The E.E. was walking along beside a supply trailer that was sitting in the middle of the supply road. They slipped in the loose rock, causing their left foot to slide under the trailer.

The EE stated: while driving a 4 man diesel EE ran upon a small gob pile, while backing up off the pile, the steering wheel spun around striking EE's left wrist

The EE WAS OPERATING A MINER LOADING COAL IN THE #1 ENTRY. WHILE WAITING ON A CAR TO COME BACK AND LOAD A ROCK FELL FROM THE TOP, HITTING THEM IN THE UPPER MIDDLE PART OF THEIR BACK.

The EE was standing on a small gob pile dropping a miner cable, the gob pile shifted under EE's feet straining EE's right knee

THE EE WAS CUTTING A PIECE OF BELT. THE KNIFE SLIPPED, CUTTING EE'S LEFT ARM.

The EE was lifting a 5-gallon fuel can and felt pain in EE's lower back.

the employee was bolting in the #7 entry on the #3 unit. EE was tramming EE's bolter over a large rock that was laying on the mine floor. the rock shifted hitting the top of EE's left foot.

Employee was installing bottom belt rollers when EE experienced pain in EE's lower back. Employee began missing work on 1/10/24.

The employee was standing on the opposite side of the operator of the bolter on the left side of #3 unit. EE was at the back of the bolter next to the rear tire when the bolter trammed up aprox 6 " and ran over EE's toes on EE's right foot. there were no bones broken. the employee did not start missing any work time until 01/02/2024

The employee was cutting a piece of curtain with a belt knife, the knife slipped cutting their left thumb.

The Injured employee was driving a golf cart down the slope. as EE neared the bottom EE failed to maintain control. the injured employee exited the moving golf cart and it ran over EE's left foot.

A Rock fall occurred in the 214 Submain South at Spad# 1+50 to 2+25 in the #5 entry 18.5'x 10'x 55' there were no injuries due to the fall

The employee was putting up a cable bolt & it hung on a rock laying on the mine floor. The employee felt a strain in shoulder and neck.

The employee stated: was driving a ram car and ran over a rock in the haul road and felt pain in right shoulder.

A roof fall occurred in the 2nd North Mains at spad # 23+10 to spad # 30+10 in the #8 entry. There were no injuries due to the rock fall.

the employee was moving an I Beam when EE felt pain in EE's lower back

The employee was cutting a piece of belt to make a splice when the knife slipped cutting employee's right leg

A rock fall occurred at approximately 4:05 P.M. in the 221 panel West. #2 Unit at spad 6+30 Between the #3 and #4 entries. There were no injuries due to the rockfall

The employee was dragging curtain in the #3 entry, stepped in a hole, twisted left ankle and fell on some rocks

AFTER THE EMPLOYEE INSTALLED A ROOFBOLT THE EMPLOYEE DROPPED THE ATRS FROM THE SIDE CONTROLS AND STARTED TO WALK BACK TO THE OPERATORS DECK, A ROCK FELL FROM THE TOP HITTING THE EMPLOYEE'S LEFT FOREARM THEN ROLLED DOWN EMPLOYEE'S RIGHT LEG

A roof fall occurred at approximately 9:28 P.M. on the 1st North Main Beltline between Spad# 18+90 and Spad #19+60. There were no injuries due to the rockfall.

The employee stated : was putting up a piece of hog paneling when a rock fell out hitting employee in the nose

The employee was installing a roof bolt in the #5 entry, a piece of draw rock fell hitting EE's left ankle.

The employee was sitting in the deck of a pinner moving it. The employee was holding to the top of the canopy with left hand, the canopy came into contact with the top mashing two fingers

The employee was removing a fire suppression canister for a scoop charger out of a scoop bucket, it caught EE's right pinky finger between the canister and the charger causing an abrasion

The employee was climbing out of the deck of a scoop. They put their left foot on the ground and felt pain in left groin.

Employee was installing roof bolts when a rock 2'X 3'X 5" fell out and struck EE in the right shoulder. The rock then rolled off EE's shoulder and landed on EE's foot. The employee immediately felt pain in the foot and shoulder.

