Employee was cleaning the seal on the finish mill door when the 3/8" hose coupling failed, allowing the hose to strike the employee in the face resulting in a bruised eye, the employee was prescribed eye drop on 9/6/2022 making the injury reportable.

Mostrando los 6EE reached overhead to disconnect an air line from a quick connect 3/8 air hose. The EE failed to bleed the pressure off the line before disconnecting it. EE lost control of the air hose it bounced off the structure and came back and struck EE on the upper lip requiring three stitches.
Employee opened a port to try and clear the chunk after closing the manual gate. Once the chunk cleared, built up pressure in the line was released. This resulted in cement dust being blown into EE's face, knocking off safety glasses. The employee utilized an eye wash station to clean out eyes. Within several hours after the incident reported having pain in the right eye.
Blow back of material struck face
A line containing motor oil ruptured and injured employee. The high pressured oil cut into employee's clothes, and bruised and abraided his chest and finger. An incorrect pump was used, and identified as a grease pump after the safety incident. The incorrect pump over-pressurized the line causing the it to break at the weakest point in the system.
INJURED CLOSED A 3 INCH PLUG VALVE ON PIPING FROM A C150SS COMPRESSOR AND THE VALVE EXPLODED. INJURED SUFFERED A SEVER LACERATION TO THE LEFT FOREARM AND MULTIPLE FRACTURES TO THE SMALL BONE I N THE FOREARM.
