Fall measured approximately 25 feet long x 16 feet wide x 5 feet high. Fall has been timbered off and danger signs posted on all approaches. This mine is in non-producing status.

Employee was lifting cable bolts off of the top of 5ft bolts that were in an insert to load the roof bolter.

Safety Department was notified on 08/19/2024 that injured has a hernia (unrepaired) diagnosis. Injured returned to work on 08/19/2024. Standard time charge of 50 days applies.

Employee was assisting in unloading supplies when a pallet of block on the fork lift hit the water line causing it to fall, striking the employee on the left ankle and track rail.

Employee was bending a roof bolt in #8 entry when the bolt slipped and smashed EE's right pinky finger between the bolt and drill chuck

Employee was changing sides with miner to clean up. Shuttle car operator had backed up to allow room. As he was trammimg the miner back to the face, the shuttle car op. started advancing and struck miner op. in the back.

Miner in #5 entry MMU 004 while replacing Rub rail on RT miner rub rail slipped off bracket and fell on Employees right foot. Did not miss any work until 10/06/2015.
Trying to put pad duster back on track and he reached for a track jack and hurt his back. **Possibly contesting accident**

Injured along with 2 (two) other employees was hanging miner cable, When Injured felt a pull in lower back.
On 8/5/2015 Employee was attempting to collapse a grease jack on a shuttle car. While prying on the conveyor chain with a pry bar, bar slipped out of the hole, came back and his hands hit the bridge of his nose and front teeth resulting in a broken tooth and a small laceration on his nose. Employee did not miss any work due to this incident. Dental repairs were made on 8/12/15.
Employee was setting up to take 1st lift out of block. Injured bent over to throw miner slack (cable) next to rib, when without warning, Rib Rolled off and struck him on the lower left leg and ankle pinning him between the miner and the rock. went to hospital and received X-rays RX.
Employee was walking two breaks outby the section loading points in the neutral on the belt line and fell over a rock. Radio microphone and tag reader struck him in his right side
While replacing guarding after replacing tail roller @ tail of plant feed Belt. The guarding fell catching employees left Middle finger between the guarding and structure: Employee received medical attention where he had Debridement left middle finger, local tissue re-arrangement.
While taking a flat car around a curve, the car derailed. Injured attempted to uncouple the car but it was in a bind and would not uncouple. Injured used a slate bar to pry the pin loose. Draw head uncoupled violently and caused the slate bar to strike him on the right side of his face and neck
EE stumbled and fell while working in the plant.Medical att.was immediately.Drs exam indicated neg xrays but did validate advanced degenerative disc disease as noted in the attached drs notes on pg9 (handwritten pg#s). Employee would have returned to work but was unable to due to complications being caused by the advanced(DDD)which was discovered by the med.staff during the exam

On 6-8-15 Employee was coming back from the miner with a load of coal employee hit two holes and jarred his back. On 7-16-15 Employee was going up #6 entry to the miner he hit a hole in the intersection causing his lower back to slam into the shuttle car seat. The safety department was notified on 8-17-15 Employee was under doctors care. Employee began missing work on 8-20-15.