The employee was hanging a charger cable, another person pulled on the came cable pulling the employee down to the ground causing left knee to pop. The employee did not start missing work until 05/04/2023

The overwind switch for the emergency Hoist was not working in the down travel position.

The injured employee and another worker were moving a scoop charger, they were taking down the cable. The injured employee was hooking up the excess cable with a pull strap. Part of the cable that was still hanging fell hitting employee in the back pushing employee's face onto the back of a lotrac

The employee stated: Was unrolling a line curtain in #9 entry, when employee felt a pain in lower back

Employee was walking up #8 Entry, when a piece of drawrock fell, hitting employee in the left hand. Causing employee to receive sutures.

While Driving a spad into the rib with a hammer the employee hit the back of their left hand causing a contusion.

A roof fall was found at approximately 12:20 A.M. in the 216 panel west at spad # 30+50 #7 entry in the primary escapeway. There were no injuries due to the rockfall.

The employee was bolting on the #3 unit in the #9 entry. EE was attempting to connect EE's top steel to EE's bottom steel when a piece of rock hit EE's upper left bicep causing EE to receive sutures

A roof fall was found at approximately 11:45 A.M. on the 215 Submain West Spad #19+50 #7 entry Primary Escapeway. The time of the fall is unknown. There were no injuries due to the rockfall.

A ROCK FALL OCCURRED ON THE 1ST NORTH BELTLINE AT XCUT 26 IN THE #6 ENTRY BETWEEN SPAD 16+80 AND 17+50. THE FALL WAS 70X18X12 FEET. THERE WERE NO INJURIES DUE TO THE FALL.

A ROOF FALL WAS FOUND AT APPROXIMATELY 8:30 PM ON THE 1ST NORTH MAIN BELT BETWEEN SPADS 17+50 & 18+20. THE FALL WAS LOCATED IN THE #6 ENTRY. THE TIME OF THE FALL IS UNKNOWN. THE FALL WAS FOUND DURING THE EARLY TUESDAY NIGHT PRESHIFT. THERE WERE NO INJURIES DUE TO THE ROCKFALL.

A ROCKFALL OCCURRED IN THE #6 ENTRY (BELT) ON THE 2ND NORTH MAINS AT SPAD 54+20. THE ROCKFALL WAS APPROXIMATELY 18X40X10 AND THERE WERE NO INJURIES FROM THE ROCKFALL.

EMPLOYEE STATED EE WAS DROPPING THE PINNER CABLE ON THE #2 UNIT WHEN IT CAUGHT ON A PIE PAN, WAS STRETCHED TIGHT AND PULLED APART STRIKING EE IN THE LEFT HEAL. EE WAS WORKING ON THE POWER MOVERS CREW.

EMPLOYEE STATED THEY WERE IN THE BATTERY SHACK, STANDING IN A HIGH SPOT WHEN THEY TURNED TO LEAVE AND RAN INTO A LOW SPOT WITH THE SIDE OF THEIR HEAD CUTTING THEIR LEFT EAR. EMPLOYEE RECEIVED 8 SUTURES IN THE LEFT EAR.

EMPLOYEE STATED EE WAS CARRYING A BUCKET OF MINER BITS TO THE MINER WHEN EE FELT PAIN IN EE'S RIGHT SHOULDER WHEN EE SWUNG AROUND TO PUT THEM ON TOP OF THE MINER. EE IMMEDIATELY FELT PAIN IN EE'S RIGHT SHOULDER AND NECK. ACCIDENT BECAME REPORTABLE ON 10/31/22.

EMPLOYEE WAS BOLTING IN THE #3 ENTRY ON THE #2 UNIT WHEN EE TRIED TO PUSH THE ROOF BOLT UP ENOUGH TO GET IT IN THE CHUCK WHEN EE FELT PAIN IN EE'S LOWER BACK.

EMPLOYEE STATED EE WAS CARRYING A TOP ROLLER WHEN EE TRIPPED OVER SOME LOOSE CABLE BOLTS AND FELL INTO THE DIESEL MANTRIP. EE'S LEFT INDEX FINGER WAS CAUGHT BETWEEN THE ROLLER AND SIDE OF THE MANTRIP CAUSING A CUT AND ABRASION TO EE'S INDEX FINGER. EE HAD 3 SUTURES PUT IN EE'S LEFT INDEX FINGER.

