Mostrando los 20Employee struck a drill steel coupler with a hammer trying to remove a broken piece of strikerbar that had broken off in side of the coupler. On the second strike of the hammer an approximately 2 inch long sliver broke off the strikerbar section and impaled about 6 inches above the right wrist of the employee. The employee removed the debris and needed 2 stitches at medical clinic.
Employee was loading trucks in pit and load shifted and material (shale) came over the bucket striking the windshield and came into cab and broke the dash. Dash cut caused a laceration and contusions to left leg and left little finger requiring two stitches to finger and leg.
Employee was closing clamp on discharge hose on 4' Thompson pump and the camlock clamp broke. This cut the employee's left hand, requiring 8 stitches.
Employee was pulling screens off a screen box. Screen broke free and he hit his elbow on box. Reported to his supervisor but refused medical treatment. Requested medical evaluation on 2/6/2015. Diagnosis: Lateral Epicondylitis (aka tennis elbow).
Burns to left hand and arm. Also to neck from Radiator Cap on Komatsu PC600.
Cutting plate with torch, block under plate kicked out, letting plate strike his right middle finger and took off the nail.
Employee was Lifting buckets over a 6 hour period when he developed pain in his right hip and right groin region. This did not become reportable until 11/7/12 when he was placed on restricted duty.

EE cut his right leg above the knee. He was cutting flashing rubber with utility knife.
Operator thought he was having a Heart Attack. Taken to the hospital and confirmed he was not having a Heart Attack. Kept overnight for observation.
While operator was climbing down from screen, he hit his tailbone on a lifting hook.
Loading anfo bags with dolly onto powder truck. Dolly failed causing 9 bags and dolly to come back on operator and tail bone landed on corner of plastic pallet.
Cone crusher lost clamp pressure and employee was trying to keep rock from gravity feeding. The chute that feeds the cone fell off and hit his left index finger at lower half of the nail causing a cut.
Employee was removing drive shaft from transmission on a F350 ford. While using a punch and a hammer. He missed the punch and hit his thumb on his left hand. Causing a cut on his thumb.
Employee complained about severe pain in left chest area and side. This is not work-related. EE is having tests run and results are not known at this time.
INDIVIDUAL STEPPED TO THE EDGE OF A STEP BENCH, SLIPPED ON LOOSE GRAVEL AND FELL 9'2". THE FALL OCCURRED WHILE HE WAS SUPERVISING A CREW SETTING CONCRETE FORMS ON THE LOWER BENCH. A SAFETY BAR RICADE FENCE HAD BEEN TAKEN DOWN TO MOVE MATERIALS FOR THE CONCRETE WORK AND NOT REESTABLISHED.
WORKER HAD BEEN WELDING AND STOPPED TO RAISE WELDING HOOD AND A FOREIGN BODY FELL INTO LEFT EYE.
