Mostrando los 14Employee was climbing down ladder of 988F loader when employee missed ladder with employee's left hand putting all employee's weight on employee's right arm. Employee has a right bicep tear (employee weights 500 plus)
Employee was working on AC unit for a 988F loader. EE was inside the cab cleaning dust out of the vent when EE cut finger requiring stitches.
On 1/28/16 we had annual refresher, EE states chest was hurting some, but did not go to Doctor. 1/29/16 EE went to work then chest still hurting so called spouse, spouse took EE to ER. We called MSHA 800 and reported. This injury was not job related, EE had inflammation of the cartilage (bone that attaches the ribs to breastbone)
At 1:50pm employee called foreman to help him down from crusher platform. While cleaning up for crusher maintenance employee slipped, his foot hung between conduit and I-beam, his body weight falling against his pinned leg causing leg injury. He was taken to hospital where it was later determined to be fractured.
Employee had been getting in and out of service truck. He went to get out of truck when his right foot slipped off running board but his left foot stayed on causing employee to hyper extend left knee. Ground was muddy.
Employee was using a utility knife to cut belt for skirtboard when knife slipped causing a laceration to his left pinky finger that required 4 stitches.
Employee was working on conveyor, he was using a hammer to put a bolt throught a turnbuckle when he missed bolt and hit his left thumb causing a laceration requiring sutures.
Employee was carrying a 4" pipe about 8' long. He went to lay pipe on forks of a lull when he caught finger between pipe and end of fork on lull mashing his finger. He fractured tip of his little finger.
Employee was using a pocket knife, to cut tape that was holding stirring paddle, off a bucket of backing. The knife slipped, causing a laceration to his left thumb.
EMPLOYEE WAS WALKING OUT OF SECONDARY CRUSHER BOOTH WHEN HE SLIPPED AND HIT HIS MOUTH ON A ROCK CAUSING A CHIPPED TOOTH TO THE BACK RIGHT SIDE.
EMPLOYEE WAS HELPING CO-WORKER SET CRUSHER LID BACK ON CRUSHER. WHEN HE WAS POSITIONING THE LID, HE CAUGHT HIS RT. RING FINGER UNDER THE LID CAUSING A FRACTURE & FINGERNAIL REMOVAL WHICH REQUIRED STITCHES.
EE WAS CLEANING UP AROUND PLANT WITH SKID LOADER. HE WAS EXITING CAB WHEN HE SLIPPED ON THE BUCKET. EE ATTEMPTED TO CATCH HIMSELF WITH HIS LEFT ARM, CAUSING AN INJURY TO HIS WRIST. AT THIS TIME THEY DO NOT KNOW IF HE FRACTURED HIS WRIST OR STRAINED IT. THEY PUT EE IN A SPLINT UNTIL THEY CAN DO AN MRI. HE IS ON LIGHT DUTY.
EE WAS OPERATING BACKHOE. HE STOPPED WORKING AND WENT TO STEP OFF EQUIPMENT. WHEN HE STEPPED DOWN TO GROUND, HE TWISTED HIS LEFT KNEE. EE REPORTED THE INCIDENT TO HIS FOREMAN, BUT DID NOT GO TO THE DOCTOR UNTIL 8/6/04 (X-RAYS) AND AGAIN ON 8/10/04. STRAINED KNEE. KNEE IMMOBILIZER & LIGHT DUTY WORK. DOCTOR'S REPORT RECEIVED 9/8/04.
