Deep Mine #35: 546 citaciones de la MSHA Coal

Received a CWP claim with diagnosis on 4/9/2014. Employee last worked on 3/18/2013 and is no longer employed with the company. Accident date of 3/18/2013 is to accommodate online reporting per MSHA (Denver) instructions.

A roof fall was discovered at 10:45 am in the #5 entry primary escape way. The fall measured 20' x 20' x 8' thick. No equipment was involved and the area was previously mined on 10/16/06.

Received a CWP diagnosis for employee on 12/19/2012.

Employee was bolting top while starting his starter steel when a piece of coal from the roof fell hitting the drill head. It broke and a small piece hit him in the mouth causing a cut to his lip resulting in 4 stitches.

Employee was operating a shuttle car in the #5 entry and felt pain in lower part of stomach and groin area. A doctor's visit on 11/27/12 showed a hernia and surgery was set.

A roof fall occurred at approximately 2:30pm at spad #2499 in the right return. There were no injuries and ventilation was unaffected. The fall measured 20' x 30' x 6' thick.

Employee was working on a water clamp and had his right hand resting on the lifting arm next to the canopy post. The other operator came over to assist and started to lift the drill head higher. This mashed his fingers and resulted in fractures of his middle, ring, and pinky fingers and stitches.

Employee was taking miner water line apart. When the line came apart it was twisted and sprung around hitting him on the elbow and forearm. He went to the doctor on 11/16/12 and an x-ray revealed a fracture on the end of the bone (spur broke completely away from the bone).

They had installed 2 rows of bolts and the employee was letting another employee put up a cable bolt. He was kneeled down when an undetected piece of draw rock fell striking him on the right wrist causing a laceration resulting in 10 stitches.

Employee started to remove steel from hole when a piece of draw rock fell from the top striking him on the top of the right foot.

Employee had been hanging curtain and was walking back to scoop when he stepped on a rock and twisted his left knee. He continued to work up until 6/23/12.

Employee slipped in the mud along the belt line and twisted his left knee. He continued to work until he had surgery on 6/22/12.

A roof fall was discovered in the #4 entry in intersection. This area was mined in 12/31/2007. No one was injured, there was no equipment involved, and no airways were blocked. The fall measured 25' long x 20' wide x 8' high.

A roof fall occurred at spad 463 between the #3 and #4 entries on the 2 Southwest Mains. This area was mined 5/21/2006. No one was injured, there was no equipment involved and there were no airways or travelways blocked. The fall measured approx. 15' long, 18' wide, and 3.5 to 4' thick.

A roof fall was found at approximately 2:20pm in the primary escape way 1 Break inby #4 tail piece measuring 40' long, 16'wide and 8' thick. This area was bolted with 4' fully grouted roof bolts and had 2 12' cable bolts in between every other row. There was no indication this area needed any additional support.

Severe thunderstorms were moving through the area of Deep Mine #35 on the evening of May 10th 2011. During on of these storms a bolt of lightening struck the mine office causing a fire at the mine office, with area wide power outages and no phone communications. The mine was evacuated and there were no injuries to any of the mine employees related to this incident.

Employee was using a slate bar for prying tool, when slate bar slipped hitting him in head. Resulting in laceration to scalp requiring 8 staples.

Employee was using a claw hammer to straighten cable hangers ,when employee hit hanger with the hammer a piece of metal chipped off of the head (1/4 x 1/8 ) penetrating in the left arm just above watch band requiring medical removal.

Employee was driving out a broke miner bit out of a bit lug with a punch when he hit his right index finger with a sledge hammer. (Requiring 2 stitches)

Employee was putting bolts into rib with truss bolter when rock fell from rib striking middle finger on right hand causing a cut to finger. (Requiring stitches)

Roof bolt operator was installing rope bolts into the mine roof. When operator was in the process of changing steel out of drill head a rock 42 long X 17 wide X 3 thick fell hitting him on the hand fracturing left thumb.

The employee has been diagnosed with pneumoconiosis (black lung) and has exercised his rights as a Part 90 miner.

Employee stated that he twisted his left knee while walking to check the # 5 Entry. He had surgery on 04/02/2010 but has continued to perform his regular duties as foreman.

Employee was putting up the inside bolt on the last row. He was about 3' deep in drilling the hole when draw rock fell out striking him. It was 6" thick by 12" long requiring 3 stitches to his upper lip.

An unintentional roof fall was discovered on 2 SW Mains in #6 entry(intake) S.S.1201, approx. 2500' off the working section. The fall measured 40'L x 8'H x 18'W. The cause of the fall appeared to be a slip with a coal rider seam and shale roof. This fall does not interfere with ventilation or with escape travel and will not be cleaned up at this time.

Employee was taking a cover off of a shuttle car with a socket and ratchet. While loosening the bolts the socket broke causing him to fall and land on his right shoulder.

An unintentional roof fall with no injuries occurred in the #7 heading at the last open crosscut. The fall measured 60' X 19' X 12'. The fall does not affect the emergency escapeways or ventilation and will not be cleaned up at this time.

