A continuous miner operator was cutting coal out of the #7 heading while positioned on the lefthand inby corner of the #7 entry. One piece of the corner that consisted of coal and rock sloughed off striking EE on the left shoulder and back area. The employee was placed off of work beginning 7/21/25.

Employee was bolting in the #6R cut. Removing the steel from the outside bolt on the fifth row when a rock measuring 29.5'' X 28'' L X 3'' Thick fell striking the employee's right pinky finger. The entry was 19.5' wide x 73'' high and damp. Fracture to right Pinky requiring surgery.

The Employee Knelt down to change the water filter on the RCM SN JM7225 in the #5 entry on 2 section. Employee had left hand on top of the miner when a rock measuring approximately 12" by 9" by 7" fell striking the employee's left pinky finger. The area was approximately 6 feet high. This incident resulted in a partial amputation.

The employee was on the #3 section in the #4 face Bolting on the #3 RB. The employee had finished drilling in the first row of the second cut. The employee placed left hand on the ground behind self to get up when a rock measuring.36''L X 7''-15'' w X 2.5'' thick struck employee on the left hand. Area was approx 70'' high and damp. Swelling /1cm cut to the left hand. Under protest

The Employee was bolting in the #5 face on the #2 section when a rock measuring 10"x6"x1/4" fell from the top striking the employee in EE's hard hat causing a laceration to the left side of EE's neck and ear. The entry was approximately 74" high and damp. The employee received 5 sutures.

An owl shift utility worker was struck in the left arm by a piece of draw rock as EE attempted to drill a hole for bolt installation in the #3L crosscut. This resulted in three cuts to the left forearm that required 16 stitches.

The Employee was observing the bolt crew drill a test hole when a piece of rock approximately 3' L by 4' W By 1" to 6" thick fell striking EE on the hard hat, causing a 1 inch laceration on the back of EE's head. the entry was 18'6" wide by 62" high and damp to wet. The employee received 3 staples.

The employee was bolting in the #6R entry on the # 2 Section. When a piece of rock approximately 18"long x 8"wide x 2"thick fell from a mine roof which had a height of approximately 6'6" striking EE's right arm causing a laceration approximately 2 inches. The employee received 7 sutures.

The employee was located at the tram controls of the #7 DDR Roof bolter S/N 2016-451 when a piece of rock measuring 27" long by 18" wide by 5" thick struck EE on the back of the hard hat and the upper back. The area was approximately 7'10" high and damp.

The Employee was in #9 entry on the # 3 section roof bolting. While trying to maneuver the drill steel into the hole in the roof mat a piece of rock fell striking EE's rights hand. The area was approximately 6.5 feet high and damp.

The employee was on the #2 section in the #5 entry on the off side of the roof bolter. The Employee was attempting to install a glue bolt when a rock 15.5" long X 4" wide fell striking EE on the right arm just above the elbow. The area was approximately 7' high and damp. The laceration was approx 2 inches.

Employee was bolting second cut out of #5 entry, first row outside bolt. Piece of rock 3'L x 2W'x 5 1/2"T fell and hit canopy, then hit the employee on the outside of left ankle. Causing puncture to boot and ankle. Small cut and swelling to Left ankle area.

The employee walked from #5 entry into an unbolted cross cut between #4 and #5 entry. When EE was struck by a piece of rock. The entry was approximal 60" high and damp. The employee sustained crushing injuries.

Roof bolter operator was in the #2 entry putting up the inside roof bolt when a piece of roof rock fell striking the employee in the back between the shoulder blades. Employee is currently off of work.

An offside roof bolter operator was injured while attempting to remotely extend EE's ATRS Pad out. While positioned to extend EE's ATRS pad out manually, a piece of draw rock fell on the operator side ATRS causing the offside of the ATRS to raise and strike the offside employee in the face area. This resulted in a concussion and damage to teeth.

While attaching a pull rope to a scoop bucket, the employee was struck on the right hand by a loose section of mine roof.

The employee was struck on the left leg by a section of mine roof while they were cleaning out the operator's deck of a shuttle car.

While miner was in #7 face, the miner operator reached to get wash down hose to spray off miner. Was struck in right hand with a roof rock, causing laceration to top of hand that required stitches.

Employee checked the top before bolting and noticed a loose rock and tried to pull it. When the rock would not come down, EE drilled the hole for the bolt and when EE inserted the bolt into the hole and went to push the bolt up into the roof, the rock fell and hit EE's left pinky finger causing a laceration.

The offside roof bolter operator was in the process of installing a 2nd roof bolt in the 2nd row when they noticed a piece of draw rock moving out of the corner of their eye. A piece of draw rock measuring approximately 45" by 25" by 1/2" fell striking them just below the left elbow resulting in a cut that required stitches.

A piece of draw rock fell on a roof bolter operator as they were walking back to drop the ATRS to move the bolter up. One row of bolts had been installed.

A piece of draw rock fell from between the last row of bolts and the 2nd from last row of bolts in the #9 heading. The rock struck the employee on the left leg resulting in a broken leg.

Employee was roof bolting when a piece of rock fell from the mine roof striking the employee in the left foot ankle bone area. The employee was taken to hospital where ee received seven sutures.

