEMPTING TO RESTART THE TRUCK, EE DID NOT APPLY THE EMERGENCY BRAKE & EXPIRED AIR SU PPLY IN THE TANKS. THE TRUCK COASTED BACKWARDS & OVERTURNED. THE CB MICROPHONE HIT EE IN THE MOUTH CAUSING STITCHES. THERE WAS NO TIME LOST.

EE AND OTHER PULLING AND STRETCHING A 3" WATER DISCHARGE HOSE FROM A WATER PUMP IN THE PIT UP OVER A HILL TO THE OTHER SIDE TO CLEAR THE WATER FROM THE PIT. WHILE TUGGING TWISTING AND PULLING THE HOSE, HE FELT A PULL IN HIS STOMACH AREA.
HE SLIPPED WHILE CARRYING A 5 GALLON BUCKET OF GREASE, TWISTING HIS BACK.
THE LIMESTONE CRUSHER HAD PLUGGED WITH ROCKS REQUIRING A CRANE TO REMOVE BIG ROCKS WHOLE. SMALL ROCKS WERE REMOVED BY HAND. EE REMOVED A ROCK TOO HEAVY TO BE REMOVED BY HAND, HURTING HIS BACK WHICH IS A CHRONIC CONDITION.