A rockfall occurred on the 3rd west mains at spad #49+10 in the crosscut between the #5 and #6 entries. The fall measured 18x12x55. The fall was found around 6:00p.m. and was immediately called in on the MSHA hotline. No one was injured due to the rockfall

EMPLOYEE STATED WHILE THEY WERE LIFTING THE LIDS ON A RAM CAR BATTERY (GREEN BATTERY) IN THE #3 UNIT BATTERY BARN THEY SLIPPED ON A WET ROCK AND TWISTED THEIR LEFT KNEE. THIS ACCIDENT BECAME A REPORTABLE LOST TIME WHEN THEY WERE TAKEN OFF WORK 10-10-22.

Employee stated: while getting off a mantrip to load supplies employee twisted right knee and felt pain. The employee finished shift

EMPLOYEE STATED: EE WAS PUTTING UP A BOLT IN THE #6 ENTRY ON THE #3 UNIT WHEN EE FELT PAIN IN EE'S RIGHT SHOULDER. EMPLOYEE WAS STRAIGHTENING ROOFBOLT. EMPLOYEE HAD PREVIOUSLY HAD SHOULDER PAIN FOR APPROXIMATELY 3 MONTHS.

EMPLOYEE STATED THEY WERE EXITING THE SCOOP WHEN THEY CAUGHT THEIR RIGHT FOOT BETWEEN 2 ROCKS CAUSING THEM TO FALL DUE TO THE FOOT BEING CAUGHT. THEY HAD LOWER LEG PAIN AND RIGHT ANKLE PAIN. THIS BECAME REPORTABLE 9-12-22.

A ROCKFALL OCCURRED ON THE 1ST NORTH MAINS IN THE #6 ENTRY (BELT) BETWEEN SPAD 14+70 & 15+40. THE FALL WAS IN THE PILLAR LINE AND WAS APPROXIMATELY 18X60X11. THE ROCKFALL WAS FOUND AT 12:14 AM AND IMMEDIATELY CALLED IN ON THE MSHA HOTLINE. NO ONE WAS INJURED DUE TO THE ACCIDENT (ROCKFALL).

EMPLOYEE WAS STANDING ON TOP OF THE BELT SLIDE WHEN EE SLIPPED OFF THE BELT SLIDE. THE BELT WAS DAMP AND SLICK. EE HAD PAIN IN EE'S LOWER BACK AND RIGHT LEG. EE WENT BACK TO THE DR. ON 9/8/22 AND DID NOT REPORT TO WORK ON FRIDAY 9/9/22. THE ACCIDENT BECAME LOST TIME ON FRIDAY 9/9/22.

EMPLOYEE STATED WHILE CLEANING UP AROUND A DUMPSTER, EE WAS CARRYING A PALLET WHEN EE TRIPPED AND FELL HITTING EE'S RIGHT KNEE ON A PIECE OF A CONCRETE BLOCK. THE EMPLOYEE STARTED MISSING WORK ON 9-9-22.

EMPLOYEE STATED WHILE INSTALLING EE'S LAST DRILL STEEL TO FINISH EE'S HOLE, A PIECE OF ROCK 12"X12"X1/2" FELL CATCHING EE'S THUMP BETWEEN THE ROCK AND POT. THE LOCATION WAS ON THE #2 UNIT IN THE #3 ENTRY AT SPAD 15+75.

A ROCKFALL WAS FOUND IN THE LEFT RETURN OF THE #3 UNIT IN THE 2ND NORTH MAIN AT SPAD 92+00 TO 93+40. THE ROCKFALL DAMAGED A LEFT RETURN BRATTICE.THERE WAS NO ONE INJURED IN THE ROCKFALL. THE FALL WAS APPROXIMATELY 100X19X12.

EMPLOYEE STATED WAS IN THE # 1 ENTRY ON THE #2 UNIT WHEN REACHED FOR A ROOF BOLT STICKING OUT OF THE GOB. WHEN EE PULLED ON THE ROOF BOLT EE FELT PAIN IN LOWER BACK AND LEFT LEG.

