Employee was plugging up a new set of scoop battery jumpers when it arced and flashed causing his eyes to burn. Employee was treated at Raleigh General Hospital and was treated with eye drops
Employee was at #8 belt heads line splitter waiting for power to be set up outby his location. When power was supplied to the kva it created an arc. He was standing about 5' from flash. His eyes were flushed and he was given eye drops. He returned to work on his next shift. The arc was created by a loose phase lead.
Electrician was helping scoop operator change batteries. EE had female end of jumpers and was connection it to scoop when wiring pulled loose inside bell creating an arc. Palm of his left hand was burned. Was treated and released at CAMC. He returned on his net shift. He followed-up at St. Marys Burn Clinic. The doctor removed dead tissue with no infection and released him.
Power knocked on CM (JM6432). Operator and scoop man were tightening the waterline swivel near a damaged place in the cable. Someone reset the circuit breaker and cable went phase to phase creating an arc. The scoop operator may have received. a shock when the arc occurred as he stumbled away from the cable. EE was kept in the hospital two days for observation.
EE was installing a fuel tank on a tractor inside the truck shop, when lightening struck a nearby transformer on a power pole. The electrical charge traveled down the power pole and disbursed out through the ground, making contact with EE. He was transported to CAMC General Hospital, evaluated, given fluids as a precaution and released back to duty.
Injury was changing a valve insert in Komatsu 930 E Rock Truck when a bolt of lightening struck the ground approximately 60 feet from him. The electricity traveled through the ground and entered his body at the left shoulder and exited through both of his feet causing burns. He was transported to CAMC General Hospital, evaluated, monitored and released.
Employee was troubleshooting the contactors, when the miner was energized the contactors blew up, causing flash burns to ee's eyes.
EE was helping assist the foreman in hanging a D-Box cable whe EE grabbed the cable he came into contact with a bad place in the cable and was shocked. EE was checked by Ambulance personell and was deemed okay with no medical problems.
EE WAS BACKING THE SCOOP DOWN THE #2 ENTRY TOWARD SECTION. HE BACKED INTO A CROSSCUT TO TURN AROUND AND STRUCK A SET OF SCOOP BATTERIES THAT WERE ON CHARGE. WHEN HE GOT OUT TO CHECK HE PLACED HIS HAND ON THE SCOOP CANOPY AND RCVD. A SHOCK.
EMPLOYEE WAS BLOCKING THE SECTION FEEDER AT THE SAME TIME THE MAINTANENCE CHIEF WAS TROUBLE SHOOTING THE FEEDER PANEL. EMPLOYEE HAD BEEN WARNED TO STAY CLEAR. WHEN EMPLOYEE RAISED UP FROM THE FEEDER, HE USED HIS HAND TO PULL HIM- SELF UP AND CONTACTED THE FEEDER PANEL WHICH RESULTED IN AN ELECTRICAL SHOCK.
THE EMPLOYEE RECEIVED 2ND DEGREE BURNS TO THE HANDS AND FACE WHEN A MOLDED CASE CIRCUIT BREAKER (400 AMPS) ARCHED AND FLASHED.
WHEN SETTING A CIRCUIT BREAKER ON A POWER CENTER INJURED SUFFERED A FLASHBURN WHEN THE CIRCUIT BREAKER BLEW UP.
INJURED WAS CHECKING FOR LOW VOLTAGE ON CIRCUIT BREAKER WITH MULT--TESTER.WHEN HE WAS GOING TO CHECK THE 3RD PHASE ON LINE SIDE OF BREAKER THE FLAS OCCUREIT WAS DETERMINED THAT THE 3RD PHASE C AME OUT AND SHORTED TO THE OTHER PHASE IN CLOSE PROXIMITY TO IT CAUSING THE FLASH.INJURED SUSTAINED 2ND DEGREE BURN TO HIS RIGHT HAND AND ARM.HE HAS ALSO BEEN HOSPITALIZED FOR THIS INJURY.
CADWELD JOINT EXPLODED WHEN MAKING GROUND CABLE ATTACHMENT TO BEAM AND BURNT RIGHT FORE AND INDEX FINGER AND RIGHT HAND.
INJURED WAS INS7ALLING ROOFBOLTER CABLE IN THE SWITCH GEAR.THE CABLE IN THE ROOF BOLTER WAS GROUNDED AND WHEN THE POWER WAS TURNED ON,THE SWITCH BOX GROUNDED FROM PHASE TO PHASE CAUSING A BURN .(PRESCRIPTION 9ECEIVED)
