While workers were cleaning up material that was removed from a new coal transfer borehole using a continuous miner, a section of rib/brow fell from the inby portion of the entry and continued outby pulling rib bolts. The material rolled down the rib and struck the miner operator causing fatal crushing injuries to chest and abdomen.

While conducting retreat mining on 6-Left Panel off B-4 Mains, a coal/rock outburst occurred inby break #35. The outburst involved the continuous miner operator who suffered fatal blunt force injuries to his head, chest, and legs.

Employee was found pinned between the #4 mobile bridge unit and the section lo-lo conveyor belt structure. He was apparently troubleshooting a report of weak tramming on the left side of the mobile unit. There were no witnesses to the accident.

A non injury accident (roof fall) occurred in the #2 entry from break 20 to 22 and through the break at 22 to entry #3. The fall occurred 5ft high up to 8ft high. We are cleaning the fall up and rehabbing the #2 entry. We are cleaning it up and using 6ft resin rods as well as 12ft rope bolts every other roll.

The area was previously supported with 5' Hy-Tech bolts and 12' super bolts. The fall did not affect persons or ventilation. The fall was cleaned up and the area re-supported.

Employee was moving a sump pump. The pump line was tied off at the pump and lo-trac forks. While the operator was pulling the waterline with the lo-trac, the waterline got caught on the feeder causing the rope hangers to break allowing the pump to strike the operator on the left side of the face. He suffered fractures to the left side of his face.

When the MBMs were moved, his thumb was caught between the collar and the hinge pin, resulting in the amputation of the tip of his left thumb.

EE has made a claim under our worker's comp alleging that he injured his back while lifting a 50 lb bag of block bond on 2/24/07. He had not made any mention of injury that day to his supervisor or to his co-workers. It was 12 to 14 days before we learned he was seeking treatment. I then had to wait to hear from him to get details as he had had his phone number changed.

IN PICKING UP ANOTHER ENTRY AT THIS LOCATION, THEY HAD CUT OUT THE ROOF BOLT BUT HAD THE END STILL STICKING OUT OF THE MINE ROOF, EXPOSING A SHARP EDGE. IT RESULTED IN SUTURES ADDED TO HIS RIGHT EYE.
