Employee inflating airbag to move wall and natural seam in rock shifted rock causing a tear in bag and air released through tear blowing dust/dirt in employees eyes.
Employee was breaking rock and he slipped and cut his leg on granite
He was setting up wire saw when a wall was being pulled down and a pc came loose once wall fell and struck him in the elbow

Employee slipped on mud and fell (same level) hurting his right upper thigh.
Employee was lining up an I-beam when it slipped and hit his finger.
Employee was on ladder propped against loader. At top of ladder went to step off onto loader and ladder slipped causing him to fall 7-0 ft to ground. Right knee injury.
Aggravation to feet from previous incident, 9/6/11 dr note to restrict to sit work, worked 9/7/11 sitting, Family dr appt 9/8-12/11. returned 9/13/11 and no sit work available. Out till dr releases.
He was putting a wedge in hammar using a 2lb hammer, hit his left fore thumb area causing laceration.
Employee was clearing debri and trees around transformer pole. He cut chain saw off, moved against his leg before chain had stopped spinning.
Using sledge hammer to drive wedges missed wedge and hit rock - hammer bounced off rock and hit him in nose. Was in a kneeling position.
Employee pulling on wire for wire saw & felt a pain in lower abdomen.
Silicosis
Using a sledge hammer to drive wedges and felt his back pop. Went to Dr., Dr put on light duty for 3 weeks. P.T. for 3 weeks as well.
Employee was pulling on airline and a knife blade was hung up on line and punctured his forearm.
He was walking across stone & slipped on mud, falling. Hit his right arm & shoulder. Went to Dr who said sprain, no recheck unless problems. He wanted to go back to Dr on 10/2/08. Had a note to return 10/9/08 for recheck. Went back to Dr 10/8/08 with lumbar strain & high restrictions with no walking. Out as of 10/9/08.
Employee was hitting wedge with hammer & missed wedge hitting rock. A piece of rock chipped off & came back hitting him in the eye knocking off his glasses.
Employee injured right shoulder-sprain and has been through dr visits and found a rotor cup tear out for surgery on 11/17/08. moving items/with hand truck when weight shifted tilting hand truck and pulling on shoulder.
Employee was operating 90 ton truck crane. When lifting a block to lower the main fall, brake locked and would not release load. Block swung out past riggers on crane causing load to shift crane and caused a rocking motion. Operator jumped out of crane onto granite. This crane was being operated from bottom of quarry pit.
EE fell off ledge, 12' to next ledge, catching himself on his knees and hands.
Employee was grinding on derrick swing shaft with 2 other ee for repairs when a piece of metal flew up under his glasses.
He was sitting on rock, drilling a dog hole, & when he got downoff the rock, one foot went between 2 rocks & when he turned, he twisted right knee. Taking meds for inflamation.
He was cutting aluminum air line & a piece of aluminum got into his eye. On 2-19-07 he said was still bothering him & went to Dr.
Hit pulling dog with hammer to set dog in place, to put granite block from floor. Small piece of metal struck in nose and would not come out. Piece of metal was approx. 1/8" long.
Drilling & rock dust came up & into eye. Had safety glasses on.
EE was using burner torch and stepped back with left foot into a hole and hyperextened his big toe on his left foot.

EMPLOYEE WAS WALKING FROM COMPRESSOR HOUSE TO PIT AND SLIPPED ON WET METAL RAIL AND FELL TWISTING HIS KNEE AND HEARD IT POP. LIMITED TO CLIMBING LADDERS.
Exposed to large impact wrench while holding tire for support while operator working wrench and noise and air blew in left ear. Report due to audiogram on 9/12/05.
EE WAS USING A CHANNEL TORCH & HOT ROCK FLEW UP UNDER HIS GLASSES TO THE LEFT SIDE OF HIS NOSE AREA AT CORNER (INSIDE) OF EYE. WHEN HE REMOVED HIS GLASSES THE HOT ROCK MELTED TO FRAME & PULLED SKIN FROM NOSE OFF.
EE was hammering a wedge into a piece of granite when the wedge flew up and hit EE in upper lip.
EMPLOYEE WAS DRILLING A LIFT HOLE IN GRANITE BLOCK WHEN PART OF IT SHEARED OFF CAUSING EMPLOYEE & ROCK TO FALL APPROX. 10'. EE HAD HIS SAFETY BELT ON WHICH KEPT HIM FROM HITTING THE BOTTOM.
DOGS SLIPPED ON BLOCK CAUSING A SMALL PIECE (7" X 5" X 2 1/2) TO TRAVEL APPROX 30', STRIKING EE IN LEFT FOREARM.
DOGGING A HOLE WITH PLUG & DRILL WHEN DRILL SLIPPED & FELL STRIKING THE EMPLOYEE ON HIS RIGHT CALF.
EMPLOYEE WAS WALKING ON LEDGE, SLIPPED AND LOST HIS BALANCE AND FELL DOWN BRUISING HIS TAIL BONE.
EMPLOYEE WAS HOOKING UP A BLOCK WHEN HE PULLED THE CHAIN AND HOOK TO ONE END. HE TURNED IN AN AWKWARD POSITION, SPRAINING HIS BACK.
Operator was operating crane with left hand when a lever popped back striking his hand causing a sprain.
Employee bent over to pick up pump and his knee popped. EE did not tell anyone until 9-9-04 at 8:00 am. I took him to the doctor at 1:30 pm and ee is on light duty.
Employee step down off of piece of granite foot slip-causing rt. forearm to brush granite wall cutting his rt. forearm
Moving ladder when it slipped, striking ee on rt forehead causing a laceration that took 5 stitches.
EMPLOYEE THOUGHT THE MACHINE WAS SHUT OFF. WHEN HE UNJAMMED THE TIP IT STARTED UP CAUSING HIS LEFT RING FINGER TO BE PINCHED.
EE was lighting had held burner when it back fired and flume shot back and burn his left index finger.
EE WAS HOOKING CHAIN TO PAN WHEN CHAIN TIGHTENED UP AND WRAPPED AROUND HIS HAND - CAUSING IT TO BE BRUISED.
EMPLOYEE HAD A SEIZURE IN THE OFFICE.
EE WAS HOOKING DOGS ONTO BLOCK WHEN LOST BALANCE ON LADDER AND SLIPPED & FELL APPROX 3 FT SPRAINING HIS LEFT KNEE.
