Employee was removing rock from stopped jaw crusher when he had a little discomfort in his lower abdomen. He thought it was just a strained muscle.

Worker cut finger while working on D8N dozer. Required seven stitches. Returned to work same day limited duty.

EE was working on an elevated conveyor when he fell from it.

There was screen material in the back of a pickup truck that was being used to build a guard. It was snowing and blowing heavily and the employee got up to go to the pickup. He was walking with his head turned to keep the snow out of his face and ran into the screen material in the back causing a laceration to left side of nose requiring 7 to 8 stitches to close.

This employee and another were welding in the welding shop. Employee went home and started experiencing pain in both eyes. He went to the doctor at 12:00 am and was told he had flash burns to both eyes.

Employee climbed down a ladder at the main plant. When he got to the bottom a rock from above fell approximately 5' and struck him on the hard hat. Employee complains of neck pain and stiffness. The rock was approx 5 inches.

EE claims he hurt his hand while hitting a rock with a sledge hammer.

EE had a 25 dB or greater shift in his hearing in both ears.

EE was changing screens on the plant and strained his back.

EE misjudged distance from last step on loader to the ground.

Employee was working on shaker screen. He had lifted and secured a discharge lip on the top deck of the screen so that he could work on the second deck of the screen. After climbing into begin work on the screen a gust of wind caused the discharge lip above him to fall. The lip struck the employee on the back and caused him to hit his chest on the deck in front of him.

While installing a radiator on a 972 loader, jammed his finger while handling the radiator.

Employee was washing windshield on loader and fell off.

Employee was replacing vertical shaft with finger between pry-bar and shaft when the shaft slipped and pinched finger.

Employee stated she was having pain in her right shoulder and left wrist. She states the pain is from years of repeated motion due to operating a wheel loader.

Injuried employee sitting on water cooler, truck exits plant, rear tire struck a metal bar, base of metal bar struck employee on left leg. This caused a compound fracture.

Employee was running to turn off conveyor belt when he slipped and fell. The employee received a lower back strain from the fall.

CUTTING A PIECE OF BELTING FOR A PATCH, RAZOR KNIFE SLIPPED AND CUT LEFT THUMB

Employee had been shoveling lime abc all day. Upon arriving at home, employee removed boots and rubbed eye.

Employee was climbing into the cab of a piece of equipment when he struck his head on the frame of the cab door. He worked the rest of the shift without telling anyone until the end of his shift. He said he had some slight neck pain but did not require medical attention.

Folding D-10 dozer track for shipment. While folding tracks master link folded under to far. Employee straightend master link with right hand. At this point his left hand was between track pads. The tracks flatened cutting and breaking three fingers on his left hand.

While walking around the water truck, employee slipped in mud resulting in pain to left shoulder and middle of back.

While performing maintenance on a conveyor belt, the knife slipped cutting thumb on left hand.

Employee was improperly lifting a steel pipe, causing lumbar strain.

Employee driving forklift to pallet storage area. While crossing railroad tracks, left fork hit second rail stopping forklift abruptly. Driver's chin struck steering wheel causing EE to bite tongue.

The plant foreman directed ee to remove did old rollers and frames from an old conveyor to a rock conveyor they were rebuilding. he made improve decision by nut asking for help when he discovered that the old 36" troughing roller and frame were heavier than normal.

Two employees were lifting a 108 lb roll of plastic and placing it in a cradle stand. One of the employees missed the cradle when setting it down, the plastic roll twisted, and he dropped his end. The other employee, still holding up his end, was jerked forward and to the left as a result of of the other end falling.

Employee was changing sheer bolt in feeder through access door when door came unlatched and fell striking employee on his right hand.

Foot slipped while exiting water truck and he strained his back.

On August 27, a section of rail was removed from the pallet storage area and backfilled. Rain caused the backfill to compact and subside forming a small pothole. Employee was carrying a finished pallet to the storage yard with a forklift and hit the pothole. This impact caused the forklift to "buck" violently hurting employee's neck.

