When entering the driver's side of EE's car EE noticed dirt/sand on EE's seat EE stopped EE's entry action and tried to give the seat a quick brush with EE's hand, when beginning that motion EE hit the upper car door opening with the top of EE's head, jarring EE and closing EE's jaw quickly to "rattle" EE's teeth.

Employee was removing the parking brake assembly from loader, it was too heavy for him to hold and pinched his hand on the machine frame causing a laceration on his palm requiring 8 stitches. No time lost.

Employee was splicing conveyor belt with proper splicing equipment. As the belt was being tightened with comealongs the clamp became wedged against a trough roll. It was not noticed that it was stuck. As the belt was pulled tighter it finally let loose and slid off the end of the clamping bar struck him in the right shin.
Repairing bin got a piece of rust in his eye. went to eye doctor had it removed was given antibiotics. Was released to return to work. Had no medical back up to be out of work however he did not show for work on 4/5/13.

While opening the Cat 980K loader door, his hand slipped while reaching for the door and he fell injuring his right ankle. (strain)

He was fitting steel into a chute that he was building. He strained his back climbing up to the chute work area where he was installing the new steel.

Exiting machine, his foot slipped on track. He managed to catch himself before falling to the ground. Someone had located a truck mirror on grab rail on cab; this mirror has been removed.

CLIMBING OUT OF HIS TANDEM DUMP TRUCK TO CHECK TRUCK AND FOOT SLIPPED ON THE FUEL TANK STEP. HE GRABBED THE SAFETY RAIL AND STRAINED HIS RIGHT ELBOW.

EE CLIMBING OFF HIS LOADER WHEN HIS RIGHT FOOT CAUGHT IN THE BOTTOM STEP FLEXIBLE STEP CAUSING HIS RIGHT SHIN TO STRIKE METAL STEP ABOVE THE BOTTOM STEP.
EE AND PARTNER LIFTED A 200# FLYWHEEL TO 0LACE ON THE SHAFT OF A SCREENER. THE EE HAD BEEN TOLD TO USE AVAILABLE HOISTING EQUIPMENT TO LIFT THE FLYWHEEL BUT DECIDED TO LIFT MANUALLY.

EE WAS CLEARING ROCK FROM PLUGGED JAW CRUSHER WHEN HIS FINGER WAS STRUCK BETWEEN TWO ROCKS RESULTING IN A FRACTURE TO HIS RIGHT LITTLE FINGER. EE WAS WEARING ALL REQUIRED PPE INCLUDING GLOVES.
