Miner was attempting to bring disabled truck down from the upper most harvest area. As he was dumping load, breaks let loose & truck went down uncontrollably. As it came to the next lower level, it hit a berm causing the miner to be ejected through the right hand window & was presumed to have been struck by a part of truck.

While climbing down from a ladder, miner grabbed an unsecure handle and was thrown off balance. Miner proceeded to jump off ladder and land on their feet due to off balance. Upon impact, miner fractured their right ankle. Height was approximately 6ft-9ft.

Miner parked a D9 Dozer and was climbing down to end shift. Miner's foot slipped and Miner was able to catch self on handrails. Miner had 3 points of contact during exit. Surface of step was slippery due to wet weather. Miner strained right shoulder, no lost work days. Miner is able to work Miner's normal duties with Miner's work restrictions.

While clearing material from hopper, Miner was using a hand held Digging Bar. Miner overextended Digging Bar and pinched left thumb between Digging Bar handle and Hoppe, causing hairline fracture. No surgery, No loss workdays.

Chain was holding screen panel upright on the floor to the mast of the forklift. Chain had an opened hook securing the chain to itself. The chain was relieved of tension and caused the hook to come loose, causing the screen panel to fall. since then, the chain hooks have been changed to clip hooks for this operation to prevent from coming loose.

A hillside collapse due to a geological anomaly. The equipment operator had made approximately 10 passes at the new cut. As the equipment operator was backing up to make another pass/cut, the hillside collapsed. The force pushed in the window and partially filled the cab of the dozer.
Employee stated they were exiting the pit in a haul truck with a full load. They further stated they had their foot sideways on the "lock-up" button and hit a bump which caused them to move up and down on the air-ride seat causing them to feel pain in their left knee.

EE was working in the upper harvest area when he began to cough for quite a while and began to feel poorly and began feeling winded with chest pressure. He had bronchitis and dust came in to enclosed cab which he believes to be the cause of aggravation.
Employee was closing exit gate to mine and pulled muscle when lifted the gate. Stanchion Post.

The dispenser on the lime silo @ the long belt was malfunctioning due to clumps in the mix. While attempting to adjust & shut down the dispenser a gust of wind blew lime dust into the employees eyes. When he attempted to rub it our with his hand he hurt then got dirt in his eye.
Employee was inspecting conveyor belt going to the HR screen, went to step up on concrete curb and foot slipped off. Resulting in sprain to knee. Subsequently learned he had a ton meniscus.
Miner was chipping dirt slag from inside chute to cone 2 feed belt going from screen 1. Working in small space miner was holding chip gun chest high for extended period and shoulder became sore. He did not report this at time and sought medical assistance from chiropractor, who said he had separated shoulder, which he moved back into place. Reported incident on 9/24/12.

Employee banged elbow, causing carpal tunnel to flare up. Now waiting for surgery.
Operator was doing pre-operation check when a snake jumped at him & he jumped back and fell.
EE was greasing a muddy loader when he lost his footing and fell off loader.
EE lost footing while installing side panels on loader. Did not lose any time.
Employee was grinding in a screen to put in a new screening deck. Particles of dust were in his eyes causing an infection in his eye.
EE WAS SHOVELING AND STANDING IN MUD. SUCTION DEVELOPED BETWEEN MUD AND SHOES. WHEN EE MOVED, HE WAS UNABLE TO MOVE HIS FEET. HE TRIPPED AND DURING FALL, HURT BACK.

WHILE DRILLING HOLES IN TRUCK BED FOR A COMPARTMENT, DRILL BIT SLIPPED AND SLAMMED DOWN ON TOP OF HIS HAND.
HIS FOOT SLIPPED OFF LADDER WHILE HE WAS TRYING TO CLEAN THE REAR CAB WINDOW, CAUSING FOOT FIRST FALL TO GROUND.
