Employee had pulled walk thru roof bolter in #6 entry to bolt and set the T-bar on the roof bolter. The employee had grabbed a couple of bolts to lay in the bolt tray. EE placed the bolts down in the tray when a piece of undetected draw rock fell striking EE on the right ring finger resulting in a fracture to the end of the finger and required stitches.

Employee states while bolting top, a piece of draw rock fell out striking him on the left forearm. The starter drill steel stayed in the top and he was getting ready to add drive steel when the starter steel fell out. While reaching to catch the steel, draw rock fell from top striking him resulting in 13 stitches.

Employee was scaling loose rock after setting the ATRS against the top when a piece of loose material dislodged from the roof behind him. The material pushed his hard hat down hitting his safety glasses causing a small cut on the bridge of his nose and caused him to bite his lip which required 3 dissolvable stitches on the inside. The cut on his nose was glued.

Bates Contractor was drilling hole for rib bolt. While stopping to install his wrench to continue drilling the hole, he reached over the drill pot and material from the brow sloughed out hitting his nose resulting in a laceration requiring 14 stitches.

Employee was drilling last bolt hole in the cut when an undetected slick sided rock slipped out hitting the top of the canopy breaking with one piece of the rock striking him on the left side of his back resulting in a contusion and 1.5" long laceration with no stitches required.

Employee states while drilling a hole, draw rock fell from the top striking him on the left side of his face and left eye area.

Bates Contractor disengaged the T-bar from the mine roof, when a piece of draw rock dislodged from between the rib and the outside bolt striking him on the left ankle. The contractor (5UF) submitted a 7000-1 on 10/27/14, however it should have been submitted under this MSHA ID.

Employee started to drill hole when a rock broke and fell striking employee's right hand (middle and ring finger) cutting them.

Employee swung off side drill station out to put up first bolt. A rock fell and struck him on the right leg from the knee down resulting in a bruise.

Employee states that when he started his steel to drill a hole a rock fell from the top striking his right foot causing a laceration to his big toe. This resulted in 7 stitches.

Employee had began installing the inside bolt when a rock fell from the roof striking him on the back of the left leg resulting in a simple fracture.

Bates Contractor was bolting last row in the #4 entry inside bolts when a piece of draw rock fell from the face area and broke on T-bar striking him on the upper right arm resulting in 11 stitches.

While bolting the drill operator completed the second row of bolts and was advancing the drill to the 3rd row. The ATRS was set and when he placed his left hand on top of the leveling bar of the ATRS for balance to exit the drill a piece of rock fell from the roof striking his left hand resulting in a fracture.

Bates Contractor put up the last row of bolts in the #4 entry when a piece of draw rock fell and broke on T-bar striking him on the left hand. His hand was on the drill wrench when the rock struck him. Resulted in fracture and 3 stitches to his left pinky finger.

Employee was installing outside bolt on first row of support when a rock fell around drill steel to the drill pod. While removing the rock from the drill steel a small piece of rock that had been on the canopy fell off striking his left index and middle fingers. This resulted in a fracture to the end of his index finger and 4 stitches.

Employee was installing rib bolt on walk thru drill when a piece of draw rock fell hitting canopy then striking him on the left shoulder resulting in a cut that required 6 stitches.

Employee was starting to drill first hole. A piece of draw rock fell striking him on the right shoulder then striking his right knee resulting in pain and swelling to his knee.

Employee was bolting top while starting his starter steel when a piece of coal from the roof fell hitting the drill head. It broke and a small piece hit him in the mouth causing a cut to his lip resulting in 4 stitches.

They had installed 2 rows of bolts and the employee was letting another employee put up a cable bolt. He was kneeled down when an undetected piece of draw rock fell striking him on the right wrist causing a laceration resulting in 10 stitches.

Employee started to remove steel from hole when a piece of draw rock fell from the top striking him on the top of the right foot.

Employee was installing the 3rd bolt on the 3rd row of bolts when a 2' long x 2' wide x 0 to 4" thick rock fell striking him on the hard hat and sliding down to his left shoulder.

Employee was attempting to align steel to start drilling a hole. A piece of draw rock kicked out knocking hand from steel and mashing it against drill pot.

A rock measuring 3'x 2'x 0" to 5" thick unknowingly sat down on the roof bolter canopy. While he was swinging the drill head over to install the 2nd bolt, he saw the rock coming off the top of the canopy. While moving out of the way of the falling rock, he hit his head on the mine roof causing pain to his neck and shoulders, he began losing time away from work on 05/17/2011.

