Table Rock-Quarry #3 Metal/Non-Metal

The employee was riding in the jump seat of a Komatsu HM400-3 during training. The trainer hit a rut in the road causing the employee to strike EE's head on the handle beside EE. The employee suffered a concussion.

While cleaning off cat walk, individual walked beneath area and was struck on the hard hat, knocking the individual to the ground. Entire crew was warned by supervisor that cleaning of cat walk was in process. Individual ignored warnings.

On Monday, January 7, 2019 a miner was injured while operating an articulating truck on surface mine property. The miner was driving the articulating truck down a hill when it struck a berm. The associate was treated at a local hospital for injuries incurred in the accident. Minor damage was sustained to the articulating truck.

Loading a magazine onto a trailer, went to release the chains on the magazine, magazine shifted, laceration to right lower leg.

Sweeping bed of truck, broom handle broke at a weak spot and cut right hand

Employee was climbing a ladder and somehow missed gripping the ladder with EE's right hand about 5 feet up. EE fell from ladder but landed on feet at ground level. EE's right foot actually landed on another employee's foot causing EE's to roll and break at the ankle area.

Employee was hanging gaurds etc. on the plant. He noticed his elbow getting sore but did not say anything because he has arthritis. He worked all day. (the 28th) Came into work the next day (29th) with his elbow red and swollen. The doctor said he had an infection in the elbow and should be fine with antibiotics.

Injured was changing a tire on a Bobcat skidsteer. He proceded to remove tire and wheel assembly. The tire and wheel assembly then fell, smashing his middle finger between the inner wheel and axle hub. His finger began to slightly bleed. Employee then notified his supervisor. He was then driven to the local hospital where he recieved ten stitches.

EE WAS CLIMBING DOWN THE LADDER ON THE POWDER TRUCK. WHEN HE SLIPPED, HIS ARM WAS CUT. THE CUT WAS SO CLEAN THAT HE HADN'T REALIZED HE WAS CUT UNTIL SOMEONE POINTED IT OUT. IT STOPPED BLEEDING SHORTLY AFTERWARD.

EE WAS LOOSENING THE BOLT & WHILE TRYING TO BREAK IT LOOSE THE WRENCH GAVE WHEN THE BOLT LOOSENED ABRUPTLY. HIS HAND SLIPPED & STRUCK THE NEARBYHAND RAIL & SMASHED HIS RIGHT THUMB A LITTLE BUT PUT A CUT ON IT AS WELL

EE WAS GREASING THE BEARINGS ON THE PRIMARY CRUSHER. HE WAS STANDING ON A WALKWAY LEANING OVER AHANDRAIL TO REACH A BEARING APROX. 24" FROM HANDRAILL. THE HANDRAIL WAS WAIST HIGH TO THE EE. HE LOST HIS BALANCE & FELL TO HIS KNEES ON THE WALKWAY. AS HE FELL HIS FIGHT SIDE SCRAPPED HANDRAIL.

HE WAS DOING CLEAN-UP AROUND TH ECRUSHE RWITH A BOB-CAT LOADER. WITH THE BUCKE DOWN CLEANIG U SPILLAGE, HE HHIT SOLID ROCK ONT EH SURFACE. HE WAS WEARING TH ESEAT-BELT, BUT IT JARRED HIM INJ RINNG HIS BACK.

EE WAS PRYING A ROCK JAMMED IN FEEDER HOPPER TO JAW CRUSHER. HE OVEREXERTED HIS LOWER BACK WITH STEEL PRY BAR.

EE WAS CARRYING A 5 GALLON CAN OF ROCK DRILL OIL UP A STEEP HILL TO THE DRILL (ROADWAY DIRT AND ROCK, EXTREMLY STEEP). EE STUMBLED OVER A ROCK, LOST HIS BALANCE AND TWISTED HIS BACK. EE NEEDED TO USE A PIECE OF EQUIPMENT TO CARRY THE OIL TO THE DRILL. BACK SAFETU WAS ADDRESSED AT THE FOLLOWING SAFETY MEETING.

TRUCK I PARKED AND AIR TANK BLEEDERS OPENED WHEN DRIVER LEFT EARLY. ANOTHER DRIVER TOOK TRUCK 1 TO HAUL UP THE HILL. TRUCK 2 WAITED UNTIL 1 WAS OUT OF SIGHT AND STARTED UP. 1 STALLED OUT ROLLI NG BACK INTO 2 BOTH CUT INTO BANK WITH NO INJURIES OTHER CLOSED VALVES AND DROVE TRUCK 1 UP THE HILL. A LOADER HOOKED A CHAIN TO 2 AND STARTED UP HILL. THE CHAIN BROKE DRIVER JUMPED CLEAR BUT

EE WAS TRYING TO REMOVE ROCK STUCK IN JAW CRUSHER WITH SLEDGE HAMMER.

WAS STANDING ON A CONVEYOR BELT USING A WRENCH FOR REGULAR MAINTENANCE OF PLANT WRENCH SLIPPED AND HE FELL ONTO EDGE OF CONVEYOR

WAS STANDING ON A ROCK CONVEYOR SERVICING IT. WHEN FINISHED JUMPED DOWN TO THE GROUND FROM A THREE FOOT HEIGHT SAID HE FELT SHRP PAIN IN HIS BACK.

WAS SCALING A WALL WITH A SIX FOOT SCALING BAR. A SMALL PIECE OF ROCK FLEW FROM THE WALL STRIKING HIM IN RIGHT EYE.