EMPLOYEE STATED WHILE ROOF BOLTING, A PIECE OF ROCK FELL OUT OF THE 9 LEFT TURNOUT AND STRUCK EE IN THE LEFTSIDE. THE ROCK CAME OUT OF A SLIP AND HIT THE CANOPY OF THE ROOF BOLTER AND ROLLED OFF AND HIT EE.

Employee stated while running the miner in the #6 entry employee heard the top pop. When employee looked up a piece of rock fell from between the roof-bolts striking employee on the head and right shoulder. The area's roof-bolt spacing was correct. The employee did not miss any work until 6-1-22.

The employee was pinning. EE was trying to put the top steel on the bottom steel when a rock fell from the top hitting EE's right hand.

The employee stated: was pulling down some loose top with a slate bar. After EE pulled down the piece that EE was working on, and started to walk away, another piece fell hitting EE in the left hand, fracturing EE's little finger.

The employee stated: Was bending over to bust a header board. Employee's corded light came off of employee's hard hat hitting employee in the mouth chipping front tooth

THE EMPLOYEE STATED: EMPLOYEE WAS CUTTING THE BAND ON A BUNDLE OF ROOF BOLTS AND GOT A FOOT CAUGHT BETWEEN THE ROOF BOLTS AND THE BUMPER OF THE BOLTER.

Employee stated while loading the bolter, the 6' bolt hit the bolter breaking EE's left ring finger. After further investigation from Mine Management, it was determined that the injury was due to horseplay not with the employee loading the pinner. The injured employee was a Contractor.

AN UNPLANNED ROCK FALL OCCURRED IN THE #1 ENTRY FROM SPAD 99+70 TO 101+80 ON THE 2ND NORTH MAINS LEFT RETURN ON THE #3 UNIT. THE FALL HAPPENED INBY THE UNIT TAILPIECE. THERE WERE NO INJURIES FROM THE ROCK FALL.

EMPLOYEE WAS PUSHING A ROOF BOLT INTO THE HOLE EMPLOYEE HAD DRILLED WHEN EMPLOYEE FELT A POP IN RIGHT SHOULDER.

EMPLOYEE WAS GETTING READY TO PUT THE RIB PIN IN THE XCUT WHEN EMPLOYEE SWUNG OVER TO THE EDGE OF THE PILLAR WITH THE CANOPY AND A PIECE OF ROCK 3'X2'X1" FELL FROM THE RIB STRIKING THE EDGE OF THE CANOPY. A PIECE OF THE ROCK HIT EMPLOYEE'S LEFT CHECK CUTTING THE CHEEK.

EMPLOYEE WAS WALKING THROUGH A BACKUP CURTAIN AND STEPPED IN A HOLE TWISTING BACK.

A ROCKFALL OCCURRED ON THE 2ND NORTH MAIN BELT 15'INBY SPAD 74+50. THE FALL WAS 30X14X10 IN THE PILLAR LINE. THERE WERE NO INJURIES FROM THE ROCKFALL. THERE WAS NO SLIPS IN THE AREA OR WATER PRESENT IN THE FALL AREA.

Employee was putting cross over pad on bolter when the pad flew back and hit them in the front right tooth. The pad broke the front right tooth.

EMPLOYEE WAS HANGING THE MINER CABLE AND FELT PAIN IN EE'S GROIN AREA.

A rockfall occurred on the 2nd Southwest belt at spad 32+50 in the #5 Entry. The fall was 45x18x9. There were no injuries from the rockfall.

Employee was roof-bolting on the #3 unit in the #6 entry. When the bolter moved up the employee swung the canopy over toward the rib to install the rib pin and employee's right hand was holding onto the canopy top which caused employee to mash right fingers between the canopy and rib.

Employee was straightening a roof-bolt with 1 hand when the bolt turned and they felt pain in their neck and right shoulder.

A ROOF FALL OCCURRED ON THE 1ST SOUTH MAIN BELT AT SPAD 21+70 (x-cut 35). THE FALL WAS 55X18X10 ON THE BELT. THERE WERE NO INJURIES DUE TO THE FALL.

Employee was mining the face of #12 entry at spad 16+50 on the #1 unit when a piece of rock 2'x8'x3" fell between the rib pin and rib striking EE in the left ribs and back. There were no signs of loose rock or any other hazards in the #12 entry.