EE was getting back in the truck. While climbing the access ladder he lost his grip at the third step (approx. 3') falling backwards to the ground and landing on his back, striking his right elbow on the ground causing it to swell instantly. EE was taken to Immanuel hospital in Omaha where x-rays indicated a broken elbow requiring surgery.

EE WAS SEALING BAGS ON INGLETT MACHINE. BAG ON CONVEYOR STARTED TO FALL OVER. EE LEANED OVER TO PICK IT UP. EE WAS USED TO 1 CF BAGS AND DIDN'T ANTICIPATE THE WEIGHT OF 2 CF BAG. EE LIFTED THE BAG AND FELT PAIN IN MIDDLE OF HIS BACK. ON 6/1/04 EE REPORTED THAT HIS BACK WAS WORSE AND TREATMENT WAS NECESSARY.

EMPLOYEE WENT AROUND A CURVE & HIT A BUMP IN THE ROAD & JAMMED HIS BACK. EMPLOYEE STATED THAT THE SEAT ON THE LOADER HIT HARD. IT WAS DETERMINED ON 04/19/04 THAT THIS MAY BE A COMP. INJURY SO I AM REPORTING IT TODAY.

EE WAS WELDING ON NEW TRUCK DUMP WHEN A PIECE OF SLAG GOT UP UNDER HIS WELDING HOOD AND LANDED IN HIS LEFT EYE. EE WAS WELDING OVERHEAD WHEN ACCIDENT HAPPENED.

EMPLOYEE WAS USING A CUTTING TORCH TO CUT SOME STEEL. WHEN HIS GLOVE BECAME LOOSE ON HIS HAND HE RELEASED PRESSURE ON THE TORCH TO TIGHTEN HIS GLOVE AND THE TORCH FLIPPED IN HIS HAND DUE TO A TWIST IN THE HOSE CAUSING A BURN TO LEFT FOREARM.

EE CUT BEARING OFF CYLINDER AND TOUCHED RIGHT HAND PALM ON HOT CYLINDER.

EE WAS MAKING WELDING REPAIRS TO A HAND RAIL AT THE EAST CRUSHER, WHEN A SPARK CREATED FROM THE WELDING PROCESS ENTERED HIS EAR.

EMPLOYEE WAS FEEDING BAG FILM INTO BAG ADVANCE BELTS ON BAG MACHINE. WHEN HE ADVANCED MACHINE, IT PULLED HIS MIDDLE FINGER (LEFT HAND) INTO "V" BELTS. HE WAS HOLDING CRIMPED END ON TOP INSTEAD OF AT BOTTOM AWAY FROM PINCH POINT. I BANDAGED HIS FINGER - EMPLOYEE FINISHED HIS SHIFT. HE WAS SORE FOR NEXT FEW DAYS. NO WORK RESTRICTION, NO LOST TIME. HE WENT TO DR ON HIS OWN RECD STITCH

EE CHECKING FUEL IN COMPRESSOR IN BED OF SERVICE TRUCK, SLIPPED AND STRUCK GROIN ON TRUCK BED CAUSING LACERATION TO SCROTUM AND URETHRAL BLEEDING.

EE WORKING WITH TWO OTHER EMPLOYEES ATTEMPTING TO STAND LIGHT POLE INTO RECEIVER WHEN POLE SLID OFF RECEIVER AND FELL, BOUNCED UP AND STRUCK EE UNDER CHIN CAUSING LACERATION AND BROKEN DENTURE S.

WHILE ON CATWALK TIGHTENING BOLT ON APRON PAN EE LEANED OVER A PIECE OF I-BEAM SUPPORT WHILE PULLING ON A WRENCH HE FORCED HIS LOWER LEFT RIBS AGAINST THE I-BEAM RESULTING IN BRUISED RIB
