Employee felt sudden sickness. It was reported that the employee bent over and vomited; followed by stomach cramps, dizziness and nausea. Out of precaution the onsite supervisor transported the employee to Urgent Care. Employee was given two IV bags then released.

Mostrando los 4Emp running to the Hawk control panel to shut it down. Approx 10 feet from the control panel, is a small pile of material. The employee tripped on or near this material causing EE to fall headfirst into the top corner of the control panel which is positioned approx 61 3/4" off the ground. The employee sustained a laceration to the top of EE's head resulting in 13 stitches.
Employee was using an 8-10 lbs. hammer to assist with the removal of a tooth off a loader bucket. A piece of the tooth broke off, roughly 2-3 mm in size, and struck the employee on the back lower side of EE's right leg.
On October 31, 2024, at approximately 2:25 PM miners were positioning a new conveyor on site, two welds on the conveyor snapped and the conveyor folded contacting a miner as EE was in position to put the locking pins in place.
