Operator was loading material in the crusher. A large rock became stuck, when ee climbed the stairs to inspect the problem ee's foot slipped and ee fell. EE hit ee's side on the rail and fell to the ground.

Slipped while climbing up in excavator and fell onto rocks below injuring ribs on left side.

EE was working on light tower. Two pipes were stuck together, while pulling and twisting to get them apart he hurt his back.

Employee picked up a rock that was in the mine traffic area and strained his back.
Employee became disoriented and was walking along mine road. He was taken to local hospital, then transferred to Austin for further testing. The hospital has still not drawn any conclusive reason for his being disoriented. Perhaps medication issue. He is being seen by a neurologist and his pcp. He has not returned to work as of this time.
Cause-Heavy smoker. Heart Attack-Stint put in this date and plan to do by-pass surgery in (3) weeks.

EE WAS STANDING ON BACK OF MAINTENANCE UTILITY TRUCK OBSERVING WORK BEING DONE ON GEAR BOX ON CONVEYOR BELT FOR SCREENING PLANT. EE STEPPED BACK WITHOUT LOOKING & STEPPED OFF TRUCK FALLING APPROX. 4' TO THE GROUND. BRUISING ON LEFT SIDE & RIBS FROM IMPACT OF FALL.
