An employee was throwing a breaker when it arc flashed. Employee received minor burns to face and hands.
Employee was on top of the railroad car with his harness intact and roped. He lifted the pole into the air, at that point the power line arced a discharge towards his pole. He stood straight up and fell off the railcar. Received 3 burns, chest, right arm, left leg. The electricity exited his body through his left ankle.
MCC went to ground. Power was turned off and breaker was repaired. When MCC was renergized, the breaker blew and caused a flash burn to hand. New breaker was believed to be faulty.
EE HAD DUMPED A LOAD OF MATERIAL FROM THE DUMP TRAILER. EE WAS IN PROCESS OF GETTING RESIDUAL MATERIAL FROM TRAILER WHILE IT WAS IN THE ELEVATED POSITION THE TRAILER CONTACTED POWER LINES EE HAND, LEFT HAND ON CONTROL VALVE & WAS ELECTROCUTED. RECD BURNS TO BOTH KNEES, LEFT HAND & LEFT FOOT
