Employee's right hand came into contact with a running chainsaw with the bar guard still on. Employee is not sure if they squeezed the guard to make the chain bite in and it chewed through the guard contacting ring finger, middle finger and thumb or if it came off as they were grabbing it.

While climbing a fixed ladder mounted to the side of the sand box, employee lost footing and slipped and fell down the ladder. Employee caught self with both hands on the bottom step resulting in hypertension of hands at the wrist joint. Employee's left thumb also caught the bottom step and was hyperextended as well. This resulted in a hand/thumb injury.

Employee improperly departed Morooka Track rig by jumping from rear platform. After making contact with the ground EE felt left knee "give out".

Employee was using chain saw to cut down a tree. The saw kicked back and made contact with their right knee resulting in a laceration on the anterior surface of the right knee.

Employee was attempting to re-engage thrust rollers onto a thrust ring on wash plant trommel. An excavator utilizing 3/8" chains was connected to the thrust roller stanchion to pull it back when the chain broke and employee was struck on the side of the head by either the flying chain or a chain hook. EE was medevac'd to Hospital where EE was diagnosed with a fractured skull.

Employee was removing bolt from water pipeline discharge flange with a 15" crescent. Discharge hose was attached to the pump which was suspended about 4-5 feet. While applying pressure to loosen bolt/nut the wrench unexpectedly slipped off of the nut resulting in handle hitting the employee in the forehead.

While hooking up a rigging chain to a steel plate that was being prepared to be moved, the employee yanked on the chain to remove the slack, which caused the plate to fall from a leaning vertical position and hit the outer surface of employees left leg knocking ee to the ground.

While performing routine maintenance on water pump, employee smashed finger while trying to remove guard resulting in a laceration on the tip of the right thumb.

While operating router tool, employee lost grip on tool and cut palm of hand on router bit.

Employee was driving track vehicle when cargo load required adjustment. While adjusting cargo load employee slipped & fell forward landing in prone position on deck. Due to incline position of vehicle, employee then rolled off deck & fell appx 5 feet & landed on spongy vegetated ground resulting in injury to left shoulder. Employee was wearing all required PPE at time of event.

Employee egressed off drill rig by improperly climbing around safety handrail & step down via front bumper. Bumper was not equipped with grip tape as it is not meant for use as step. Employee used 3 points of contact but was unable to maintain grip after foot slipped. Employee reports backwards fall from 4 feet onto flat gravel surface landing on back & hitting head on ground.

Employee was climbing down from the Morooka rig when weld attaching safety chain to support post broke resulting in a fall from appx 4 feet high onto a flat ground level surface which was frozen and covered with packed snow. Employee landed directly on tail bone/lower back. Employee was wearing all required PPE including a hardhat, and reports no head/upper extremity involvement.

Initially employee thought they strained their back. After seeking medical attention it has been determined employee has a Herniated Disc in their back.

Employee was using a pry bar to move a pin which came loose suddenly unloading force on bar causing employee to step back resulting in twisting left knee.

While working with metal working tools, metal shaving went into right eye.

Stepping down off tractor, stepped on rock and twisted knee

Walking across yard and had sharp pain in back and leg and legs buckled.

Employee was welding and fabricating on a pump box. Some time during the night (early morning)a piece of metal dust/sliver got into his right eye.

Pain in lower stomach.

Employee was moving undercarriage rollers and pinched/cut right index finger between roller.

Employee stepped off a pump deck and twisted his ankle.

Employee was transferring excavator teeth from pallet to an empty steel storage drum. While throwing teeth in the drum, employee smashed his finger against the drum.

Employee shut down a 4" pump that had aired up. When disconnecting the discharge hose from the pump, hot water and steam blew back onto the employee's right arm.

Operator was running equipment and complained of back pain.

Lost control of the pressure washer spray wand and it struck him in the face and left eye.

EE was driving truck, there was a curve at the bottom of the hill; he was going too fast and the truck landed on its side.

EE came off night shift and complained of stomach/abdominal pain.

Employee says he fell off fuel truck and injured hand on walkway grating. employee quit job and picked up paycheck and then mentioned he wanted to have fingers x rays.

