Huff Creek Mine Coal

A non-injury roof fall occurred in the No.2 Intake Entry-Secondary Escapeway at x-cut No.32 on the (A) Mains. The roof fall is approximately 20'ft in length, x 20'ft in width, x 10'-15'ft in thickness. *See attached roof fall map. There were no injuries associated with the roof fall.

A non injury accident (roof fall) occurred in the #2 entry from break 20 to 22 and through the break at 22 to entry #3. The fall occurred 5ft high up to 8ft high. We are cleaning the fall up and rehabbing the #2 entry. We are cleaning it up and using 6ft resin rods as well as 12ft rope bolts every other roll.

A roof fall was discovered on this date. The fall was located on the B-2 Mains in the number 5 entry at crosscut 44. The fall measured 25' by 18' and was 7' in thickness. The area was previously supported with 5' Hy-Tech bolts and 12' super bolts. The fall did not affect persons or ventilation. The fall was cleaned up and the area re-supported.

A roof fall occurred in the 2-East Right Return at crosscut #15 in the #5 entry. The fall measured 25' by 20' and was 8' in thickness. The area was originally supported with 6' torque tension bolts and 8' cable bolts. No persons or ventilation was affected and the return air course was re-routed.

Employee was unloading water line when it twisted in their hand, cutting their left index finger. Employee was taken to ER where they received 4 sutures and was released to full duty.

Employee was cutting belt away from MRS Valve when knife slipped, cutting left ring finger. Employee was transported to ER where the employee received 5 sutures and was released back to full duty.

Employee was bolting top when bolt bent, mashing right hand against mast head and crushing right ring finger from the nail bed up. Employee was taken to the ER where ee required sutures and was referred for further treatment.

A fall was located on the B-3 belt in the #3 entry at crosscut 15. A fall had previously occurred here in 2017. An additional 2 feet of rock fell in this location on this date. The area was previously supported with 6' resin bolts and 12' cable bolts. No persons or ventilation was affected. The fall was cleaned up and the area was re-supported.

A roof fall was discovered on the A-Mains roadway at crosscut 35 in the #3 entry. The fall measured 24' by 20' and was 10-12' in thickness. The area was previously supported with resin bolts, truss bolts and beams. No persons or ventilation was affected.

Employee was bolting 4R crosscut, break 9 when draw rock fell, hitting EE in the left arm and causing a three inch cut.

A roof fall was discovered on this date. The fall was located in the #1 entry of A-Mains between crosscuts 27 and 29. The fall was of various heights. The area was previously supported with 5' Hi Tech bolts. No persons or ventilation was affected and the fall is being cleaned up.

The roof fall was located on B-2 Mains at crosscut #41 in the #2 entry. The fall measured 20' by 20' and was 10' in thickness. The area was previously supported by with 5' Hi Tech bolts and 12' cable bolts. The roadway and lifeline was re-routed and the fall will be cleaned up. No persons or ventilation was affected

While operating a roof bolter walking behind it a chain broke and a piece of metal struck employee in the right eye.

During preshift exam a roof fall was discovered on the 2-East belt inby crosscut #29. The fall measured 16' by 16' and was 6-8' in thickness. No persons or ventilation was affected. The area was previously supported by 6' torque tension and 12' cable bolts. The area was cleaned up and re-supported.

Repairing a tire on mantrip, using a 4lb hammer, ee missed rim & pinched tip of right index finger.

EE was helping take ATRS pads off on 1 side the pad on other side turned over & hit self in face & fell against rib. EE had broken teeth & palate.

A roof fall occurred on the B-2 Mains #3 entry at crosscut #44. The fall measured 20' by 20' and was approximately 8' in thickness. The area was previously supported with 5' Hi Tech bolts and Super Bolts. Ventilation was not affected nor were any persons. The fall will be cleaned up and re-supported.

Employee reached behind a tool box to retrieve some welding rods. As employee pulled the welding rods out the employee lacerated left forearm on a sharp edge of the toolbox. Employee received 10 sutures to the wound.

Employee has a history of back issues. Due to ongoing back pain employee sought medical treatment on 8/21/2017. Employee was diagnosed with Facet Joint Hypertrophy and Early Vacuum disc phenomenon (Degenerative Disc Disease)

A piece of draw rock fell striking employee on the back of the legs and ankles causing a fracture to left heel.

Employee has been suffering from reoccurring back pain. On this date the employee sought medical treatment for the back pain. Employee was diagnosed with subtle degenerative changes in the back and was taken off work.

While loosening a chain ratchet from a motor, the employee felt left shoulder pop. Employee was diagnosed with a shoulder strain.

Removing coupling from water line, struck by a line.

As employee was leaving the operator deck of the MBM, slipped and fell striking right ribs against the bridge causing a fracture of one rib. Employee has continued working restricted duty.

A reportable roof fall occurred on this day on the MMU-003 section. The fall was located on the 1-Right Panel inby crosscut #28 in the #4 entry. The fall measured 20' by 20' and was 10'+ in thickness. The area was re-supported and mining continued.

A roof fall occurred on this date. The fall was located on 1 Right Panel off 3-East Sub Mains at Crosscut 27 in the #3 Entry. The fall measured 30' by 30' and was 10-12' in thickness. A mobile bridge unit was involved but there were no injuries. The area was previously supported with 8 and 12 foot cable bolts.

While employee was hanging the miner cable a piece of draw rock fell striking right hand which was still on the cable. The employee received a laceration on right middle finger which required sutures. EE was wearing gloves at the time of the incident and returned to work next scheduled shift.

A roof fall was discovered on this date. The fall was located on the MMU-006 just outby crosscut #10 in the #4 entry. The fall measured 20' by 18' and was 8-10' in thickness. The area was previously supported with 6' Tension rebar bolts and 8' and 12' tensionable cable bolts. The fall did not affect ventilation.

As employee was installing a cable bolt, the strap that was between the bolt and top began spinning and struck the employee on right cheek causing a laceration that required sutures.

A roof fall has occurred in the 13-Left Panel off B-Mains. The fall was located between the #10 and #11 crosscuts in the #4 entry. The fall measured 40' by 18' and was 8' in thickness. The area was originally supported with 6' torque tension bolts with supplemental 12' cable bolts. There were no injuries and ventilation was not affected.

Employee was moving a sump pump. The pump line was tied off at the pump and lo-trac forks. While the operator was pulling the waterline with the lo-trac, the waterline got caught on the feeder causing the rope hangers to break allowing the pump to strike the operator on the left side of the face. He suffered fractures to the left side of his face.

