Employee had just finished servicing a location and was moving to the next location. In between downtime employee went to drink and have a snack. During this time a piece of 1-inch-thick dry rock fell and hit the employee in the left wrist causing a laceration.

Employee was attempting to repair a broken cat track on the miner (CM132) in the #4 Entry on #6 Section, when a piece of rock fell out from the top of the rib and struck EE's right ankle causing a fracture to the medial malleolus bone in EE's right ankle.

The crew was in the process of forming up walls to pump concrete around the steel sets. Employee was evaluating the roof conditions around the steel set area and unexpectedly, a rock fell from the top area striking employee and causing injury.

Employee was attempting to install the first row of bolts when they noticed a piece of draw rock looked loose, they turned to get the rock bar when a piece of rock fell out of the top striking left heel causing laceration to heel requiring sutures and partial laceration of the Achilles Tendon.

Employee was hanging water line, as EE reached out EE missed the hook and caught self with EE's right hand on the rib, when a piece of rock above EE's right hand came out striking EE's hand resulting in abrasions and contusions.

Employee was preparing to service the #1 face, as EE removed a timber that was leaning against the rib, a piece of rock fell out striking employees right foot causing contusion to EE's foot.

Employee was setting bits on longwall drum when a rock struck EE's hand against the drum causing a fracture to EE's left ring finger and sutures to both the left ring finger and left little finger.

Employee was riding in mantrip. A chunk of coal fell into the mantrip and caused injury to employee's finger.

REPORTABLE ONLY DUE TO SUTURES - Employee was in position to pull tailpiece forward on the Longwall, when a piece of rock fell hitting employee's wrist causing laceration requiring sutures.

REPORTABLE ONLY DUE TO STITCHES - Employee was riding in man bus traveling into the track turn out when a rock fell striking employee's elbow causing laceration requiring sutures.

Employee was replacing bits on the headgate shear drum located in the No. 4 entry when a rock fell out of the top, from between bolts, hitting employee on back and legs causing fracture to left lower leg.