EE WAS PRYING OUT A ROCK STUCK IN DUMP CHUTE WHEN BAR WAS STRUCK BY ANOTHER ROCK & FLIPPED INTO JAW & SLAMMED THE OTHER SIDE OF THE JAW AGAINST A WALL.

AN EE WAS DRIVING TOO FAST ON A 4 WHEELER IN AN WEAR UNFIMILIAR TOO HIM. WHERE WERE A WATER DAMAGE DITCH 2.5" DEEP. WHEN HE HIT THE DITCH HE LOST CONTROL AND HIS RT LEG WENT UNDER 4 WHEELER RE AR TIRE.

SLIPPED WHILE GETTING OFF OF D9 TRACTOR AND TWISTED KNEE.

EMPLOYEE WAS PICKING UP EMPTY OIL BARRELS WHEN HE FELT TEARING SENSATION IN GROIN AREA. THIS CAUSED HIM MUCH PAIN.

EMPLOYEE WAS WELDING IN BACK OF SCREEN. A PIECE OF SLAG FLEW INTO HIS EYE.

EE WAS HELPING PUT OUT A FIRE IN SMALL BLDG & GOT ANSUL SUSPENDED IN AIR IN HIS RIGHT EYE. IRRITATED EYE DUE TO ABRASION OF ANSUL

EMP LIFTED BOX FROM SMALL AC INTO TRUCK. EMP FIRST NOTICED INJURY ONE DAY LATER.

EE WAS USING FORLIFT TO PLACE ITEMS DOWN. WHEN WEIGHT AND OR PRESSURE WAS REMOVED OFF OF THE FORKLIFT IT CAUSED FORKLIFT TO SWING AROUND. SLIDE SIDEWAYS AND HIT EE. HE DID HTINK HE WAS CLEAR F ROM THE MACHINERY.

EE WAS CHANING THE NOZZLE AT THE END OF THE SPRAY GUN. TRIGGER WAS ACCIDENTLY OPENED AND INJECTED WATER AND DILUTED SOAP INTO THE BASE OF THE MIDDLE FINGER LEFT HAND.

EE SAID THAT HE WAS PULLING AFUEL HOSE BACK INTO THE BOAT AND SLIPPED IN THE MUD CAUSING HIM TO FALL AND INJURED KNEE.

FELL ON FROZEN GROUND IN FRONT OF DREDGE LANDED ON MY KNEE AND MY KNEE IS SWELLING.

EMPLOYEE WAS UNLOADING FREIGHT FROM AIRPLACE USING ACABLE WINCH TO LEG FREIGHT DOWN RAMP WITH PALLET JACK CABLE CAME OFF AND EMPLOYEYE WAS UNABLE TO GET OUT FO WAY PALLET JACK. JACK KNIFED AN D PINNED HIM AGAINST WALL PALLET JACK HIT HIM ON LOWER LEGS. BROKE RIGHT LEG ABOVE ANKLE.

EMPLOYEE FELL OFF A PUMP WHILE WORKING ON IT. HE LANDED ON HIS TAIL BONE ACROSS A 2" PIPE. HE BROKE HIS TAILBONE.

EMPLOYEE IS BELIVED TO HAVE A HERNIA. EMPLOYEE IS UNSURE EXACTLY WHEN THE ACCIDENT HAPPENED. EMPLOYEE DOES LIFT, STREDCH & BEND THROUGH OUT THE WORK DAY.

EMPLOYEE WAS WALKING ON WALKWAY & SLIPPED & FELL & HURT HIS BACK.

EMPLOYEE WAS CUTTING LUMBER ON TABLE SAW. PIECE KICKED BACK, THROWING HAND BACK INTO BLADE AND CUTTING LEFT HAND RING FINGER.

EMPLOYEE WAS STARTING PONY MOTOR & MOTOR BACK FEED THROWING HANDLE BACK STRIKING HIS UPPER ARM.

EMPLOYEE WAS MOVING A 2" X 12" X 20' ROUGH CUT BOARD SLIPPED & FELL ON BOARD HE WAS CARRYING. EMLOYEE BRUISED HIS RIBS.