Employee reported having reocurring back pain. On this date he sought medical treatment. The results of an MRI shows spondylosis (osteoarthritis) from L1-2 to L5-S1 vertebrae. Employee began missing work on 5/7/2015 for treatment.

On this date the employee stated he was having pain in his lower back. He started missing work because of medical advice on 10/7/2014, and on 10/10/2014 it was determined that the illness was work related. The employee has degenerative disc disease affecting L3, L4 and L5 making this an occupational illness.

While setting a motor and speed reducer for a belt drive, a chain on a come along hoist broke and struck employee in the face around the left eye causing a non-displaced fracture and a laceration that required sutures.

While employee was lifting a bag of rock dust, he felt pain in his lower back. He began missing work on 1/10/2014.

A coal/rock outburst occurred while retreat mining on this date. The outburst occurred on 6-Left Panel off B-4 Mains inby Break #35 in the #3 entry. There was one fatality and one serious injury related to this outburst which have been previously submitted on submissions # 1769828 and #1769459.

While conducting retreat mining on 6-Left Panel off B-4 Mains, a coal/rock outburst occurred inby break #35. The outburst involved the Mobile bridge operator who suffered multiple blunt trauma injuries involving the chest, right arm, and back.

As employee was loading belt tools into a mantrip, he twisted and felt pain in his lower right back. He has been diagnosed with a herniated disc in his lower back.

A roof fall was discovered on at this time. The fall was located on C-West, #2 entry, at crosscut #2. The fall measured 25' by 30' and was 6' in thickness. Ventilation was not affected, nor were any employees. The area was originally supported with 5' H/Tech bolts and 12' cable bolts.

On this date, the employee started having back pain. It has been determined that the employee has degenerative disc disease and that the employee has an occupational illness. He had surgery on his back on 10/23/2013.

While operating the continuous miner a piece of rock flew back from the cutting head and struck the employee in the left leg causing a laceration requiring sutures.

On this date the company was notified that this miner had developed black lung disease.

His regular job is shoveling belts and he began having pain to his lower back and said it became worse as the morning progressed. He complained to his supervisor and was taken outside. He was able to drive himself for medical evaluation and treatment.

A section roof fell outby the MRS units causing them to be trapped. The fall was located in the 4 LT panel inby the 19th x-cut in the #2 entry. The area was previously supported with 5' HY-Tech bolts and 12' cable bolts. There was no persons or equipment involved with the fall.

While retreat mining was being conducted in the #4 entry inby the 19th x-cut lithe 4 LT panel, a section of roof fell outby the MRS units. The units were could not be moved outby because of the fall. The fall measured 10' long by 18' wide and was 5' in thickness. There were no injuries.

The employee was moving a piece of belt structure when he felt pain in his right shoulder. He continued working until 8/8/2011 when he began missing work due to a torn rotator cuff.

Employee was working with a crew construction seals underground. He dropped a concrete block and mashed his right thumb between two blocks. He required medical treatment and received stitches to close the wound.

As the employee was removing the drill steel from a rib bolt hole, a piece of rib rock slid down the steel, striking his left hand. He suffered a fracture and laceration of his left index finger.

When the employee took a bolt out of the rear end of a scoop, the rear end dropped down and caught the employee's left hand between the impact wrench he was holding and the floor of the shop. The employee suffered a laceration to the palm of his left hand.

Employee had been having issues with his lower back which were not attributed to a particular work activity. Employee sought medical treatment and was diagnosed with degenerative disk desease in his lower back. He began missing work on 3/24/2011 to begin receiving treatment for his condition.

A roof fall was discovered on this date. The fall was located in the 3-South return at crosscut #4 in the #1 entry. The fall measured 40' by 20' and was 6' in thickness. The area was originally supported with 5' torque tension bolts. The area was dangered off. No equipment or persons was affected.

Employee began having back pain on 12/01/2010. He sought medical treatment and on 1/13/2011 he was diagnosed with degenerative disc disease and was taken off work for further evaluation.

While the employee was lining up drill steel to start a bolt hole, he moved the drill head and caught his left thumb between the steel and the ATRS rock deflector. He suffered a laceration on his left thumb requiring sutures.

As the employee was tramming the scoop up the roadway, a piece of draw rock fell striking the scoop. When the employee stopped the scoop suddenly, he was thrown into the side of the operators deck straining his back. He started missing work on August 16th, 2010.

While the employee was repositioning a strap while roof bolting, he caught his wrist between the drill head and the rock deflector on the roof bolter. He suffered a contusion to the wrist which required medical treatment. He returned to work for his next scheduled work shift.

A roof fall was discovered in the B-4 Returns at crosscut #8 in the #1 & #2 entries. The falls in each intersection measured 20' X 20' and was 5-7' in thickness. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts. The falls were dangered off and the returns will be rerouted.

While drilling a hole to install a roof bolt, a small piece of drawrock fell and struck the operator on the upper lip causing a laceration requiring sutures.

A roof fall was discovered on this date. The fall was located on B-4 Mains in the #2 entry between break 4 and 5. The fall measured 18' by 20' and was 6' in thickness. The area was previously supported with 5'Hy-Tech bolts and 12' cable bolts. Ventilation was not affected.

A roof fall occured on this date. The fall measured 18' by 25' and was 6' in thickness. The fall was located in the #1 entry of B-4 mains between breaks 4 and 5. Ventilation was not affected. The area was previously supported by 5' Hy-Tech bolts and 12' cable bolts.

A roof fall was discovered on this date. The fall measured 19' by 40' and was 8' in thickness. The fall was located on the B-4 Mains on the #1 entry at break #6. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts. Ventilation was not affected.

A roof fall occurred on this date. The fall was located on 002 section in the 7-Left Panel off C-Mains in the #1 entry at break #13. The fall measured 25' by 18' and was 8' in thickness. No miners or equipment were affected. This section has been moved to another location in the mine. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts.

Employee struck a can of paint against a roof bolt plate. The paint can burst, causing the employee to suffer a laceration to his right middle finger, requiring sutures.

While employee was bolting a strap slid off the ATRS and stuck employee on the cheek causing a laceration requiring sutures.

While trying to move a piece of rock out of a travel way, the employee felt pain in his lower back. He began missing work on 8/20/2009.

The employee was assisting another miner to load utility doors onto a scoop when he said he felt a pop in his lower back. The area became increasingly painful over the next couple days and he sought medical attention.

