EE stated that he was roof bolting when a piece of draw rock fell striking his right forearm. EE was treated at the ER at Middlesboro ARH where xrays showed no fractures. EE went to the Dr. on 11-3-10 and was admitted to the hospital with cellulitis in the area of the injury.

EE stated that he was standing at the back of the bolt machine as the foreman was checking a test hole in the roof, when a piece of draw rock fell and struck him on the right index finger. EE was taken to the hospital and recieved 13 stitches to the tip of the finger. X-rays showed no fractures. EE returned to work on the next scheduled shift.

EE was installing cable roof bolts, when a small piece of rock appx 6" in dia. X 1/8 " thick fell and struck him on the nose, resulting in 12 stitches to the 1" long laceration.

EE stated that he was installing a man door, when a piece of draw rock, appx. 1"x36'x36" fell and struck him on the back. EE continued to work until 8-8-08, when he consulted a Dr. that took him off work and started physical therapy.

Employee was roof bolting when a piece of rock approximately 12"x12"x2" fell from the roof and struck his right little finger mashing it against the head of the roof bolter. He was transported to the hospital and received 20 stitches.

Employee was backing the miner out of a cut when a small piece of rock fell from the roof, struck him on the back and caused him to fall on the remote box for the continuous miner. He was treated for bruising to the abdominal area and returned to light duty work on the next shift.

EE WAS WATCHING THE MINER CABLE WHEN A PIECE OF ROCK APPX. 12 INCHES X 12 INCHES UP TO 1 INCH THICK FELL FROM THE ROOF AND STRUCK HIS RIGHT RING FINGER. HE RECEIVED 9 STITCHES AT THE HOSPITAL AND RETURNED TO WORK ON THE FOLLOWING SHIFT.

As employee was moving a buggy, a piece of draw rock fell striking him on the neck and back, causing a small laceration on his neck requiring sutures and contusions on his back.

EE WAS HANGING CURTAIN WHEN A PIECE OF ROCK FELL AND HIT HIS RIGHT FOOT BEHIND STEEL TOE ON BOOT, BREAKING LITTLE (5TH) TOE. EE IS WORKING RESTRICTED DUTY.

EE WAS TRAMMING MACHINE FORWARD WHEN A PIECE OF ROCK FELL FROM BETWEEN THE LEFT ROW OF BOLTS AND THE RIB STIKING HIM ON THE SWHOULDER & HIP.

SMALL PIECE OF DRAW ROCK FELL DISLODGING HAT. ANOTHER PIECE FELL AND CUT SCALP AT HAIRLINE REQUIRING STITCHES.

EE WAS DRIVING A SHUTTLE CAR, WHEN A PIECE OF DRAW ROCK FELL, STRIKING HIS RIGHT LEG.

EE WAS INSTALLING A ROOF BOLT IN #4 RIGHT, WHEN A PIECE OF DRAW ROCK STRUCK HIM ON THE INDEX FINGER OF HIS LEFT HAND.

INJURED WAS RUNNING THE CONTINUOUS MINER BY REMOTE CONTROL WHEN A PIECE OF ROCK APPROX 3'X 3'X4"FELL OUT FROM BETWEEN THE ROOF BOLTS AND STRUCK HIM IN THE HEAD AND ON THE SHOULDER AREA.

INJURED WAS STRUCK BY PIECE OF ROCK THAT FELL OUT BETWEEN ROOF BOLTS.

I NAME WAS TRAMING S COOP 2 TO 3 BREAKS FROM FACE IN BELT ENTRY TOWARD FEEDER AS HE WAS TRAMING A PIECE OF DRAW ROCK ABOUT 2 TO 3IN T HICK ABOUT 3FT TO 4FT LONG FELL HITTING HIS NOSE CHIN LEFT SHOULDER & LEFT HAND CAUSING A LACERATION TO THE TOP OF HIS LEFT HAND

PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS & STRUCK EMPL IN LEFT FOOT. 2' X 1' X 4" PIECE OF ROCK.

EE WAS HELPING ON MINER AND A PIECE OF DRAW ROCK FELL AND STRUCK HIM ON THE LEFT FOOT.
