Walking outby from #2 face of 8W at 41 block intersection the rib corner "popped" and they put a hand up to cover their face. A rock came out of the upper rib corner and struck their hand causing an open fracture and dislocation to the distal phalange of their right index finger. No problems with equipment or compliance issues, training or PPE.

Employee was walking to miner when piece of rib fell onto EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Injured employee was operating a shuttle car when a small piece of coal fell from the rib and into the operator's cab and struck their left little finger. Employee suffered a small abrasion/contusion to the finger. Employee worked their next shift but sought treatment the following shift. Employee began missing work on 9-12-25 and missed 2 shifts of work.

Employee was unloading a beam from the scoop. When they went to set the beam down, the rib rolled and large pieces of material struck them in the back, neck and back of their head.

In St Leo Mains walking from loader to miner after getting a water bottle at #1-#0 crosscut 109 block the rib "popped" dislodging a softball sized projectile that struck them in the left side of the face resulting in contusions to left eyebrow and nose. No problems with equipment or compliance issues, training, or PPE.

While drilling a piece of the mine rib measuring 12x12 fell from above the work area striking the employee in the back of the head.

Employee was changing bits on the tailgate drum at midface, when the coal face blew out and struck the employee. Employee received a fracture to T9, T10, and T11. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency not a factor.

Employee was rib pinning. When EE removed the augers, EE was struck by a rock on their arm. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was bolting at the miner when a piece of rib struck EE's left knee. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was in the x-cut 2-3 at 42blk. when the rib popped. Employee turned to leave the x-cut and air line came down striking the employee in the nose. The employee sustained fracture to nose and began missing work on the 11-7-2023.

The injured was standing behind the miner as it backed out of the face on the 5W Section. The rock binder fell out from the rib, and struck the EE in the left arm and wrist. The EE finished their shift, but began missing work on 7/24/23.

A fully trained miner was roof bolting when a rib rolled and struck EE in the head. The injury was not the result of any compliance issues nor were any protective guards involved.

EMPLOYEE WAS GETTING READY TO PUT THE RIB PIN IN THE XCUT WHEN EMPLOYEE SWUNG OVER TO THE EDGE OF THE PILLAR WITH THE CANOPY AND A PIECE OF ROCK 3'X2'X1" FELL FROM THE RIB STRIKING THE EDGE OF THE CANOPY. A PIECE OF THE ROCK HIT EMPLOYEE'S LEFT CHECK CUTTING THE CHEEK.

The Employee was standing on a ladder to place a cap piece on top of a temporary roof support when a piece of coal fell from the face and struck the ladder and employee. Employee fell to the ground and the piece of coal landed on right leg.

A fully trained miner injured a shoulder when a rib fell on it. The accident was not the result of any compliance issues nor were any protective guards involved.

Employee was assisting panline bolter when a piece of rock binder fell out of the face and struck them in the left shoulder and neck. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

A fully trained miner was greasing a feeder when a rock fell from the rib hitting EE's shoulder. The injury was not the result of any compliance issues nor were any protective guards involved. Employee did not begin missing work until 6/5/2021.

A fully trained miner was installing a rib bolt when a piece of rock fell from the rib hitting EE in the cheek. The injury was not the result of any compliance issues nor were any protective guards involved.

The EE was rib bolting on the 2 West Section, in the #3 entry from 54-55 block. The EE drilled the hole for the rib bolt and turned to get a bolt and glue. The rib rolled out, striking EE's left knee hyperextending it. The EE finished shift, and began missing work on 2/6/21.

EE was bolting on the miner in the 2 West Section. A loose rib out in front of the injured fell and struck the rib deflector. A smaller piece broke off and struck the EE in the right hip area.

Employee was shoveling the ribs in the 4th East Mains when a piece of coal fell from the rib and struck the employee on top of hard hat and left foot. Employee was diagnosed to have sustained contusion and neck strain.

EE was on-shifting in the Left Return 38 block of 1 North. After signing the date board, a rock rolled from the rib/brow and contacted EE's left foot and ankle. EE finished the shift.