Employee was installing pogo sticks behind the line curtain when a piece of draw rock fell striking him on the base of the neck and shoulders.

Employee was starting to drill with steel in the mine roof when an undetectable piece of draw rock fell measuring 12X8 thick fell vertically with the end striking his right foot behind steel toe. The area employee was bolting in was 6 high. Injury resulted in fracture to the 4th metatarsal on his right foot.

Employee was standing on drill platform when coal slid down the T-Bar hitting him on the Right leg. Employee continued to work until 12/08/2010.

Roof bolt operator was installing rope bolts into the mine roof. When operator was in the process of changing steel out of drill head a rock 42 long X 17 wide X 3 thick fell hitting him on the hand fracturing left thumb.

Employee was putting up the inside bolt on the last row. He was about 3' deep in drilling the hole when draw rock fell out striking him. It was 6" thick by 12" long requiring 3 stitches to his upper lip.

Employee bent over to pick up a roof bolt to install when a piece of draw rock, measuring 4' X 19" X 4.5", fell from the top and struck him in the back.

Employee was leveling the bottom to start the first row of block, when a piece of coal measuring 4' X 2' X 3" struck him on the left forearm, causing a fracture.

Employee was climbing out of operator's compartment. He laid his hand on top of canopy, and his gloved hand was struck by a piece of rock.

Employee stated that he finished cleaning the area and reached in scoop to get a bag of rock dust when a rock, measuring 12" X 18" X up to 2", fell from top striking his left hand. He was initially released to regular duties but after a follow up was taken off duty. He began losing time 8-12-09.

Employee was pushing up the 2nd bolt, when a piece of rock 6" x 4" fell and struck him on the left arm, causing a laceration that required sutures.

A piece of draw rock fell striking his hand while it was on top of the drill canopy casuing a cut requiring sutures.

As the injured employee was swinging out the drill head to install a roof bolt, a piece of drawrock slid down the drill canopy. The rock struck the employee on his lower right leg resulting in a non-displaced fracture to his right distal fibula.

The employee alleges that as he was cleaning a piece of rock off the drill canopy another piece of draw rock fell from the roof striking him on top of his left hand. The piece of rock caused a small laceration that required three stitches to close the wound.

Employee was operating roof bolter with his right hand laying on the bottom when a piece of draw rock fell striking him on the middle finger of the right hand.

Inj EE was struck on hard hat by draw rock that slid down his back onto his left lower leg resulting in fractures to 4th & 5th metatarsal bone on left foot.

EE was drilling hole to install roof bolt & an undetected piece of draw rock fell, striking the drill canopy & bounced over striking EE on left cheek causing a laceration requiring stitches.

The EE alleges that he had an accident 6/8/07, but did not report incident until 7/2/07. Alleges small piece of drawrock fell, striking him on right wrist as he was starting to drill hole for roof bolt. Small piece of rock impaled wrist & was surgically removed 7/19/07 at which time injury became reportable.

The EE was behind the line curtain getting ready to advance the curtain, when a piece of drawrock 30" X 28" X 2.5" thick fell, striking the EE on his left lateral lower leg, resulting in an abrasion and contusions to the leg. The EE continued to work until 5/22/07.

The employee was struck on his left forearm by a small piece of drawrock resulting in a small laceration that required 3 sutures to close.

As the employee was operating the roof drill, a small sharp piece of draw slate (3"x3") fell striking him on the upper lip resulting in a small laceration requiring sutures to close.

As employee was operating a Lo-trac, a piece of undetected draw rock fell striking him on the right side of his head, causing a possible concussion.

Employee was installing a roof jack when loose material fell striking him on his lower back, resulting in bruising and pain.

EE was operating roof bolter when a rock fell from overhead, striking him on the right forearm, causing a laceration that required stitches.

Employee was installing a resin bolt, when an undected piece of draw rock fell striking his left foot causing a fracture.

Employee was hanging curtain when a piece of draw rock fell from the top hitting his little finger of his left hand causing a fracture.

EMPLOYEE WAS OPERATING A ROOF DRILL. AS HE APPLIED PRESSURE TO THE STARTER STEEL, A PIECE OF ROCK DROPPED OFF AND STRUCK HIM ON THE RIGHT SIDE OF HIS NOSE, CAUSING A LACERATION THAT REQUIRED STITCHES.