An unintentional non-injury roof fall was discovered during a weekly examination. The fall was approximately 40' X 5' X 17'. This fall does not interfere with ventilation or escape travel and will not be cleaned up at this time.

Employee was pulling cable reel off of the roof bolter, when he slipped and fell, twisting his knee. Started losing time 9-9-09.

A non-injury unintentional roof fall was discovered in #2 entry of the 2nd Southwest Mains. The fall measured approximately 40' X 18' X 7' and does not interfere with ventilation and will not be cleaned up. The area has been timbered and cribbed around the parameters of the fall.

Non-intentional, non-injury roof fall occurred in the #1 entry 90' inby spad 1815 in the intersection. Fall measured 19' wide by 19' long x 7' - 8' high. Ventilation was not affected. Fall will be cleaned up and area supported by cribs.

A fall of the roof 90' X 18' X 8' fell in the # 6 entry 90 feet inby SS # 1865, 5600 feet outby the face. 4' fully grouted glue bolts were on pattern in this area with 12' cable bolts as supplemental. Cause of the fall was a coal/shale slip.

The miner was de-energized. EE was moving cables to prepare to cut a corner.He put the rope on the bolt on top of the J-hook and went outby the miner.He started the miner and the rope slipped off and he leaned in to pick the rope up and was pinned against the rib with the boom of the miner.He freed himself and walked to the front of the miner.

A roof fall occurred 10,600 feet outby the working section in the #6 entry near spad # 485. The fall was 40' long and 18 feet wide and 5 feet thick. 4 ft 3/4 glue bolts and 12 ft. cable bolts had been installed in this area. This was a transitional area from sandstone to shale.

A roof fall occurred in #7 entry of Southwest Mains 12' outby SS#683. The fall measured approx 60'l x 17'w x 2-5'thick. No equipment was involved & no injures occurred. Ventilation was not affected, fall will not be cleaned up.

A roof fall occurred in the #5 entry of Southwest Mains 75 feet outby SS #1202. The fall measured 120' long x 18' wide and 8' to 10 feet high. No injuries occurred and no equipment was involved. Ventilation was not affected. The fall will not be cleaned up.

A roof fall occurred in the # entry of soutwest mains at ss# 1128 that measured 30'l x 6-20' w x 10' h. The fall occurred on the no.4 belt line. No injuries occurred. Ventilation was not affected. The fall will not be cleaned up.

Employee was operating roof bolter with his right hand laying on the bottom when a piece of draw rock fell striking him on the middle finger of the right hand.

A roof fall occurred in the #8 entry southwest mains at SS#1121 that measured approx 18'l x 18' w x 14' h. No equipment was involved, no injury occurred, ventilation was not disturbed. The fall will not be cleaned up.

Employee stated he was lifting miner cable & felt pain in lower back area. The EE cont'd to work until 7/7/07, at which time he was taken off work by a medical specialist.

EE was drilling hole to install roof bolt & an undetected piece of draw rock fell, striking the drill canopy & bounced over striking EE on left cheek causing a laceration requiring stitches.

Employee was taking a tail roller out of a cont mining machine when his hand slipped allowing the tailroller to fall striking him on top of left foot.

Employee was using a 5/8 high strength chain hooked to his shuttle car to try to pull a continuous miner. As the shuttle car started pulling on the miner the chain broke. The chain whipped around hitting the employee on the left side of the face above the eye and right cheek causing lacerations and a fracture of the cheek.

As employee was operating a Lo-trac, a piece of undetected draw rock fell striking him on the right side of his head, causing a possible concussion.

Employee was installing a roof jack when loose material fell striking him on his lower back, resulting in bruising and pain.

EE was operating roof bolter when a rock fell from overhead, striking him on the right forearm, causing a laceration that required stitches.

Employee was installing a resin bolt, when an undected piece of draw rock fell striking his left foot causing a fracture.

Employee was hanging curtain when a piece of draw rock fell from the top hitting his little finger of his left hand causing a fracture.

Turning a right hand crosscut in the #7 entry, after loading (8) eight shuttle cars with coal an ignition occurred 71' 11" inby SS#476. No injury was reported no ventilation was disturbed and no equipment was damaged.

Employee was bending a roof bolt when he experienced a hernia of the groin that required surgery.

EMPLOYEE WAS WALKING FROM #3 ENTRY TO #2 ENTRY. AS HE TURNED THE CORNER TO GO TOWARD THE FACE, HIS HARD HAT CAME IN CONTACT WITH A BELT HANGER USED TO ANCHOR SHUTTLE CARS, CAUSING PAIN IN HIS NECK.

Employee was installing a 5' resin bolt. As he went to raise the drill boom he inadvertently hit the lever to the steel clamps that hold the dril steel, catching his right middle finger, causing a laceration that required stitches.

EE stated that he was removing a grease fitting on an Eimco Continuous Miner when the fitting broke free striking the left lens of his safety glasses. He received a laceration around his left eye requiring stitches and a scratched cornea.