Employee was building a stopping when a piece of draw rock fell from the mine roof and struck the employee in the nose causing a laceration. Employee received 3 sutures.

The employee was scooping in the #1 heading. The employee placed left hand on top of the scoop panel while operating the scoop with right hand. A piece of draw rock fell on left hand causing lacerations to third, fourth, and fifth fingers.

A piece of draw rock fell on the continuous miner operator while in the #4 heading on the active section.

Employee was roof bolting in the #4 face when a piece of rock fell from the mine roof knocking off hard hat. As the employee turned to get hat a second piece fell from the mine roof striking the employee in the top of the head causing a laceration. The employee went to Raleigh General Hospital and received 3 staples. Employee was off work for 1 day.

Employee just finished mining in the #3 entry and turned to walk outby and a piece of 8 1/2' x 28" x 14" fell between the right side bolts and rib striking employee on the back of head, right shoulder, and right heel.

Employee had just finished scaling top and had returned to the drill station to start bolting, as EE reached for a bolt a piece of rock 14"x19"x1 1/2" fell and hit the top of EE's right hand. EE was taken to hospital, x-rays were negative. EE had to wait to be seen by orthopedic and the orthopedic has EE off for 2 weeks for physical therapy.

Employee was mining in #7 entry when a piece of draw rock fell and hit ee on the head. Employees left knee had soreness so the doctor had ee off for 2 days. EE returned to regular job 2 days later.

The offside roof bolter operator on 3 section in #2 entry was struck in the back of the right leg by a piece of rock while roof bolting. The operator was transported to Raleigh General Hospital where x-rays showed a broken fibula, broken pinky toe, and cracked ankle. Operators return to work is unknown at this time. Operator worked for Lightning Contracting Services MSHA ID-8CV.

The employee was scaling the mine roof. A piece of rock fell on employee's head and back, driving them to the ground.

Employee was roof bolting when a rock fell striking the employee in the left arm. Employee had a cut approximately 2 inches in length. Employee then went to Med Express where they received 4 sutures.

Roof Bolter Operator was in the #6 entry bolting top and as the employee was putting up their last row of roof bolts a piece of rock fell out striking the employee in the nose area resulting in a fractured septum.

On the #1 section, roof bolter was in # 7 entry. A piece of rock fell from 10 feet high. The rock was 3' long by 2' wide tapering to 4-5 inches thick. Rock fell at the front edge of ATRS. Employee turned to get away and the tapered end of rock struck employee's left heel.

Injured was bolting roof on offside of machine, while ee was drilling a piece of rock 2 1/2" thick x 30" long x 20" wide fell sriking ee on head and shoulder.

While drilling a hole to install a roof bolt a piece of rock fell from the mine roof striking the drill pot and breaking the rock into pieces. One of the pieces of rock flew under the employee's safety glasses striking the employee in the left eye. Employee received eye drops for a scratched eye.

Rock fell from top catching employees right index finger between the canopy of roof bolter and the rock.

EE stated that ee was putting on outside bolt when draw rock fell from top hitting ee in right side arm (mid bicep to mid forearm) & shoulder. Report from mine stated draw rock was approximately 24' long, 8'-10' wide & 3'-4' thick.

Employee was installing first row of 5 foot roof bolts in the #8 face / heading. Employees reached into the air to tart steel and a piece of draw rock fell and smashed EE's hand on roof bolter. Resulted in a broken left pinky finger. Employee was wearing the required PPE.

Employee was drilling a bolt hole in #6 entry when a piece of draw rock fell around the drill steel and struck the employee on the head, causing a compression type injury to neck and back.

Employee was changing a belt stand out on #2 belt at 19 block when a piece of draw rock fell. The piece of rock struck the right side of his hard hat and right cheek this caused a laceration that required sutures. The employee was treated and released to full duty NO days were missed.

Employee was traveling outby between a parked continous miner and the rib when a brow and rib fell striking him on the right shoulder, causing a slight separation of the shoulder.

Employee was roof bolting in the #2 to #3 crosscut on the #2 Section when a piece of roof rock fell striking him in the back of the head creating a laceration that took 2 sutures to close.

Rock bounced off slope hitting employee in his right hand which caused a laceration that required 11 stitches. Employee was treated and released and returned to work on his regular scheduled shift. NO DAYS MISSED

Employee was attempting to remove a tracking cable from under a piece of draw rock that had fallen on the cable. Another piece of draw rock fell and hit his right hand resulting in a laceration to his middle finger requiring stitches.

Employee was scaling the top when a rock fell and hit him in the left leg. X-rays taken at St. Joseph Hospital were negative and the employee returned to work on his next scheduled shift. The employee continued having pain and was seen by an orthopedist, which recommended surgery on September 25, 2014. *The Employee was operated on September 25, 2014 and will be off work.

Employee was operating scoop when a piece of draw rock approximately 16" in diameter & 1/2" thick struck him on the side of his head causing his head to strike the frame of the scoop deck.

Employee was putting bolt in hole when a piece of rock fell striking his left middle finger, cutting the end of the finger and nail. He was wearing metacarpal gloves at the time.

Employee was bolting a brow in the #7 entry. The starter steel was in the brow about 2" when the brow fell out and caught the employees left hand between it and the drill head, lacerating his thumb.