After having incidents of back pain over several years, employee has had to cease working due to back pain. An MRI performed on 7/15/2009 shows degenerative disc disease at the L4-L5 and L5-S1 and to a lesser degree in other areas. He said that this did not occur due to one particular event. He began missing work on 7/15/2009.

A roof fall was discovered during an exam. The fall measured 20' by 20' and was 5-7 feet in thickness. The fall was located in the 1-Left bleeder entry off B-3 Mains, #4 crosscut, between the #2 and #3 entries. The fall was dangered off.

A roof fall was discovered on this date. The fall was located on C-Mains, Break #17 in the #5 entry. The fall measured 20' by 20' and was 8-9' in thickness. The area was previously supported with 5' HY-tech bolts with supplemental 8' superbolts. There were no injuries or equipment involved.

A roof fall was discovered during a preshift exam. The fall was located inby the 2-Left-1 head drive which is located in the #13 x-cut. The fall consisted of laminated shale. The fall measured 20' wide by approx. 40' in length. The fall was cleaned up and the roof resupported.

A roof fall was discovered on this date. The fall measured 20' by 20' and was 5-6' in thickness. The fall was located on 2-North Mains at the #88 crosscut in the #3 entry. The fall did not affect ventilation. The fall has been dangered off.

A roof fall was discovered in the #1 entry of the 3-Left Panel off 2-Left #2. The fall measured 18 feet by 60 feet and was 6 feet in thickness. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts. No equipment was involved. The area was dangered off.

As the injured employee was walking down the # 2 entry in the 1-Left panel a piece of draw rock measuring 36" by 20" and 2 inches in thickness fell striking his right arm causing a laceration requiring sutures.

A roof fall was discovered on this date. The fall was located on 1-left 2 belt between the #60 and #61 breaks in the #3 entry. The fall measured 20' x 70' and was 8' in thickness. The area was previously supported with 5' hy-tech bolts and 8' superbolts. The fall area will be cleaned up and re-supported. The fall was immediately reported to MSHA.

A roof fall was discovered on this date. The fall is located on 6-north, break 4, between then #2 and #3 entries. The fall was approx. 15' long by 20' wide, and 5' in thickness. the area was previously supported with 5' hy-tech bolts and 8' superbolts. The area will be dangered and cribbed off. Ventilation was not affected.

A roof fall was discovered during an examination on this date. The fall was located at break #9 of 2-Left between the #2 and #3 entries. The fall measured 14' by 20' and was 8' in thickness. The area was previously supported by 5 foot torque tension bolts and supplemental superbolts. Ventilation and travel were not affected. The fall was cribbed off.

As employee was installing a bolt into a hinge pin collar on the #3 MBM, he was told that the #1 & #2 MBM was going to be moved to pull back Lo-Lo belt. He replied that he would be out of the way and to proceed. When the MBMs were moved, his thumb was caught between the collar and the hinge pin, resulting in the amputation of the tip of his left thumb.

A roof fall was discovered on this date. The fall was located in the #3 entry, #4 crosscut of 6-North. The fall measured approximately 20' x 30' x 12' in thickness. The fall did not affect production or ventilation. The area was previously supported with 5' hi-tech bolts and 8' super bolts. The area was cribbed and dangered off.

A roof fall was discovered on this date and time. The fall was 40' in length, 20 feet in width and approx. 5-6 feet in thickness. The fall was located approx. 70" inby the #22 crosscut in the #1 entry on A-mains return. No equipment was involved.

Employee was plastering a stopping when a piece of draw rock fell from between roof bolts, struck his back, went down his leg, and struck his right ankle. He suffered a fracture of the ankle.

As employee was adding transmission fluid to a diesel mantrip, the hood on the mantrip fell striking the employee on the right thumb causing a laceration requiring sutures.

A roof fall was discovered on this date. The fall was located in the #1 entry at break #41 in the B-2 returns. The fall was approx 20'h x 30-35' l extending inby towards break #42. The area was originally supported with 5' high tech bolts & supplemental 12' super bolts.

EE has made a claim under our worker's comp alleging that he injured his back while lifting a 50 lb bag of block bond on 2/24/07. He had not made any mention of injury that day to his supervisor or to his co-workers. It was 12 to 14 days before we learned he was seeking treatment. I then had to wait to hear from him to get details as he had had his phone number changed.

A roof fall was discovered at Break 70 on 3-North in the #9 entry. The fall was 20' x 60' x 5' in thickness. The fall extended through the break to the #8 entry. The area was originally supported with 5' High Tech Bolts with 8' Super Bolts as supplemental support.

A roof fall was discovered in the #2 entry of 6-East that measured 20' x 200' x 7' in thickness. The fall extended from break #6 to break #8. The area was originally supported with 5' High Tech bolts with 8' Super bolts as supplemental support.

Elder-Ridge Contracting Company (4 men) were changing bearings on a snub roller of the Huff Creek Mine slope belt located on the surface. The crane boom failed, causing boom to fall striking 3 contractor employees. Injured received on fractured toe and questionable cracked ribs(3).

A roof fall was discovered on the B-Mains #2 Belt at the 39th break. The fall was located in the intersection and measured approx 20' X 20' X 10' in thickness. The area was originally supported with 5' High Tech Bolts and 8' Super Bolts.

A roof fall was discovered int he old belt entry (#3 entry) of the 5 west panel at break #4. The fall was approx. 20' x 20' x 8'. The area was originally supported by 5' hi-tech bolts and spotted with 8' super bolts.

Employee was driving a 4-man ride along the roadway when a piece of draw rock of undetermined size fell & struck his upper lip & chin causing lacerations requiring sutures.

A roof fall was discovered in the #5 entry of the 10 Right Panel; from the intersection of the #11 crosscut to outby corner of #12 crosscut. The fall was approximately 80' long, 20' wide and 8' thick. the area was originally supported with 5' high tech bolts and 8' super bolts.

A roof fall occurred on the A-mains section at #28 crosscut. The fall was approximately 20'x20'x8' thick. The area had been supported with 5' hight tech bolts. The fall was in the #4 entry.

As employee was removing bolts from a head drive speed reducer, the ratchet he was using slipped causing him to strike his right elbow on the speed reducer. He continued to work and received medical treatment to the elbow on 1/15/07.

He was drilling a roof bolt hole when a rock approx 21" X 22" X 2" thick fell and struck him on the arm, resulting in a laceration.

A roof fall was found in the 5 west panel at crosscut #39 in the #3 entry outby #3 seal. The fall was approx. 8' high x 20' wide x 50' long. 5' high tech bolts supplemented with 8' super bolts and been installed in the area.

