Employee was traveling up a grade when he ran out of fuel stalling the engine. Without maxi brakes that lock up when depletion of air the truck could not start, it started to coast backwards and struck a birm and rolled onto its side. Employee was wearing his seat belt.

Employee was descending down the ladder of an end dump and slipped on the second step from bottom and fell to ground.
EE WAS ATTEMPTING TO REMOVE THE DISCHARGE HOSE FROM A 4 INCH WATER PUMP WHILE THE HOSE WAS STILL UNDER PRESURE. ONCE THE CLAMPS WERE RELEASED, THE METAL END OF THE HOSE SPRUNG BACK AND HIT EE IN THE MOUTH.

TRUCK WAS LOADED PREPARING TO TARP TO LEAVE - OTHER TRUCK WAS APPROX 10' -15' BEHIND ALSO TRAPING TO LEAVE. TRUCK ROLLE FORWARD INTO OTHER TRUCK PINNING DRIVER BETWEEN THE BACK OF HIS TRUCK & THE FRONT OF THE OTHER TRUCK. DRIVER WAS MOVED BY LOADER OPERATOR - DRIVER WAS ADMINISTERED CPR - AMBULANCE CALLED CRUSHING INJURIES.