During the Weekly examination of the left return (1st South Main) off the #3 unit, in the #1 entry between spad 16+80 thru 18+20 a Roof Fall was found in the Examiners travel entry. The fall was 18'x10x200 feet.

EMPLOYEE STATED THEY WERE RE-TAPING THE MINER CABLE AND AFTER TAPING THE CABLE THREW IT OVER AGAINST THE RIB AND FELT PAIN IN LOWER BACK. EMPLOYEE FINISHED THE SHIFT.

EMPLOYEE STATED WHEN TRAVELING FROM THE #10 ENTRY BACK TO THE FEEDER ON THE #1 UNIT THEY HIT A HOLE IN THE TRAVELWAY JAMMING LOWER BACK.

The employee was driving a ramcar up the #6 entry, while turning in the xc that was taking them to #7 entry, the car hit a bad spot in the road and it jammed their lower back on the right side.

The employee was pulling roof bolts off the top of bolter. The bolt they were pulling off was stuck under a pile of other bolts. The bolt they were pulling on suddenly became unstuck, pinching the inside of right ring finger against the coal rib and the end of the bolt, causing the employee to receive sutures.

Employee was reaching for EE's lunch box on the back of the bolter when EE's left foot went under the bumper. The back of the bolter was low to the ground and EE's foot was caught between the bumper and bottom. The bolter was located in the intersection at spad 5+50 in the #5 entry. The shift was over at the time of the accident.

Employee stated EE parked the diesel scoop at the new #1 unit setup area in #8 entry between spad 11+90 and 12+60 when a piece of rock 4"x9"x1/2 to 2" thick fell from the edge of a kettle bottom. EE was located more than 2500' off the active working section #3 unit. This area had been worked out in Jan. of 2021.

PRIDE MINE HAD A UNPLANNED ROOF FALL OCCUR ON THE #1 UNIT, IN THE #8 ENTRY AT SPAD 16+70 IN THE INTERSECTION. THE FALL WAS 18X18X10 AND WILL NOT BE CLEANED UP.

Employee stated was operating a scoop on the 1A supply road when EE hit a hole and jammed their back. EE was taking belt structure to the unit for the belt move.

Employee stated that while loading 6' roof-bolts onto the roof-bolter employee felt pain in right shoulder. This accident became reportable on 5/7/21. This was the employee's first day not reporting to work.

Employee stated had just arrived at the bolter at shift change and installed 1 bolt. While getting the 2nd bolt ready to install, a piece of rock 20"x15"x1" to paper thin fell striking EE on the hard hat, left shoulder and back.

Employee stated was walking to the power box to put the power backup on the right miner when employee stepped in a rut and turned right knee. Accident became reportable on 5/4/2021 when employee missed employee's 1st workday.

Employee stated that EE was working on batteries on a scoop when the batteries arced and caused a flash burn to EE's face and eyes.

Employee stated was driving the Ram Car and hit a hole and jammed back. The #11 entry EE was traveling had been mucked out from the weekend. EE did not see the hole EE hit.

The employee reached to get something from the middle of the bolter, when employee pulled arm back, employee cut the upper back side of arm, requiring sutures.

The employee was installing a 6ft roof bolt. Employee was straightening the pin out and felt pain in right shoulder.

Employee stated, was bolting top when a piece of rock in front of the ATR'S fell outby striking them in the right calf and ankle.

Employee stated EE dropped EE's canopy and the canopy struck EE's neck causing pain in EE's neck and back. EE also fell and struck EE's ribs when EE fell.

Employee stated: was unloading tools off ride and felt pain in lower/mid back.

Employee stated while sitting on a bucket running the right-side miner a piece of rock 1'x 1'x 1"- 0" struck EE's right hand and pinched hand between EE's thumb and pointer finger on the handle on the remote control box. 4 sutures.

Employee stated the roof-bolter had finished and employee was telling them where to place the bolter when a piece of rock hit employee on the top of the hand. Employee had placed right hand on the bolter while talking to them. Employee had on gloves.

Examiner and MSHA inspector were making the 1st West Mains intake and found a rock fall at 24+50 in the #6 and #7 entries.

Employee was cutting a piece of belt with a belt knife and the knife slipped and cut just above right knee.