He struck a sprocket on a bridge conveyor with a sledge hammer and a piece of the metal chipped off and struck his left hand. He continued to work and had the metal removed by out patient surgery on 6/23/06.

A roof fall occured on the 003 section belt at # 13 crosscut inby spad # 21520. The fall was approx. 8' high X 20' wide X 18' long. The area had been bolted with 5' High tech bolts.

He was installing lo-lo conveyor belt structure on the B-mains belt. The operator of the fork lift being used to install the structure, bumped the structure while employee's finger was in between two pieces of the structure.

A roof fall occured on the B-mains section in the #4 entry at #52 crosscut. The fall was approx. 20'x20'x12'thick. The area had been supported with 5' high tech bolts and supplemented with 8' super bolts.

HE WAS WALKING UP TO THE CONTINUOUS MINER WHEN A PIECE OF ROCK BOUNCED OFF OF THE FRONT MOBILE BRIDGE HOPPER & STRUCK HIS LEFT LEG. HE CONTINUED TO WORK & BEGAN MISSING ON 4/04/06.

EMPLOYEE STATED THAT HIS LIGHT WENT DEAD & HE STEPPED IN A HOLE IN THE MINE FLOOR, TWISTING HIS KNEE.

As he was operating a roof bolting machine, he bumped hard into ATRS portion of the machine with his shoulder. He continued to work, but started missing work on August 14,2006 to have rotator cuff surgery.

A roof fall occurred on the 005 section between #41 & #42 crosscuts in the #1 entry. The fall was approximately 14' x 12' x 6' high. The area had been supported with 5' high tech bolts and supplemented with 8' super bolts.

A roof fall occured on the 3 north roadway at #40 crosscut. The fall was approx. 20'x20'x6' thick and located in the #4 entry. 5' high tech bolts had been installed and supplemented with 8' super bolts.

Employee was moving gob out of a crosscut with a diesel scoop. The scoop dislodged a piece of victaulic water line which struck the employee on the right arm. The employee started receiving physical therapy for the injury on 1/25/07.

While roof bolting, a small piece of rock fell from the roof and struck his nose. He started missing work on 5/15/06 and had cosmetic surgery.

A fall occurred between #4 & #5 entries at crosscut #32 inby spad #24813. The fall was approximately 20' x 10' wide x 5' thick. The area had been supported with 5' high tech bolts and supplemented with 8' super bolts.

A small piece of rock fell from the roof and struck him on the nose.

A roof fall was found in the 5 west panel from the #2 entry to #4 entry at spad 24235.

He was operating a Lo-track and cleaning the bottom on the 7 east belt line when a piece of rock fell from between bolts and struck him.

A ROOF FALL WAS FOUND IN THE #5 ENTRY OUTBY THE #5 SEAL ON 5 NORTH MAINS. THE FALL IS APPROXIMATELY 150' INBY SPAD #9473. THE FALL IS APPROXIMATELY 5' THICK X 20' WIDE X 40' LONG. THE AREA HAD BEEN SUPPORTED WITH 5' RESIN BOLTS.

Employee stepped into a hole and twisted his ankle.

He was removing drill steel from a hole in the roof. Two pieces separated and the deepest piece fell from the hole and pinched his finger against the bolt wrench.

Employee was pulling structure when a rail slipped from his hand and hit the top of his left foot causig a contusion.

A roof fall was discovered in the # 1 entry of A Mains panel; inby # 28 crosscut, extending to midway of # 29 crosscut. Fall is approximately 60' long; 18' wide and 7 feet high. The affected area was originally bolted with 5' hi-tech bolts and cribs added in the area.

He was changing a cell in a scoop battery. When he placed the old cell on the mine floor, fluid splashed up into his eye.

A rock fall was discovered int he #5 entry of the Bii Panela: in the #49 crosscut. Fall was approx. 30' x 20' x 8' long. Fall was 4 breaks outby the face of the 005-0 section. The area orignially was bolted with 5' hi-tech bolts; and some 8' super bolts added for supplemental support. MSHA was notified of the fall.

Employee was in the process of drilling a roof bolt hole when a rib rolled out and struck him. He continued to work and started missing work on 12/19/05.

A rock fall was discovered int he #5 entry of the % west Panel; at the #24 crosscut. Fall was approx. 18' wide x 18' long. Height was undetermined. The area had originally been bolted with 5' hi-tech bolts; and some 8' super bolts added for supplemental support. MSHA was notified of the fall.

A roof fall was discovered int he #4 entry of the 5 west panela; between the #49 and #50 crosscuts, just inby spad 24080. Fall was approx. 70' x 20' x8' long. The area had orignially been bolted with 5' hi-tech bolts; and some 8' super bolts added for supplemental support. MSHA was notified of the fall.

A roof fall was discovered between the #39 and #40 crosscuts in the #6 entry of A Mains Panel; inby spad #21153. The fall was approximately 80' long; 20' wide and 8' high. The fall was located in the intake escapeway area. The escapeway route was rerouted around the affected area.

EE STATED THAT HE WAS OPERATING A DIESEL LO-TRAC, PLACING A MOTOR IN PLACE FOR A FUTURE BELT HEAD DRIVE INSTALLATION. HE GOT AGAINST THE COAL RIB WITH THE LO-TRAC, CAUSING SOME MATERIAL TO FALL FROM THE RIB. THE RIB ROLL STRUCK HIM ON HIS RIGHT LEG, CAUSING A FRACTURE TO THE TIBIA AND FIBULA IN HIS RIGHT LEG.

AS HE WAS HELPING LIFT A RAM JACK FOR A SCOOP INTO THE SCOOP BUCKET, HE GOT HIS RIGHT LITTLE FINGER CAUGHT BETWEEN THE JACK AND SCOOP BUCKET. IT RESULTED INTO A FRACTURE AND LACERATION TO THE END OF THE FINGER

A roof fall was discovered at the 48th crosscut of the 5 west mains panel in the #3 entry. The fall occurred in the belt entry. The fall is approx 30' l, 20' w and 7' h. The area had been bolted with 5' high Teck bolts, and spotted with 12' bolts.

HE BENT OVER TO PICK UP A ROOF PLATE AND FELT PAIN IN HIS BACK. HE CONTINUED TO WORK & STARTED MISSING WORK ON 01/13/06.

He felt pain in his shoulder while lifting a 4" water line.

A roof fall was discovered ont he 5 west belt at #15 crosscut int he #3 entry at spad #2350. The fall was approx. 23' long, 20' wide and 7' high. the area had origninally been bolted with 5' bolts, supplemented with 8' super bolts.

