Employee was loading a SC in the face when a brow rolled off and hit him in the right shoulder. Due to complications he had an operation on his shoulder on May 15, 2009 and is missing work.
Roof bolter was installing a roof bolt on the rib side and as he put two pieces of steel together a piece of draw rock fell from the mine roof and struck his right middle finger lacerating it. The laceration required (4) sutures. This is to report medical treatment only.
Was bolting in #3 entry and while drilling, 8" rock blew out and struck his knee
On 6-4-08, employee had a piece of rock fall from the top and hit him on the right foot as he was bolting top and continued to work. His foot swelled up and he started missing work on 6-10-08 and returned 6-17-08.
Roof bolter operator was bolting in #2 entry on the off side of the machine, when a piece of draw rock 5.5"L X 18"W X 3.5" thick fell from the top and struck the drill head and then struck him on the right forearm. He required (9) sutures. This is to report medical treatment only.
According to the employee he drilled a hole and was in the process of trying to get the pinner steel out of the hole when the drill pot fell on his knees and thighs. He was in a kneeling position. The cause of the fall is unexplained. When checked the machine operated properly. Due to complications he started misssing work on 4-4-08.
Operator installed outside roof bolt and began to swing head in. A piece of rock 18"L X 8" W X 2" thick fell and struck his right hand which he had placed near the drill pot. He sustained a simple fracture to his ring and little finger of his right hand. Medical treatment was to splint (tape them together). This is to report medical treatment only.
Employee was bolting top, when a piece of coal with rock fell from the rib/roof and hit him in the side/back, pushing him into the roof bolter controls.
As the roof bolter operator lowered his canopy to swing over and set another bolt cap coal measuring approx. 5" wide x7" thick X 22" long fell and struck him in the lower back.
A piece of draw rock fell and hit the employee in the upper back and neck area as he was operating the bolt machine.
Injured stated that he was removing draw rock (from roof) with hammer when "wrong" rock fell out. Operating roof bolter operator's side waiting for other guy to get finished. EE states that he bent over and the rock fell and hit back of head causing contusions.
EE stated that while operatig a roof bolting machine, a section of rock spun out of the hole he was drilling striking him on right upper forearm. Required stitches to close laceration.
Employee left the mantrip and was walking toward the face when a piece of rock (3'X 3'X 6") fell from the roof and hit him in the back. (Misc Mail 7/16/07 #38-add narrative-Due to complication started missing work on 7/9/07)
While roofbolting, a piece of roof rock hung up on the drill steel and as the steel continued to rotate, the rock broke off striking the injured on the left shoulder and head. This resulted in contusions to the head and shoulder.
While roof bolting in on the 1 Left section 2 left crosscut, the injured had installed the outside bolt and was swinging the drill head in to install the inside bolt and had his right hand resting on the drill head when a piece of shale rock fell stiking his rt. hand and index finger.
After moving the bolter up in a cut, the operator reached out past the canopy and laid his right hand near the head to help himself get in postion to bolt. A rock fell out and struck the top of his right hand lacerating his middle finger. He received 5 sutures. He was not wearing metacarpal gloves. This is to report medical treatment only. The injured was roof bolting with a twin head Fletcher, operating left side and was in the process of swinging the head of the roof bolter toward the left rib to install his first bolt in that row, when a piece of head coal approx. 4" thick x 30" wide x 24" long fell from between the bolted area hitting him on his left shoulder.
The individual was operating a twin head roof bolter on the 002-0 section, 2rt crosscut. he had finished installing the 2nd EE inside bolt and was dropping the safety canopy down when a piece of rock approx. 12" long x 6" wide x 0-1" thick bell off of the canopy striking his rt index finger which was resting on the drill head.
STARTED DRILLING A PIECE OF DRAW ROCK FEEL AND HIT RIGHT MIDDLE FINGER. CUTTING THE FINGER HAD TO GET 3 STAPLES.
