On 12/22/2021 the employee sustained a stroke on site and later passed away on January 5, 2022 due to natural cause.

Employee was walking down the catwalk of S-3, when EE turned towards the screen, placing weight on left foot when EE's left ankle rolled. The employee was prescribed work restrictions which are being accommodated.

This report is being submitted at MSHA's request on 2/14/2020 as a result of an incident on 2/7/2020. A third party hauler suffered a medical emergency at the mine entrance. Emergency services were called and hauler was taken offsite by an ambulance.

On 12/11/15 the employee was taken to the company clinic, was prescribed physical therapy and released to full duty. The employee hopped over a water puddle and struck his head on the bottom of the sand screw tank. He was diagnosed with a neck sprain and shoulder strain. The employee reported the incident on 12/11/15.

The employee was taken to the Company clinic on 2/2/15, was provided wrist and elbow braces along with physical therapy. Employee alleged pain in L-shoulder, elbow, hand, wrist and neck from repeated opening of a window to issue tickets. She was diagnosed with L-shoulder sprain/strain, neck sprain, L-hand/wrist tenosynovitis, L-elbow/forearm synovitis, L-upper limb mononeuritis.

On 3/3/09 the employee was taken to the hospital and was admitted for more than 24 hours. While the employee was descending ladder on loader, his leg gave out and he fell approximately 4 feet resulting in R-leg and pelvis fractures. (Employee has an underlying medical condition which may have been a causal factor).

EE WENT TO DR ON 4-28-00 AND WAS PALCED ON RESTRICTED DUTY. WHILE USING A PICK UNDER SHAKER, PICK MISSED TARGET, ARM OVEREXTENDED AND TENDON IN RIGHT ARM RIPPED.

SLIPPED ON WET GRASS WHILE OPENING GATE.

LIFTING WIRE CLOTH FROM ROCK SCREEN TO WALKWAY. FELT SHARP TWINGLE IN BACK.

OPERATING WATER TRUCK-FELT PAIN IN NECK.

MAN WAS SHOVELING UNDER CONVEYOR #29-FELT RIP IN STOMACH AND BULGE -APPARENT HERNIA.

STEPPED OUT OF LOADER ONTO LADDER SLIPPED & FELL JAMMING VERTEBRA CLOSEST TO HIP

CRUSHER I BOLT SLIPPED FROM GRASP. FINGER WAS CAUGHT BETWEEN I BOLT AND CRUSHER RIM. LAST JOINT OF RING FINGER WAS BROKEN.

FUELING SCRAPER, SLIPPED AND FELL- PULLED MUSCLE IN RIGHT SHOULDER.

STEPPED OUT OF LOADER AT END OF SHIFT STEPPED ON ROCK, SPRAINED ANKLE

MAN WAS JACKING UP CONVEYOR TO LEVEL POSITION, JACK SLIPPED, HANDLE OF JACK HIT MAN IN AREA OF RIGHT EYE. FOUR STITCHES WERE TAKEN IN FACIAL SKI AT OUTSIDE EDGE OF THE EYE.

ADJUSTING NUT O3 CONE CRUSHER SHIFTED PINCHING LEFT FOOT AGAINST CRUSHER MAIN FRAME. LEFT FOOT WAS SEVERELY BRUISED

BACK STRAIN CAU8ED BY PULLING ON HANDLE OF 3/4" RATCHET USED IN TIGHTENING BOLTS ON A VIBRATORY FEEDER IN A SAND AND GRAVEL PLANT. EMPLOYEE REPORTED STRAIN TO FOREMAN AFTER TWO HOURS WORK THE NEXT DAY 10-01187.