WHILE OPERATING A CONTINUOUS MINER FROM A REMOTE CONTROL, A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLT AND RIB AND STRUCK HIM IN THE BACK. IT RESULTED IN 3 FREACTURED VERTEBRAE. IT OCCURRED ON THE 005-0 SECTION. AT THIS POINT, SURGERY HAS NOT BEEN REQUIRED.

A FIRE OCCURRED INSIDE OF A COMBINATION POWER CENTER LOCATED AT THE #11 CROSSCUT OF THE 6 NORTH PANEL. THE FIRE WAS CONTAINED INSIDE OF THE POWER TRANSFORMER ENCLOSURE. NO FIRE DAMAGE WAS NOTED OUTSIDE OF THE TRANSFORMER BOX. THE BOX SUPPLIED POWER TO THE 5 WEST BELT DRIVE. THE MINE WAS IDLE AND NO ONE WAS UNDERGROUND WHEN FIRE STARTED.

EE WAS LOSSENING BOLTS ON A BELT FEEDER SPEED REDUCER. AS THE NUT LOSSENED, EE STRAINED RIGHT SHOULDER. EE CONTINUED TO WORK UNTIL EE HAD SURGERY ON 4-26-05 TO REPAIR A TEAR TO RIGHT ROTATOR CUFF

A non-injury roof fall was discovered at # 28 crosscut on the A Mains Panel in the # 3 entry (belt entry). The fall was approximately 20' x 20' x 12'(high). The area had been previously bolted with 5' high tech bolts.

EE WAS POSITIONED ALONG THE RIGHT RIB OF THE #4 ENTRY, WHEN THE RIB ROLLED OFF AND STRUCK EE IN THE BACK, IT RESULTED IN 2 FRACTURED RIBS AND A FRACTURE TO RIGHT HEEL ALONG WITH SOME CONTUSIONS TO HEAD, BACK, LEGS AND CHEST. INJURY OCCURRED ON THE 002-0 SECTION.

A non-injury roof fall was discovered in the #2 entry of the 005-0 section approximately 1 break outby spad #22476. The fall occurred while the mine was idle. The fall was about 6' high, 110' long and 20' wide. The roof fall was partially on the roof bolting machine but bolter was later removed.

A non-injury pillar roof fall occurred in the #4 entry, 1 break inby spad #22474, on the 005-0 section in the A mains panel. The 2 Fletchr mobile roof support machines were caught in the fall but later retrieved from the fall area.

EE STATED THAT HE AND COWORKER WERE AT THE SURFACE MAINTENANCE SHOP ATTEMPTING TO PULL OFF THE LEFT SIDE DRUMS FROM THE CONTINUOUS MINER. THEY WERE USING PORTA-POWERS (50 TON). THEY HAD A SLEDGE HAMMER AS A SPACER. THE PORTA-POWER AND HAMMER KICKED OUT AND THE HAMMER STRUCK EE ON THE FOREHEAD, CAUSING A LACERATION REQUIRING 4 STITCHES.

He stated that after dismounting from battery ram car, he slipped on a loose piece of draw rock ont he mine floor, causing him to fall to the ground. as he was falling, he tried to catch his balance with his right hand, causing soreness & swelling. he continued to work and had a specialist diagnose it on 10-27-04 as having a fracture to the 4th metacarpal bone in his right hand.

A NON-INJURY PILLAR FALL OCCURRED IN THE #2 ENTRY OF THE 2 LEFT PANEL (MMU 003-0) AT THE #43 CROSSCUT, 1 BREAK INBY SPAD #22337. THE REMOTE-CONTROLLED CONTINUOUS MINER WAS CAUGHT BY THE PILLAR FALL IN RETREAT MINING.

The results of a lumbar MRI scan showed that employee has substantial lumbar degenerative disc disease at the third, fourth and fifth lumbar levels. He started missing work due to the illness on 9-29-04.

EMPLOYEE STATED THAT AS HE WAS WALKING IN #3 ENTRY OF THE 002-0 SECTION, A PIECE OF DRAW ROCK FELL FROM BETWEEN THE ROOF BOLTS IN THE MINE ROOF AND STRUCK HIM ON HIS RIGHT FOREARM. IT CAUSED A LACERATION AND REQUIRED SURGERY TO REPAIR SOME MUSCLE AND TENDON DAMAGE.

A ROOF FALL WAS DISCOVERED BETWEEN THE #2 AND #3 ENTRIES, JUST OUTBY SPAD 22164 IN THE 1 RT BLEEDER OFF OF THE "A" MAINS PANEL. FALL IS APPROXIMATELY 6' X 20' X 50'. THE AREA WAS ORIGINALLY BOLTED WITH 5' BOLTS.

A roof fall was discovered between the #2 and #3 entries, inby spad #22164 in the 1 Rt. Bleeder off of the "A" Mains Panel. Fall is approximately 6'x 20'x 50'. The area was originally bolted with 5' bolts.

AFTER HAVING DEGENERATIVE BACK PAIN, EMPLOYEE WAS DIAGNOSED AS HAVING PRESENCE OF DEGENERATIVE PROBLEMS AT BOTH L4 & L5 DISC LEVEL WITH A RT. L5 DISC FRAGMENT INTO THE RT. S1 NERVE ROOT. HE DID NOT RECALL ANY INDIVIDUAL INCIDENT THAT WOULD HAVE CAUSED THE CONDITION OR ILLNESS. HE HAD SURGERY TO REPAIR THE ILLNESS & STARTED MISSING WORK ON 9/7/04.

A roof fall was discovered on the 3 North Panel at #45 crosscut, between #2 and #3 entries. The fall was adjacent to spad #17447. The fall was approximately 6' high, 20' wide and 45' long. The area had been originally bolted with 5' bolts. The area was cribbed off.

He stated that he was attempting to take down the roof bolter cable that was hung on the mine roof by nylon rope hangers. After not being able to get one of the hangers loose, he took a utility belt knife and cut the rope hanger down. In doing so, the knife struck his left forearm, requiring 4 stitches. This occurred on the 005-0 section.

A roof fall was discovered in the #5 entry of a mains #54 crosscut at spad #21437. The fall was approx. 20'x20'x6' high. the area had originally been bolted with 5' bolts. the roof fall fell on the high voltage cable. the area was cribbed off.

HE WAS PULLING MINER CABLE OUT OF THE WAY IN THE ROADWAY ON THE 003-0 SECTION. HIS BACK STARTED HURTING HIM. HE CONTINUED TO WORK UNTI 7/22/04. HE WENT TO THE DOCTOR THEN. HE SAW A SPECIALIST ON 8/2/04 AND MRI WAS SCHEDULED.

