While conducting retreat mining on 6-Left Panel off B-4 Mains, a coal/rock outburst occurred inby break #35. The outburst involved the Mobile bridge operator who suffered multiple blunt trauma injuries involving the chest, right arm, and back.

Mostrando los 28While employee was trying to search for a broken tie rod end on the car he was operating, he was struck by a piece of rib rock that rolled down the rib striking his left ankle causing a fracture and dislocation.
While the foreman was walking to the continuous miner, a piece of rib rock slid down the rib. When the foreman put his hand out, the rocks struck his right hand causing a laceration to his right index finger requiring sutures.
As the employee was removing the drill steel from a rib bolt hole, a piece of rib rock slid down the steel, striking his left hand. He suffered a fracture and laceration of his left index finger.
The foreman was watching production activities when a piece of rib rock fell striking him causing him to strike his nose against a mobile bridge unit. The employee continued working and sought medical treatment for his nose on 2/4/2011. The nose was determined to be fractured on that date.
While tramming a roof bolter a piece of rib fell, striking the employee on the head and neck, causing a laceration to his forehead that requiried sutures. The rib material measured 3' by 2' and was 4" in thickness.
While shoveling a tailpiece, the employee was struck by a piece of rib rock on the head and neck. The rock measured 30" by 8" and was 1" to 4" in thickness. The employee continued working and started missing work on 4/13/2009.
EE was cleaning out to access the crawler take up on the miner, a small piece of rib/coal fell from the lower rib striking him in the lower leg. EE suffered a fracured tibia and a laceration to the leg.
He was standing outside of the shuttle car when a rib roll occurred and hit his leg against the deck of the car, resulting in a strain to the right knee. This occurred on the 003-0 section at the section feeder.

EE WAS POSITIONED ALONG THE RIGHT RIB OF THE #4 ENTRY, WHEN THE RIB ROLLED OFF AND STRUCK EE IN THE BACK, IT RESULTED IN 2 FRACTURED RIBS AND A FRACTURE TO RIGHT HEEL ALONG WITH SOME CONTUSIONS TO HEAD, BACK, LEGS AND CHEST. INJURY OCCURRED ON THE 002-0 SECTION.
HE STATED THAT HE WAS HELPING WATCH THE MRS CABLES ON THE SECTION WHEN A RIB ROLL OCCURRED AND STRUCK HIS LEFT FOOT. IT RESULTED INTO A FRACTURETO THE LITTLE TOE AND BRUISING HIS LEFT FOOT. IT OCCURRED ON THE 002-0 SECTION
HE WAS WALKING UP TO THE PILLAR LINE WHEN COAL SHOT OFF OF THE RIGHT RIB AND STRUCK HIM.
A PIECE OF ROCK FELL FROM THE RIB AND STRUCK HIMON THE FOOT.
HE WAS LOCATED NEAR THE COAL RIB APPROX. 12' OUTBY THE LAST ROW OF BOLTS IN THE 3 LT. CROSSCUT WHEN A RIB ROLLED AND STRUCK HIM FROM BEHIND. THE RIB CONSISTED OF COAL AND ROCK WAS 65" HIGH. TH E PIECE THAT IS BELIEVED TO HAVE STRUCK HIM WAS FROM BEHIND. THE RIB CONSISTED OF COAL AND ROCK WAS 65" HIGH. THE PIECE THAT IS BELIEVED TO HAVE STRUCK HIM WAS APPROX. 15"X8"X115". THERE WERE
HE WAS IN THE PROCESS OF MOVING THE DRILL UP FROM THE 2ND ROW OF BOLTS TO THE 3RD ROW, WHEN A RIB (MIXED WITH COAL AND ROCK) ROLLED AND STRUCK HIM FROM BEHIND.
EMPLOYEE WAS OPERATING A CONTINUOUS MINER. HE WAS TRAMMING ACROSS SECTION AND PULLING MINER CABLE WITH MINER. HE PULLED SLACK DOWN #7 ENTRY AND PROCEEDED TO BACK MINER THROUGH X-CUT BETWEEN #6 AND #7 ENTRY. AS HE LOOKED BEHIND HIMSELF TO CHECK SLACK ON CABLE, THE RIGHT RIB ROLLED OFF AND STRUCK HIM IN THE BACK AND LEFT SHOULDER AREA, KNOCKING HIM TO THE GROUND.
INJURED STATED THAT HE WAS STANDING AT THE CONTINUOUS MINER WHEN A COAL RIB ROLLED OFF AND HIT HIM IN THE BACK AND LEGS. CAUSING CONTUSION AND STRAIN TO LOWER BACK AND LEG. THERE WERE NO FRACT URES. HE IS TO TAKE THERAPY.
INJ STATED THAT AS HE WAS OPERATING A ROOF BOLTER IN FACE THE FACE WHICH LIKED 2 ROWS OF BOLTS INBY BEGAN WORKING HE STEPPED BACK TO THE BACK OF THE MACHINE WHEN A RIB BROW ROLLED OFF & HIT HIM IN BACK & LEGS. NO BONES WERE BROKEN BRUISEDHIM
INJURED STATE THAT AS THEY EWERE BACKING THE MINER OUT OF A LIFT IN THE #5 ENTRY A RIB ROKKED OUT STRIKING HIM ON HIS LEFT LEG AND FOOT BRUISING IT.
INJURED WAS OPERATING A ROOF BOLTING MACHINE WHEN A RIB ROLLED OUT AND STRUCK HIM IN THE BAKC, CAUSING HIM TO HIT HIS HEAD AGAINST THE DRILL POT. HE WENT TO DR AND WAS RELEASED.
INJURED STATED THAT HE WAS BOLTING TOP ON THE 002-0 SECTION WHEN THE RIB ROLLED OFF AND STRUCK HIM ON THE LEFT FOOT.
INJURED EE WAS BACKING A MINER OUT OF THE RIGHT BREAK BY USING REMOTE CONTROL WHEN HE WAS STRUCKBY A RIB ROLL RESULTING IN A FRACTURED PELVIS AND 3 FRACTURED RIBS.
INJURED STATED THAT HE WAS PULLING MINER CABLE OUT OF ROADWAY WHEN A RIB POPPED OUT & STRUCK HIM IN THE LOSER BACK, BRUISING HIM. HE STARTED MISSING WORK ON 5/2/98.
INJURED STATED THAT AS HE WAS MOVING MINER CABLE OUT OF ROADWAY IN #5 ENTRY OF 002-0 SECTION, A RIB ROLL STRUCK HIM ON HIS LEFT LEG. IT RESULTED INTO A FRACTURED ANKLE, 2 FRACTURED TOES AND A FRACTURE TO A BONE ON TOP OF THE FOOT.
INJURED STATED TAT AS HE WAS STANDING BY TEH ROOF BOLTING MACHINE, MAKING UP BOLTS A RIB ROLLED OFF AND STRUCK HIM ON THE LEG AND FOOT. HE HAD SCALED THE RIBS PRIOR TO THE RIB ROLL. IT RESUL TED IN 5 STITCHES AND 2 FRACTURED BONES IN HIS RIGHT FOOT.
