EE was trying to remove a tree that was stuck at the underside of a crusher, on top of a belt. While pulling on the tree, EE felt pain on the right side of abdomen, and the injury resulted in a strain. The injury did not meet reportable criteria until after EE's follow up visit on 01/22/2024 when ee was issued a work restriction of no shoveling.

The tailgate latch was stuck, and EE went to get it unstuck. In the process of doing so, the tailgate went shut and EE's finger got caught in it.

EE has suffered a standard threshold shift in hearing compared to their baseline. EE has been retrained on proper use of hearing protection and has also went over the hearing conservation plan with a safety team member.

Employee was cutting a conveyor belt with a utility knife.

Employee was attempting to remove material by prying that was stuck in chute.

Employee was cutting down a broken tree. It twisted and hit employee on the side of the head.

Employee was cleaning crusher when his foot got caught between the motor and guard.

Employee was lifting a pump with other employees and strained his leg. Employees on site were previously instructed to use a come-along to lift anything heavy.

Employee was lifting an access door off of the shaker and strained his left shoulder.

Employee was installing a new 20HP electric motor on the #3 conveyor. A 3/4 ton chain hoist was being used to lift the motor into place. The safety latch on the hook failed resulting in the motor striking the employees left forearm.

Employee was lifting crusher parts. This job is done by more than one person.

Injured was cutting tree's for firewood and a wind gust caused the striking tree to fall in a unexpected direction thus striking the tree cutter. Injured was not an employee of the mine.

The employee was assisting a contract welder who was working in a feeder area of the plant. The injured employee was acting as a spotter in the event that the welder needed help due to working in a confined space.

The employee was lifting steel plates.

Employee fell thru a elevated walkway when the steel grating approx. 22" wide and 36" long he stepped on dislodged. the walkway was supported on both side with 3"x3" angle iron and was adjacent to a crusher. It appears the weld broke as a result of vibration.

Installing replace "shoes" on VSI crusher and one fell from cart, used to transport them, onto his foot.

Employee was shoveling from under belt for several hours before pain & tingling in hands/fingers began. He did not report any problem until 7/5/06, we then sent him for a medical examination. *Possible carpal tunnel syndrome.

LABORING AT PLANT REPAIRS--TWISTED BODY.

WAS CLIMBING UP LADDER INTO CAB OF TRUCK; WEARNING MUDDY SHOES; FOOT SLIPPED OFF RUNG; FELL TO GROUND.

EE WAS STANDING ON DECK ABOVE CLASSIFIER CHECKING HOW GATE WAS WORKING WHILE LEANING AGAINST METAL RESTRAINING RAILING. WELD FAILED & RAILING GAVE WAS CAUSING EE TO FALL INTO CLASSIFIER TANK A PPROX 6-8' BELOW, SUFFERING HEAD, FACIAL & LOWERBACK INJURIES.

EMPLOYEE WAS ASSISTING IN KNOCKING HOLES IN A CONCRETE PIPE WITH A JACK HAMMER.EMPLOYEE WAS HOLDING THE BIT IN PLACE.WHEN THE BIT BROKE THROUGH THE PIPE THE HAMMER DROPPED AGAINST THE PIPE MAS HING EMPLOYEES FINGER.

EMPLOYEE WAS DRAGGING A CABLE FOR A CRANE WHEN HE FELT PAIN IN THE LEFT KNEE.

EMPLOYEE WAS LIFTING A SHOE FOR A CRUSHER WHEN HE FELT SOMETHING PULL IN HIS GROIN.

THE EMPLOYEE WAS CLEANING UNDER A CONVEYOR WITH A SHOVEL WHEN HIS HAND STRUCK THE CONVEYOR ROLL FRAME LACERATING HIS THUMB.

THE EMPLOYEE WAS CHANGING WEAR SHOES IN THE VSI CRUSHER WHEN HE LIFTED THE SHOES AND TWISTED HIS BACK WHILE LIFTING A BACK STRAIN.

EMPLOYEE WAS ASCENDING A STAIRWAY WHEN HE SLIPPED & FELL UP THE STAIRS HITTING HIS KNEE ON THE STEP CAUSING A CONTUSION TO THE KNEE

CREW HAD REPLACED BEARING IN HEAD PULLEY #5.INJURED EMPLOYEE HAD REMOVED COME-A-LONG FROM CRANE HOOK AND WAS CLIMBING OVER SPEED REDUCER,FELL IN AN UNKNOWN MANNER.HE BRUSHED THE BACK OF FOREMA N(NAME) WHO WAS STANDING ON THE 42" WIDE CATWALK.FROM THERE HE FELL 34' TO THE GROUND,MISSING THE CATWALK ON THE NEXT LOWER LEVEL AND LANDING ON THE GROUND.

CLEANING DEBRIS FROM LOG WASHER, STANDING ON SHAFT. FOOT SLIPPED FROM SHAFT, CAUSING KNEE TO TWIST.

DR CLAIMS SIDE 7HRUST ON HOIST LEVER CAUSED INFLAMMATION OF ELBOW.ARM HAS BEEN IMMOBILIZED IN CAST.

LAYING NEW PIPE3IN DITCH TO REPLACE LEAKING WATER PIPE.NO SPECIFIC INCIDENT.
