HE WAS CARRYING A HYDRAULIC PUMP FOR AN I-H 560 LOADER FROM THE PARTS ROOM TO THE MECHANICS SHOP WHEN HE GOT TO THE SHOP & SET THE PUMP DOWN HE FELT A PAIN IN HIS LOWER BACK
HE WAS OPERATING WABCO END DUMP WHEN HE SAW STEAM COMING OUT OF RADIATOR. HE THEN STOPPED TRUC & GOT OUT & REMOVED RADIATOR CAP GETTING STRUCK ON RIGHT ARM BY ESCAPING STEAM.
HE WAS DISMOUNTING LOADER WHEN HIS FOOT SLIPPED AND HE LOST HIS BALANCE. HIS RING CAUGHT ON SOMETHING UNKNOWN ON THE STEP OF THE LOADER WHEN HE FELL CAUSING THE AMPUTATION OF HIS LEFT RING FIN GER AT THE SECOND JOINT.
HE WAS SWINGING SLEDGEHAMMER OVERHEAD TO KNOCK COAL LOOSE THAT WAS STICKING TO SIDE OF ROTARY BREAKER CAUSING TISSUE AROUND 7TH VERTEBRAE TO BE PULLED.
EMPL WALKING TO SHOP AT END OF SHIFT.AFTER WALKING A MILE IN BELOW-FREEZING TEMPS,DEVELOPED FROSTBITE IN BOTH FEET.IS A DIABETIC & CAN CONTRACT FROSTBITE EASILY.
HE WAS GRINDING ON PIECE OF METAL WITH DREEMEL TOOL & GOT A SMALL PIECE OF METAL IN HIS RIGHT EYE HE SAID HE WAS WEARING HIS SAFETY GOGGLES
HE WAS USING CUTTING TORCH TO REMOVE GALVANIZED STEEL GUARDRAIL POSTS FROM BRIDGE AND INHALED THE FUMES CAUSING CHEST PAINS.
EMPLOYEE WAS CLEANING AROUND THE ENGINE AREA WITH A RAG. AS HE PASSED NEAR THE ALTERNATOR, WHICH HAD AN ACCEPTABLE BELT GUARD IN PLACE, THE CORNER OF THE RAG GOT SUCKED UP IN THE BELT AND JERK ED HIS ARM BACKWARD CAUSING PULLED MUSCLES IN THE UPPER ARM. THIS ACCIDENT IN NO WAY INVOLVED A VIOLATION OF RULES AND REGULATIONS OR JOB SKILLS, MINER PROFICIENCY, TRAINING AND ATTITUDE.
EMPL INSTALLING CABLE ON DRAGLINE.AS HE TURNED & REACHED FOR WRENCH HE SLIPPED ON MUDDY FLOOR.LOST FOOTING AND LANDED ON ANKLE WITH ALL BODY WEIGHT.

WHEN HE PUT LOADER IN REVERSE TO CLEAN UP MATERIAL HE HAD KNOCKED DOWN ROCK FELL FROM HIGHWALL HITTING TOP OF BUCKET & BOUNCED OVER BACK & CAME THROUGH WINDSHIELD RESULTING IN OPERATOR BEING HIT WITH BROKEN GLASS & ROCK CUTTING HIS RIGHT FOREARM.
EMPLOYEE WAS CLIMBING LADDER TO OPERATORS CAB WITH HIS LUNCH PAIL THERMOS AND PAPER TOWELS IN HIS LEFT HAND AND WAS ONLY USING HIS RIGHT HAND TO HOLD ONTO THE HAND RAIL. HIS FFOOT SLIPPED OFF LADDER RUNG CAUSING HIM TO LOSE HIS BALANCE AND CATCH HIS RIGHT THUMB BENDING IT BACKWARDS AND FRACTURING IT.
MAN WAS GREASING ROLLERS ON LIMA 2400B DRAGLINE NEAR THE SPOIL PILE WHEN A ROCK ROLLED OFF THE SPOIL PILE STRIKING HIM ON THE LEG
WAS OPERATING L2ADER WITH BUCKET IN RAISED POSITION WHEN APPROACHING TRUCK (CAT 773B) WAS PULLING IN UNDER BUCKET TO GET LOADED AND STRUCK LOADER ARM UPSETTING LOADER.
EMPLOYEE WAS MO5ING A PIECE OF CRIBBING AT COAL TIPPLE. AFTER LIFTING AND MOVING THIS CRIBBING HE EXPERIENCED BACK PAIN AND PAIN DOWN LEFT LEG. AFTER PAIN REMAINED FOR A FEW DAYS EMPLOYEE DECI DED TO SEE A DO7TOR. THIS ACCIDENT IN NO WAY INVOLVED A VIOLATION OF RULES & REGULATIONS OR JOB SKILLS, MINER PROFICIENCY, TRAINING AND ATTITUDE.

EMPLOYEE WAS AS8EMBLING A HITCH BLOCK EXTENSION ON 71/2 YD.BUCKET & TWISTED IN SUCH A MANNER AS TO INJURE LOWER BACK.THIS ACCIDENT IN NO WAY INVOLVED A VIOLATION OF RULES AND REGULATIONS OR JO B SKILLS,MINER 1RQFICIENCX,TRAINING AND ATITTUDE.