EMPLOYEE AND 2 OTHERS WERE HANGING AN OUTBY AIRLOCK DOOR WHEN THE DOOR STARTED TO FALL AND EMPLOYEE FELT PAIN IN LOWER BACK. EMPLOYEE STARTED MISSING WORK ON 10/12/2020.

The Employee stated: Employee washed hands, then turned around to leave and right knee locked up

The employee stated: While moving the #3 Battery shack that EE felt pain in EE's shoulder

A ROCK FALL OCCURRED ON THE 1ST WEST MAINS BELT AT SPAD 14+70 IN THE #5 ENTRY. THE FALL WAS 19X40X12. THERE WERE NO ADVERSE CONDITIONS IN THE AREA OF THE FALL. THE ROOF WAS BOLTED PER MAIN LINE BOLTING PATTERN.

Employee stated was working with the power movers hanging miner cable when EE felt pain in the lower back and left leg. Employee came back to work on regular job but started missing on 9/24/20.

A ROCKFALL OCCURRED IN THE S/R IN THE 302 SUBMAIN WEST IN THE #4 ENTRY AT SPAD 3+50 SOMETIME BETWEEN THE AFTERNOON SHIFT ON 9/12/20 & 9/13/20 WHEN THE PRE-SHIFT EXAMINERS CAME IN AT 8:00 PM TO EXAM THE MINES FOR THE 3RD SHIFT WHICH STARTED AT 11:00 PM. THE FALL WAS 10X19X60.

The employee was roof bolting on the #2 unit in the #9 entry. A piece of rock fell out and struck EE in the left shoulder.

Employee stated EE was bolting the #10 entry. The top and bottom pinner steels were stuck together and when EE pulled them apart pulling towards EE's face the pinner steel hit EE in the chin requiring 4 sutures.

Employee was installing a 6' roof-bolt in-by the brow after the brow had been bolted by 9' cable bolts when a piece of rock fell and struck the right side of EE's hard hat causing a laceration on the right side of EE's forehead. EE received 4 sutures.

Employee stated EE was working on the belt crew putting on belt when EE felt pain in the middle of EE's back. EE was pulling on a piece of belt.

Employee was rolling up the curtain in the #11 entry when they stepped in a hole and twisted left knee which caused them to fall. The Hospital determined that they had broken the left leg right below the left knee.

Employee stated while tramming miner from #9 entry to #8 entry employee stepped on a piece of coal or rock and twisted left knee.

Employee stated while loading the roof-bolter EE felt pain in lower back. Employee had previous back injury.

Employee stated while building a brattice EE stepped on a piece of block. When EE started to fall EE tried to catch self and twisted EE's lower back. Employee stated EE had a back injury previously.

A rock fall occurred in the last open crosscut on the #1 unit at spad 5+65 in the #8 entry.

Employee stated while lifting the miner cable they felt pain in right calf.

Employee stated EE had set EE's ATRS and swing out to start bolting and had EE's finger on top of the canopy when a rock 1'x1'x1" fell striking EE's right index finger. EE needed sutures.

A rockfall was found in the 202 Panel South in an intersection at spad 30+20.The face had been driven in further than 20 feet.

Employee stated that while EE was leveling EE's drill pot 2 x-cuts outby the face area a piece of rock fell from a slip after EE drilled 2 holes in the roof striking the top of EE's right hand causing a laceration that resulted in sutures. The rock also dislocated 4 fingers on EE's right hand. EE was wearing gloves at the time of the accident. #1 Unit - Entry #11 - Spad 1+50

Employee stated that the wet down hose was pushed into the feeder and became wrapped around the pit breaker. The power was locked and tagged out and employee was manually rotating the pit breaker unwinding the hose when right hand got caught between the frame and a pit breaker bit. The employee received sutures in right hand.

Employee stated EE was holding the steel and pushing it up with the pot when the steel bent striking EE's left index finger knuckle. EE had removed EE's hand from the steel after the pot came in contact with the steel.

Employee was using a hand grinder cutting a metal band and grinder slipped off of the metal band and hit EE's left hand between the thumb and index finger requiring 3 sutures.

A rock fall was found in the North Mains at spad 30+30 in the #8 entry which fell on the High Voltage cable sending power to the #2 Unit.

