When employee turned in FMLA/Disability paperwork on 5/31/17 for upcoming knee surgery, they had noted on the paperwork that the injury occurred at work. When employee requested time off in Feb for a Dr appointment for knee pain, they were asked it if happened at work and they responded "No".

TOOK SHORT TERM DISABILITY (LAST DAY OF WORK 5/19/03). TOOK LONG TERM DISABILITY (STARTED AUG. 10, 2003).
NOTE: HAND INJURY OF 2/20/03 WAS NOT HEALING AS EXPECTED. DECISION WAS MADE TO OPERATE, RESULTING IN EE BEING OFF WORK FROM 8/25/03 TILL 10/13/03 RETURN TO WORK DATE.