A ROOF FALL OCCURRED IN THE "A" MAINS 1 RIGHT PANEL AT THE #9 CROSSCUT BETWEEN THE #2 AND #3 ENTRIES, ADJACENT TO SPAD #21897. THE FALL WAS APPROXIMATELY 5' HIGH X 19' WIDE X 50' LONG. THE FALL AREA HAD BEEN BOLTED WITH 5' BOLTS.

A ROOF FALL WAS DISCOVERED AT APPROX. 9:20 A.M. IN THE #2 ENTRY OF THE 1 RIGHT PANEL OFF OF A SOUTH SUB-MAINS; AT THE #17 CROSSCUT; 1 CROSSCUT OUTBY SPAD #22009. THE FALL IS APPROX 5' THICK, 20' WIDE AND 30' LONG. THE AREA WAS BOLTED WITH 5' BOLTS. THE AREA HAS BEEN CRIBBED OFF.

HE STATED THAT HE WAS IN THE PROCESS OF MOVING A PINCH ROLLER FOR A BELT STORAGE UNIT USING A DIESEL LO-TRAC. AS HE WAS LOOSENING THE C.M. PULLERS THAT HAD THE ROLLER SECURED, THE WEIGHT SHIFTED AND CAUGHT IN HIS LEFT MIDDLE FINGER BETWEEN THE CHAIN & FORK OF THE LO-TRAC, MASHING IT. THIS RESULTED INTO A FRACTURE TO END OF HIS FINGER & STITCHES.

A ROOF FALL WAS DISCOVERED AT THE OLD 4 WEST MAINS BELT DRIVE ASSEMBLY; EXTENDING FOR APPROXIMATELY 50' INBY SPAD # 18942. THE FALL AREA IS APPROXIMATELY 50' LONG, 20' WIDE AND 10' HIGH. THE FALL OCCURRED ON THE BELT DRIVE ASSEMBLY.

A ROOF FALL WAS DISCOVERED AT THE MOUTH OF THE OLD 4 WEST MAINS PANEL, IN AN INTERSECTION AT SPAD #18930. THE FALL WAS APPROXIMATELY 20' WIDE, 20' LONG AND 10' HIGH. THE AREA HAD BEEN BOLTED WITH 5' BOLTS.

WHILE WALKING TO THE ROOF BOLTING MACHINE ON THE 0003-0 SECTION, A PIECE OF DRAW ROCK FELL FROM THE MINE ROOF AND STRUCK HIM ON THE HEAD, NECK AND SHOULDER. THE ROCK FELL FROM BETWEEN BOLTS AND STRAPS PREVIOUSLY INSTALLED. HE COMPLAINS OF PAIN IN THE NECK AND SHOULDER AND IS OFF TAKING THERAPY.

A ROOF FALL WAS DISCOVERED AT 1:20 PM ON 3-9-04 IN THE #3 ENTRY OF THE 5 LEFT PANEL, BEGINNING AT APPROXIMATELY 50 FEET INBY SPAD #17054. FALL WAS APPROXIMATELY 50'LONG, 20'WIDE AND 7'HIGH. TH E AREA ORIGINALLY WAS BOLTED WITH 5'HI-TECH BOLTS AND SUPPLEMENTED WITH 12' SUPER BOLTS. AREA HAS BEEN CRIBBED OFF.

A ROOF FALL WAS DISCOVERED WHILE MAKING A PRE- SHIFT EXAM IN THE 13 LEFT PANEL AT SPAD #21451 IN #3 ENTRY AT #56 CROSSCUT. THE FALL WAS APPROX20' IN LENGTH, 30' WIDE AND 5-8' THICK. THE FALL SET DOWN ON THE HEAD OF THE CONT MINER. THE AREA HAD PREVIOUSLY BEEN BOLTED WITH 5' HY TECH BOLTS AND SPOTTED WITH 12' SUPER BOLTS.

After bolting a cut, EE stepped onto roof drill of ATRS to hang reflectors in the roof bolt plate. The bolter operator dropped the ATRS to move the machine. He was caught in a pinch point.

A ROOF FALL WAS DISCOVERED AT APPROXIMATELY 7:30 P.M. ON THE "A" MAINS PANEL IN #2 ENTRY, AT SPAD #20985, AT THE #28 CROSSCUT. THE FALL WAS APPROXIMATELY 20'WIDE, 20'LONG AND 7'HIGH. THE AREA HAD BEEN SUPPORTED WITH 5' HY-TECH BOLTS.

A ROCK FALL WAS DISCOVERED IN THE A MAINS PANEL OF THE HUFF CREEK MINE, INBY #35 CROSSCUT IN THE #5 ENTRY, BEGINNING INBY SPAD #21059 & EXTENDED INBY FOR APPROX. 70'. THE FALL WAS ESTIMATED AT 9'HIGH. THE AREA HAD BEEN BOLTED WITH 5'HY-TECHBOLTS. THE AREA WILL BE CRIBBED UP.

ROCKFALL WAS DISCOVERED IN THE 3 NORTH PANEL, AT #39 CROSSCUT, BETWEEN #4 & #5 ENTRIES. FALL WAS ADJACENT TO SPAD #17089. FALL WAS APPROX. 8'HIGH. THE AREA HAD BEEN BOLTED W/5' HI-TECH BOLTS. AREA WAS CRIBBED OFF.

ROCKFALL WAS DISCOVERED IN THE #2 ENTRY OF THE 4 WEST PANEL AT THE #52 CROSSCUT 1 BREAK INBY SPAD #20413. FALL WAS ~90' BY 50' & 10' HIGH. AREA BOLTED WITH 5' TORQUE TENSION BOLTS. & 12' SUPER BOLTS SPOTTED FOR SUPPLEMENTAL SUPPORT.

A ROOF FALL WAS DISCOVERED ON THE WEEKLY EXAM IN THE 3 NORTH RETURN AT #14 CROSSCUT OF THE 3 NORTH PANEL, OFF OF #1 ENTRY; EXTENDING INTO THE 2 LEFT PANEL PROJECTION AREA IN THE #2 ENTRY OF 2 LEFT. THE FALL IS APPROXIMATELY 7 TO 10 FEET HIGH; 20 FEET WIDE; AND 360 FEET IN LENGTH. THE AREA ORIGINALLY WAS BOLTED WITH 5' TORQUE TENSION BOLTS.