Employee was standing unloading supplies off of the #23 mantrip for 2nd shift when the #22 4 man ride pulled forward and pinched the back of EE's left calf between the 2 trips. Employee required sutures in EE's left calf.

Employee stated EE was rock dusting the belt on the 2nd West when the hose EE was holding became caught and jerked EE off the trailer. When EE was pulled off the trailer by the tightening of the hose, EE's right foot went under the trailer and the wheel hit EE's lower leg just below EE's knee.

Employee stated a piece of rock fell and struck right foot. The rock struck about 3 inches back on EE's boot. An xray showed that EE's big toe and small toe had a hairline fracture.

Employee stated was operating a ram car when it struck a roof bolt and broke the bolts out of the canopy. The canopy dropped and hit employee in the chest on one of the corners.

Employee stated EE was breaking a header board apart when EE hit a metal band and the band hit EE's right wrist causing a laceration requiring sutures.

Employee stated was tramming the right side miner into the face of the #8 entry when EE reached to take the strap off and a piece of rock fell and hit EE's elbow. inby station 3+50. Sutures were required.

Employee stated EE's foot wrench slide out from under EE's tray and when EE reached to push it back in while lowering EE's boom, the boom pushed the wrench down on one end and up on the other catching EE's middle left finger on the tip against the bottom of EE's tray.

Employee was bleeding air out of a 6" waterline at the fire valve on #3 Unit. The waterline had just been installed at the new unit. The employee was opening and shutting the fire outlet valve when the last section came apart at the last joint which was 20' away. The belt was chained to the roof but lunged forward striking the employee in the nose. This resulted in a broken nose.

An unplanned roof fall was discovered in the 1st North Mains intake at spad # 31+00. Ventilation was not disrupted. The primary escapeway has been re-routed and the area cribbed and dangered off. Area was previously supported at time of mining, with fully grouted 6' bolts and supplemental supports consisting of 9' cable bolts.

While swinging boom, a rock approximately 10"x12"x3" slid off canopy striking right ankle. Employee finished shift and continued to work. Incident became a L.T.A. on 2/4/19.

An unplanned roof fall occurred at 11:25 a.m. in the 1st East Panel 1 at spad 10+80, the roof fall was above anchorage, and continued into adjacent intersections. This is a worked out area, no escapeways or travelways effected. Floor to roof support were installed and area dangered off. This fall will not be loaded out.

An unplanned roof fall was discovered in the 1st south sub-main intake out-by area at spad # 0+00. The area is not traveled except for examinations, no ventilation disruption experienced due to roof fall. Area previously supported, at time of mining, with 6' fully grouted bolts and supplemental supports consisting of 9' cable bolts.

During third shift belt advancement shift, miner's left hand became caught between belt tailpiece and wooden timber resulting in fracture in left small finger.

An unplanned roof fall occurred at 4:35p.m. in the 1st East, panel #1 at spad 3+00, the roof fall was above anchorage, and continued into adjacent intersections. This is a worked-out area, no ventilation or escape way effected. Floor to roof support were installed and area dangered off. This roof fall will not be loaded out.

Attempting to put second steel in roof while holding top steel in roof with left hand, a part of a kettle bottom broke free from roof striking EE in the right hand.

Employee who had previously had a hip replacement approximately 6 years prior was building permanent ventilation stopping. Employee had completed stopping and began to seal with plaster. Employee picked up bucket of plaster and felt sharp pain in left hip area.

Miner was walking along travel way into crosscut when stepped off small mound of side of roadway, fell to ground, complained of lower lumbar back pain.

Miner complained of lower lumbar back pain while hanging continuous miner cable during third shift belt and power move.

Roof Bolter Operator, installing fully-grouted resin roof bolt, straightened bolt, felt "pop" in right shoulder.

An unplanned roof fall was discovered by a mine examiner at 1530 on March 6, 2018. The examiner reported the roof fall and MSHA hotline was notified at 1537. The incident occurred in an intersection between 2nd South Panel and 3rd South Panel in a worked-out area.

While changing battery on green ram car at #1 Battery Shack the charging lead had a splice in it and when taking the cathead (plug) out of the battery receptacle the employee said ee was shocked on the right forearm. The employee was taken to the hospital and checked out and received a return to work full release paper.

