While hauling & piling topsoil; reclaiming, with TS 24 Pan, rear slipped and began to slowly roll over. Pan rolled completly over once with operator buckled in. Pan then set upright long enough for operator to unbuckle and dismount. Pan continued to roll down slope. Operator landed on right shoulder & right side ribs.

While placing a pull cable on a D11 Dozer, miner said he felt a pain in the upper inside of right leg (groin area). Miner did not seek treatment until 3/18/10 in ER, for soreness and discoloration of affected area.

EMPLOYEE WAS IN AN AREA OF THE MACHINE WHERE HE WAS NOT SUPPOSE TO BE AT, THE OPERATOR DID NOT SEE HIM. AS THE CARRIAGE WAS MOVING FORWARD TO GET ANOTHER FLIGHT, THE EE POSITIONED HIMSELF BETWEEN A STATIONARY LIFT JACK & THE MOVING CARRIAGE WHICH PINCHED HIM & FRACTURED HIS PELVIS. THE OPERATOR HEARD HIM YELL & STOPPED THE CARRIAGE.
HE WAS DISMOUNTING FROM THE CAT D10L DOZER WHEN HE LOST HIS FOOTING WHILE STEPPING ON THE PUSH ARM. WHEN HE STARTED TO FALL, HE GRABBED AND CAUGHT HIMSELF BY HOLDING ONTO THE HAND RAILING. THE WEIGHT OF HIS BODY BEING SUPPORTED BY HIS LEFT ARM CAUSED HIS LEFT BICEP TO TEAR REQURING SURGERY.
EMPLOYEE WAS GETTING IN THE BACK OF A MECHANICS TRUCK TO FUEL AN AIR COMPRESSOR WHEN HE SLIPPED AND FELL TWISTING HIS LEFT KNEE IN THE PROCESS
It had been raining a roads were wet. One truck slip down a ramp and hit EE truck. The right rear corner causing a slight bump to the truck. He said he his his head as a result of the other truck hitting his truck.
Employee was moving a 6" water pump while operating a Cat 910 loader. The cab door was opened and he had his hand on the door frame. A gust of wind caught the door and shut it on his finger.

Employee was fueling his dozer and while he was on the catwalk getting out of the cab he slippped and landed against the door frame hitting his right shoulder. It was early in the morning. The catwalk was damp and it was dark out.
He was repairing the exhaust on the michigan loader. he was standing on the right rear tire and bent over to see if the other was handing him the new exhause pipe, slipped and fell to the ground.
His dozer was stuck in the mud so he got another operator on another dozer to help pull him out. He was pulling a cable by hand over rock, and mud trying to get it hooked onto his dozer. While bending and pulling the cable he felt a sharp pain in his lower back. He didn't report it to his doctor for three days after the injury.

EMPLOYEE ALONG WITH ANOTHER EMPLOYEE WAS LIFTING A WATER PUMP WHEN THE OTHER EMPLOYEE LET GO CAUSING THE PUMP TO FALL ON ONE END. THE WEIGHT OF THE FALLING PUMP TORE THE INJURED EMPLOYEE'S RIG HT BICEP MUSCLE.
WHILE STRIPPING OFF OVERBURDEN WITH DOZER AND PRYING ON LARGER ROCKS, ONE OF THE ROCKS BROKE LOOSE AND CAUSED HIS DOZER TO SURGE FORWARD CAUSING A JOLT TO THE OPERATOR AND SPRAINING HIS BACK.

EE WAS TRYING TO REMOVE A BUCKET TOOTH FROM A CAT 980 LOADER. HE WA USING A HAMMER TRYING TO LOOSEN THE TOOTH WHEN A PIECE OF METAL BROKE LOOSE AND HIT HIM IN THE STOMACH. 1/4" PIECE OF METAL LODGED IN HIS STOMACH AND NEEDED TO BE SURGICALLYREMOVED ON AN OUTPATIENT BASIS.