A ROOF FALL OCCURRED ON THE 002-0 SECTION OF THE 10 LEFT PANEL, BETWEEN #2 AND #3 ENTRIES AT #5 CROSSCUT. THE FALL WAS APPROX. 70' LONG 18' WIDE AND 6'-8' HIGH. PART OF THE FALL CAUGHT THE #4 BRIDGE BREAKER CAR WHICH WAS PARKED AT THE TIME ON THE BELT LINE. FALL WAS AT SPAD 20802. NO INJURIES OCCURRED.

HE STATED THAT AS HE WAS OBSERVING THE MINE ROOF, HE ATTEMPTED TO STEP UP ONTO THE WALKWAY OF THE ROOF BOLTING MACHINE. HE TRIPPE ON THE REAR HITCH AND FELL ONTO THE WALKWAY, HITTING HIS RIGHT KNEE ON THE WALKWAY. IT WAS DIAGNOSED AS AN "AVULSION OFF THE SUPERIOR POLE OF THE RIGHT PATELLA" (FRACTURE).

WE RECEIVED THE DIAGNOSIS REPORT OF DEGENERATIVE DISC DISEASE ON 10/9/03 OF THE INJURED EE.

HE STATED THAT AS HE WAS OPERATING THE CONTINUOUS MINER IN RETREAT MINING, A PIECE OF DRAW ROCK FELL OUT OF THE MINE ROOF FROM BETWEEN BOLTS AND STRUCK HIM ON THE RIGHT SHOULDER AND BACK. HE STARTED MISSING WORK ON 10-07-03 AND WAS DIAGNOSED AS HAVING A FRACTURE TO 1 RIB. THIS OCCURRED ON THE 002-0 SECTION, 2 SOUTH PANEL AT THE #24 CROSSCUT.

HE STATED THAT HE WAS HELPING WATCH THE MRS CABLES ON THE SECTION WHEN A RIB ROLL OCCURRED AND STRUCK HIS LEFT FOOT. IT RESULTED INTO A FRACTURETO THE LITTLE TOE AND BRUISING HIS LEFT FOOT. IT OCCURRED ON THE 002-0 SECTION

A ROOF FALL OCCURRED IN THE #4 ENTRY AT #50 XCUT OF THE 005-0 SECTION IN THE 4 WEST PANEL. FALL WAS APPROX. 20'X 20'X 8'HIGH. FALL OCCURRED IN THE INTERSECTION AT THE SECTION BELT FEEDER, CAT CHING THE INBY DUMPING POINT OF THE FEEDER. THE AREA HAD BEEN PREVIOUSLY BOLTED WITH 5'HI-TECH BOLTS, SPOTTED WITH 8'& 12' SUPER BOLTS, WITH RIB BOLTS AND STRAPS.

A ROOF FALL WAS DISCOVERED DURING WEEKLY EXAMS. THE FALL IS APPROXIMATELY 50'LONG; 8'THICK AND 18'WIDE. IT IS LOCATED ON THE 3 SOUTH PANEL AT THE 9TH CROSSCUT IN THE #7 ENTRY. THE AREA WAS ORI GINALLY BOLTED WITH 5' HI-TECH BOLTS.

DURING THE PROCESS OF CHANGING OUT A RAM MOTOR ON A CONTINUOUS MINER, HE RECEIVED A TEAR TO A TENDON IN HIS LEFT SHOULDER WHEN LOOSENING AND TIGHTENING UP BOLTS WITH A RATCHET. HE CONTINUED TO WORK UNTIL HE HAD SURGERY ON NOV 18, 2003 TO REPAIR HIS SHOULDER.

HE RAISED UP FROM A BENT OVER POSITION AND CAME IN CONTACT WITH A ROOF BOLT THAT HAD BEEN USED AS A RIB BOLT TO SECURE THE RIB. IN PICKING UP ANOTHER ENTRY AT THIS LOCATION, THEY HAD CUT OUT THE ROOF BOLT BUT HAD THE END STILL STICKING OUT OF THE MINE ROOF, EXPOSING A SHARP EDGE. IT RESULTED IN SUTURES ADDED TO HIS RIGHT EYE.

ROCK FALL WAS DISCOVERED INT HE 3 WEST RETURN, BETWEEN #5 AND #6 ENTRY AT THE #40 CROSSCUT. FALL WAS APPROX. 6' TO 7' THICK, 50' LONG AND 18 FT WIDE. FALL WAS DISCOVERED ON THE WEEKLY EXAM. NO PLANS ARE MADE TO CLEAN UP THE FALL. THE AREA ORIGINALLY WAS BOLTED WITH 4' TORQUE TENSION BOLTS.

ROOF FALL WAS DISCOVERED IN THE #5 ENTRY OF THE 4 LEFT PANEL, INBY SPAD #18249; BETWEEN THE #16 AND #17 CROSSCUTS. FALL IS APPROXIMATELY 70' LONG; 20' WIDE AND APPROXIMATELY 7' THICK. THE AREA IS BREAKERED AND DANGERED OFF AND WE DO NOT PLAN ON CLEANING UP THE FALL. THE FALL AREA WAS ORIGINALLY BOLTED WITH 5' HI-TECH BOLTS.

A PILLAR ROOF FALL OCCURRED AT APPROXIMATELY 4:00 PM IN THE #3 ENTRY OF THE 1 SOUTH PANEL. THE PILLAR FELL INCLUDED APPROXIMATELY 3 ROWS OF 5 FEET LONG HI-TECH BOLTS. THE AREA AFFECTED BEGAN A PPROXIMATELY 10 FEET LONG, 5 TO 7 FEET THICK.

INJURED WAS ASSISTING IN PULLING ON A CONTINUOUSMINER WITH A RETRIEVER WHEN THE CLEVIS BROKE ON THE MINER AND A PIECE OF METAL HIT HIM ON THE UPPER PART OF HIS LEFT ARM. IT RESULTED IN A LACER ATION REQUIRING APPROXIMATELY 9 STITCHES AND HAIR LINE FRACTURE TO HUMERUS.

A ROOF FALL WAS DISCOVERED AT APPROXIMATELY 2:10 PM IN THE 3 NORTH PANEL, 1 CROSSCUT INBY SPAD #17913. FALL WAS APPROXIMATELY 60'LONG; 5 TO 7 FEET THICK AND 20 FEET WIDE. THE FALL AREA HAD BEE N SUPPORTED WITH 5' HI-TECH BOLTS. THE FALL WILL NOT BE CLEANED UP. AREA WILL BE CRIBBED OFF AND DANGERED OFF.