Employee went to pull cable bolt on ground to put into roof. Cable bolt was hung on a pile of coal and when ee snatched it hard ee felt a pop in left shoulder.

Employee was bolting in #5 entry at Spad # 17+60 and as employee was walking back to pull up the bolter, ee hung the right foot under a rock lying on the bottom, tripping ee causing ee to fall twisting ankle as ee fell to the ground. Employee finished the shift and didn't go to the Doctor until that afternoon.

The right side pinner was moving from #7 to #8 entry. The Ram Car was setting idle with the pump motor on waiting for the other car to come out of #9 entry. Employee was moving cable out of the way for the pinner to pull into face of #8. EE was standing behind ram car when the car released brake causing ee to rollback striking employee in back of knee.

While loading a car employee stepped on a mound of coal and twisted their left knee.

Employee was operating a Diesel Scoop about 30' from the top of the slope. The scoop died and rolled down the entire slope and hit a concrete wall at the bottom. The employee had a bump on head and was complaining of back pain. Employee was taken to hospital seen and released. This accident did not become a lost time until 11/28/17 when Doctor took employee off work.

Employee was working on oil leaks on feeder, tightening up loose fittings when the channel locks slipped causing employee to lose balance and footing twisting left knee.

Employee was helping hang the miner cable in #8 entry when EE felt something pop in the right shoulder.

Employee was cutting #6 entry when a rock measuring 12" wide X 30" long X 1/4" to 1.5" thick broke from the top striking employee on the head and left shoulder area. Employee said neck and shoulders were hurting so after injuries were assessed employee was taken outside and transported to hospitals.

EE went to move the 4 man diesel, pulled it up about 8-10 ft. to get it out of the way. EE said that a piece of rock fell and hit them in the head and left shoulder. This happened between 2:30 and 3:30am and they continued to work until around 4:oo when they were transported outside. Also MSHA was called,because the ambulance EMT called life-flight. EE was sent to St. Vincent.

EE was taking the miner back to the face of #4 entry. Pulled cable up in the face, pinched self between the tail of the miner and the rib.

EE was removing block from a scoop bucket and went to walk away from the scoop and slipped off of wet and muddy cable and fell down. EE's left foot stayed stuck in the mud, when EE fell and twisted left knee.

HAD A ROCK FALL OUTBY # 1 UNIT IN # 9 ENTRY AT 9+00 TO 9+50 IN PILLAR LINE. Roof Fall was about 18' wide x 20' long x6.5 high

Had a rock fall in the 3rd. South Panel at 8+50 in the intersection. The fall was 19' wide x 19' long x 9' high.

Employee was repairing doors at a rock dust hole when a beam turned over and caught their right little finger, mashing and broke it. This resulted in five stitches to employee's right little finger.

EE was hanging an A-Frame and was holding it up with left knee. The A-Frame slipped off the stud and came down on EE as EE fell their right foot did not turn when their body twisting their knee. EE was treated at Muhlenberg Community Hospital and diagnosed with a fracture to the right medial tibial plateau.

Employee was stacking 8" concrete blocks on a pallet. The employee tripped over a brattice cloth, causing self to fall. The block ee was carrying fell on fingers-smashing them between another block.

Had a roof fall in the Main East. Time is unknown, Examiner and inspector found the fall while making the return air course on the weekly. Fall was at 39+20 from 2-9 in the return air course, it was about 480 feet long. The fall was inby unit # 1 in the return air course.

Building a brattice picked up a block to stack on the brattice, turning around and felt a pop in EE's middle lower back

While employee was scaling rock in #2 entry on #1 Unit he moved a smaller rock dislodging a larger rock striking him on the top of the head pushing him toward the return side rib.

Building overcast went to grab a piece of metal and the whole stack fell on top of employees knee/leg.

EE(ACC Employee) was working on the surface area of the Mine helping to drill a rock dust hole. As he was pulling a drill steel back onto the trailer he slipped on another drill steel that was lying on the trailer causing him to fall over the baffle board which resulted to a strain to his back.

While employee was in process of putting together conveyor chain on continuous miner he was cut by a piece of metal on right shoulder.
