Employee's right hand came into contact with a running chainsaw with the bar guard still on. Employee is not sure if they squeezed the guard to make the chain bite in and it chewed through the guard contacting ring finger, middle finger and thumb or if it came off as they were grabbing it.

While climbing a fixed ladder mounted to the side of the sand box, employee lost footing and slipped and fell down the ladder. Employee caught self with both hands on the bottom step resulting in hypertension of hands at the wrist joint. Employee's left thumb also caught the bottom step and was hyperextended as well. This resulted in a hand/thumb injury.

Employee improperly departed Morooka Track rig by jumping from rear platform. After making contact with the ground EE felt left knee "give out".
Employee was using chain saw to cut down a tree. The saw kicked back and made contact with their right knee resulting in a laceration on the anterior surface of the right knee.
Employee was attempting to re-engage thrust rollers onto a thrust ring on wash plant trommel. An excavator utilizing 3/8" chains was connected to the thrust roller stanchion to pull it back when the chain broke and employee was struck on the side of the head by either the flying chain or a chain hook. EE was medevac'd to Hospital where EE was diagnosed with a fractured skull.

Employee was removing bolt from water pipeline discharge flange with a 15" crescent. Discharge hose was attached to the pump which was suspended about 4-5 feet. While applying pressure to loosen bolt/nut the wrench unexpectedly slipped off of the nut resulting in handle hitting the employee in the forehead.
While hooking up a rigging chain to a steel plate that was being prepared to be moved, the employee yanked on the chain to remove the slack, which caused the plate to fall from a leaning vertical position and hit the outer surface of employees left leg knocking ee to the ground.
While performing routine maintenance on water pump, employee smashed finger while trying to remove guard resulting in a laceration on the tip of the right thumb.

While operating router tool, employee lost grip on tool and cut palm of hand on router bit.
Employee was driving track vehicle when cargo load required adjustment. While adjusting cargo load employee slipped & fell forward landing in prone position on deck. Due to incline position of vehicle, employee then rolled off deck & fell appx 5 feet & landed on spongy vegetated ground resulting in injury to left shoulder. Employee was wearing all required PPE at time of event.
Employee egressed off drill rig by improperly climbing around safety handrail & step down via front bumper. Bumper was not equipped with grip tape as it is not meant for use as step. Employee used 3 points of contact but was unable to maintain grip after foot slipped. Employee reports backwards fall from 4 feet onto flat gravel surface landing on back & hitting head on ground.
Employee was climbing down from the Morooka rig when weld attaching safety chain to support post broke resulting in a fall from appx 4 feet high onto a flat ground level surface which was frozen and covered with packed snow. Employee landed directly on tail bone/lower back. Employee was wearing all required PPE including a hardhat, and reports no head/upper extremity involvement.
Initially employee thought they strained their back. After seeking medical attention it has been determined employee has a Herniated Disc in their back.

Employee shut down a 4" pump that had aired up. When disconnecting the discharge hose from the pump, hot water and steam blew back onto the employee's right arm.

EMPLOYEE FELL OFF A PUMP WHILE WORKING ON IT. HE LANDED ON HIS TAIL BONE ACROSS A 2" PIPE. HE BROKE HIS TAILBONE.
EMPLOYEE IS BELIVED TO HAVE A HERNIA. EMPLOYEE IS UNSURE EXACTLY WHEN THE ACCIDENT HAPPENED. EMPLOYEE DOES LIFT, STREDCH & BEND THROUGH OUT THE WORK DAY.