INJURED STATED THAT AS HE WAS HANGING A CURTAIN IN THE ENTRY, A PIECE OF DRAW ROCK FELL BETWEEN THE RIB AND ROOF BOLT AND STRUCK HIM ON THE TOP OF HIS RIGHT FOOT. IT RESULTED IN A FRACTURE TO A BONE ON THE TOP OF HIS FOOT.

EE STATED THAT HE WAS OPERATING DIESEL SCOOP ALONG 3 NORTH ROADWAY AND RAN OVER A ROCK OR A HOLEIN ROADWAY, CAUSING HIM TO BOUNCE AROUND IN DECK. HE STARTED MISSING WORK ON 4-28-03. HE WAS DIA GNOSED AS DISC DISEASE. WE QUESTION WHETHER AN INJURY OCCURRED.

HE STATED THAT HE WAS ATTEMPTING TO PLACE A CRIB BLOCK UNDER A SECTION OF LO-LO BELT STRUCTURE. WHEN HE GOT HIS FINGER MASHED BETWEEN THE CRIB BLOCK AND THE FORKDS OF LO-TRAC LOCOMOTIVE. IT CA USED A LACERATION REQUIRING 1 STITCH.

A ROOF FALL WAS DISCOVERED AT APPROX. 1 30 A.M. IN THE #4 ENTRY OF THE 3 NORTH PANEL BETWEEN #74 AND #75 CROSSCUTS. THE FALL WAS APPROX. 6' TO 15' HIGH, 90' LONG AND 18' WIDE. FALL WAS AT SPAD #19007. THE AREA WAS ORIGINALLY BOLTED WITH 5' HI-TECH BOLTS. THE FALL AREA HAS BEEN CRIBBED OFF.

EMPLOYEE WAS SHOVELING BELT LINE WHEN A ROCK FELL FROM THE ROOF AND HIT EMPLOYEE ON TOP OF LEFT HAND.

A ROOF FALL WAS DISCOVERED AT APPROXIMATELY 9:30A.M. IN THE 3 LEFT PANEL OFF OF 3 NORTH INBY SPAD #16714 IN THE #2 ENTRY. THE FALL WAS ~6' THICK X 20' WIDE X 30' LONG. AREA NOW CRIBBED OFF.

A ROOF FALL WAS DISCOVERED AT 10 AM ON THE 4 LEFT PANEL AT BREAK #35 IN THE #5 ENTRY. THE FALL IS LOCATED 2 CROSSCUTS INBY SPAD #18592. IT WAS APPROX. 18'WIDE, 55'LONG, AND 7'THICK. THE FALL HAS BEEN CRIBBED OFF AND WILL NOT BE CLEANED UP.

A ROOF FALL WAS DISCOVERED ON THE 3 NORTH PANEL AT THE NO. 54 CROSSCUT. THE FALL WAS APPROXIMATELY 3 TO 7 FEET THICK BY 18 FEET WIDE. IT EXTENDED FROM THE RIGHT EDGE OF THE CROSSCUT IN #6 ENTR Y THROUGH THE INTERSECTION IN #7 ENTRY. THE FALL HAS BEEN CRIBBED OFF AND WILL NOT BE CLEANED UP.

HE WAS PUSHING A ROCK OFF OF THE TOP OF THE CANOPY OF THE ROOF BOLTING MACHINE WHEN HE GOT HIS RIGHT MIDDLE FINGER CAUGHT BETWEEN THE ROCK AND MINE ROOF.

WHILE USING A CHAIN HOOKED FROM A BRIDGE CARRIERTO MOVE THE LO-LO BELT STRUCTURE, THE CHAIN BROKE AND STRUCK HIM ACROSS THE TOP OF HIS NOSE, A LACERATION, REQUIRING 5 STITCHES.

A ROCK FALL WAS FOUND IN #7 ENTRY OF THE 3 WEST PANEL,INBY SPAD #16220. THE FALL IS APPROX. 5 FEET THICK; 20 FEET WIDE AND 3O FEET LONG.

WHILE LOWERING THE HEAD OF THE ROOF BOLTING MACHINE, HE REACHED FOR A PIECE OF DRILL STEEL. THE DRILL HEAD HIT AGAINST HIS LEFT THIGH AND KNEE, BRUISING IT. HE STARTED MISSING WORK ON 12/6/02.

WHILE DRIVING THE FILLY PERSONNEL CARRIER DOWNT HE SLOPE ENTRANCE, HE LOST POWER ONT HE RIDE. THE FILLY HIT THE RIB, CAUSING HIM TO HIT HIS KNEE AGAINST THE FRAME, RESULTING INTO A FRACTURED K NEE CAP.

WHILE PLUGGING UP THE CATHEAD ON A SCOOP BATTERY, IT CAUSED AN ARC FLASH THAT RESULTED INTO A MINOR FLASH BURN TO HIS RIGHT EYE.

HE WAS CARRYING A JACK HAMMER TO THE TAILPIECE OF THE BELT. WHEN HE STARTED TO LAY IT DOWN HIS LITTLE FINGER ON HIS LEFT HAND GOT CAUGHT BETWEEN THE BARREL AND THE JACK, CAUSING A LACERAT ION.

HE WAS LIFTING A MINER CABLE AND FELT PAIN IN HIS BACK AND HIP. HE CONTINUED TO WORK AND BEGAN MISSING ON 11-1-02.

HE HAD DRILLED A HOLE IN THE ROOF FOR A BOLT AND THE DRILL STEEL HAD HUNG IN THE HOLE. HE PLACED HIS RIGHT HAND ON THE DRILL HEAD AND THE STEEL FELL FROM THE HOLE AND STRUCK HIS LEFT HAND.

HE WAS DRILLING A HOLE IN THE ROOF FOR A BOLT WHEN A PIECE OF ROCK FELL & STRUCK HIS LEFT HAND.

A ROOF FALL WAS FOUND DURING THE WEEKLY EXAM ON 3 WEST MAINS IN THE NO. 2 ENTRY AT CROSSCUT #11. THIS AREA HAD BEEN BOLTED WITH 5' HIGH TECH BOLTS. FALL APPROX. 18' X 18' X 5' HIGH.

A ROOF FALL OCCURED IN THE #1 ENTRY ONT HE 5 LEFT PANEL 1 CROSSCUT INBY SPAD #17240. THE FALL WAS APPROX. 20'X25'X6' HIGH. THIS AREA HAD BEEN BOLTED WITH 5' HIGH TECH BOLTS.
