AFTER MSHA INVESTIGALTION, NO CAUSE OR EQUI PMENT FAILURE HAS BEEN DETERMINED. VEHICLE COMPLETELY TOTALED. EMPLOYEE RECEIVED MASSIVE TRAUMA INJURIES TO HEAD & BODY.
