Employee was driving a mantrip from the outside to the section, when a rock fell from the top and struck in the head / shoulder area knocking his hard hat and glasses off of him. He was sent by Ambulance to SMH and was granted a full release. He informed Mine Management that due to the injury he received on 10/21/14 that he would be going off work on 3/16/15.

The employee was starting to drill the outside bolt hole on his third row of bolts, as the steel started to rotate a piece of rock 30 1/2" X 14" X 2" fell from the top and struck his right foot resulting in a fracture. The employee was wearing all proper and necessary PPE.

Employee was painting the centerline in the face when a piece of drawrock fell from between the last two rows of bolts striking him on the left shoulder, back and knee.

Cleaning on the sloop bucket and a rock fell from the top & fell on (R) knee & lower back pinned ee to ground.

He was bolting while starting to drill hole. He was about to move his left hand away from drill steel when a piece of rock fell and struck his Left thumb.

Employee was place changing the continuous miner. He was standing in an intersection, when a piece of roof rock fell from between the bolts and struck him, a glancing blow to the shoulder. He fell to the mine floor and the rock flipped onto his lower left leg.

A piece of rock fell on EE and pined him to side of bolter.

Employee was pushing a roof bolt up into the hole using the drill head and wrench when a piece of roof rock fell and struck his right thumb.

Employee was reaching for a bundle of roof bolt plates while operating a roof bolter, when rock measuring 24" x 18" x 6" thick fell from the roof between 2 previous roof supports and struck the right hand. Employee sustained fractures to the smaller finger right hand to which was later amputated to the first joint.

While drilling a roof bolt hole, a piece of rock fell from the roof striking the drill head and hitting the employee on top of the hard hat.

While bolting top, a piece of roof fell on his ankle. #3 left break, spad #341.

The employee had just finished adjusting the ventilation curtain preparation for minng. He crawled outby to the right inby corner of the intersection and was on his knees talking with another employee. A piece of roof rock measuring approx. 60"x60"x1-1/2"to 3" fell at angle from between two rows of bolts. The rock struck the employee on the back and shoulders.

Working on the #5 belt, putting in stands after all stands were put in, EE was resting hand on stand when a rock fell from top, broke EE's wrist and thumb.

Employee was drilling a roof bolt hole at the corner of a block when a piece of roof rock fell and struck him on the right thumb.

Employee was drilling a roof bolt hole when a piece of rock fell between the canopy and drill pot striking his left little finger.

Employee had taken an air reading and was walking from behind the line curtain when a piece of roof rock fell from between the bolts and pushed him to the mine floor.

Employee was under the canopy at the left drill head. He pushed the extendable canopy in with his left hand. As he did this, a piece of roof rock fell and caught his fingers between the rock and canopy.

Employee was struck by falling roof rock (shale) while operating the continuous miner. The rock fell from between permanent supports causing a laceration to the index finger right hand.

Employee was retrieving center line laser from the roof bolter when he was struck by falling rock causing a laceration to the left ear. Rock fell between two (2) permanent supports.

The employee was standing on the operator's side of the roof bolting machine near the inch tram controls when a piece of roof rock approx. 28"x38"x4"to a feather edge fell from between the bolts and struck him on the right shoulder.

Employee had installed outside bolt and moved drill head to put up inside bolt. A piece of rock approximately 1 inch thick x 4 foot wide x 5 foot long fell and struck in on the back of his right ankle due to employee working in a kneeling position.

Employee was struck on the left shoulder by falling rock while operating the continuous miner in the #4 heading 111' inby spad 56357 on the 004 section. Employee suffered contusion to the left shoulder.

Roof bolter was struck by a falling rock fracturing the middle finger left hand. While raising drill steel up to the mine roof a piece of roof rock fell striking the employee on the left arm. As he pulled back the rock struck the left hand pinching it against the drill boom.

Employee was removing a drill wrench from the drill pot when a small piece of draw rock fell and struck his left ring finger near the tip. This caused a laceration that required sutures.

Employee was operating a rail ride when a piece of draw rock fell and bounced into the operator's compartment striking him on the left side of his head. 5 Staples were required to suture the wound.

CUT OVER LEFT EYE, DRAW ROCK FELL FROM TOP & HIT HIM OVER THE EYE.

Employee had his hand on a piece of drill steel, ready to start a drill hole, when a small piece of rock struck him on the forearm causing a laceration to the arm that required 4 sutures.

Employee recieved lacerations to the lip and forehead which required sutures. Employee was installing a roof bolt when he was struck in the face by falling rock.

Employee was drilling a hole for a roof bolt when he was struck by a piece of rock. The employee recieved a laceration to the upper lip which required sutures.

EMPLOYEE WAS HELPING ROOF BOLTERS (MAKING UP ROOF BOLTS) IN THE FACE OF NO.5 ENTRY WHEN A PIECE OF ROCK FELL MEASURING 41"X51"X0"-4" FELL FROM THE TOP STRIKING THE EMPLOYEES HEAD AND BACK. THE ROCK FELL FROM BETWEEN 4 PERMANENT ROOF SUPPORTS FULLY RESIN 4'BOLTS. EE SUFFERED INJURIES - LACERATIONS & AVULSIONS TO THE FOREHEAD & LEFT EAR AS WELL AS INJURIES TO NECK & BACK.

EE reported that a piece of rock fell from between permanent supports and struck right hand, neck, and shoulders. Employee sought medical treatment. The area was supported with 5' resin bolts.

EMPLOYEE WAS WALKING THROUGH CROSSCUT & A PIECE OF DRAW ROCK FELL STRIKING HIS HEAD, KNOCKING HIM TO THE GROUND, CUTTING HIS HEAD & INJURING HIS RIGHT THUMB & LEFT SHOULDER.

ACCIDENT TOOK PLACE IN A CORSSCUT BETWEEN 6&7 RETURN ENTRY 1ST LEFT PANEL 80' INBY SPAD 46522. EE AND TWO OTHER EE WERE ASSIGNED TO REMOVE KENNEDY STOPPINGS IN THE OLD 1ST LEFT PANEL. AS HE WA S REMOVING THE LAST PANELING FROM THE OLD INTAKE STOPPING A ROCK MEASURING 34" WIDE X 36" LONG X 0"-2" THICK FELL FROMT HE MINE ROOF STRIKING HIM IN THE BACK AREA. HE WAS DIAGNOISED AS HAVING

THE EMPLOYEE WAS REMOVING SMALLER PIECES OF ROCK FROM THE TOP OF THE CONTINUOUS MINER WITH HIS LEFT HAND. WITHOUT WARNING A SMALL PIECE OF SLATE FELL FROM THE ROOF STRIKING THE LEFT HAND. THE ROCK INFLICTED A LACERATION TO THE LEFT THUMB WHICH REQUIRED SUTURES FOR CLOSURE.

EE'S LEFT HAND WAS STRUCK BY SHALE ROCK WHEN THEROCK FELL FROM BETWEEN TWO PERMANENT ROOF SUPPORTS. THE EMPLOYEE WAS GIVING INSTRUCTIONS TO ANOTHER EE TO TAKE THE SERVICE CAR TO THE MINER, THE EE HAD HIS HAND ON THE SERVICE CAR. AS A RESULT OF THE ROCK FALLING A DISTANCE OF 6-7 FT. & STRIKING HIS HAND, EE LOST THE SMALLER FINGER, RING FINGER TO 1ST PROXIMAL, MIDDLE TO DISTAL JOINT.
