Cabin Hollow Coal

On the weekly examination of the bleeder system a roof fall was discovered in the number 1 entry between spad 227 and 244. The fall was approximately 20 feet long x 5 feet high x 160 feet long. The fall did not affect miners, ventilation nor equipment.

A roof fall was discovered in the #2 entry between #6 and #7 crosscut just outby spad 76. The fall is apx. 20ft long x 6ft high x 18 ft wide. No persons nor equipment were affected by the fall and it has been dangered off to possibly clean up at some point in the future.

A ROOF FALL WAS DISCOVERED IN THE CROSSCUT BETWEEN # 4 AND 5 ENTRY 2 XCUTS OUTBY SPAD 84. THE FALL DID NOT AFFECT MINERS, VENTILATION NOR EQUIPMENT. THE FALL HAS BEEN DANGERED AND CRIBBED OFF.

A rock fall was discovered in the 1st south east mains in a mined out area during a weekly examination. The fall is one crosscut inby spad 556 and 564 across the old #3 and 4 entries. The fall is apx. 140 feet long x 20 feet wide x 7 feet high. No persons, equipment nor ventilation were affected. The area has been dangered off.

EE stated that he was installing a man door, when a piece of draw rock, appx. 1"x36'x36" fell and struck him on the back. EE continued to work until 8-8-08, when he consulted a Dr. that took him off work and started physical therapy.

Employee stated that as he was positioning the roof bolter the boom canopy struck a piece of rock causing it to fall striking him in the leg. The rock was appx. 15 inches x 15 inches x 4 inches thick. He continued to work until 5-10-08 when he was diagnosed with a blood clot.

EE dropped to his knees to use a hydraulic jack, when his left knee popped and hurt. EE continued to work regular duty until he had surgery on 6/4/08. He is currently off work.

Employee stated that he was roof bolting when the roof strap spun around and struck him in the nose. He was seen at the ER where he received 9 stitches to his nose. He returned to work on his next shift.

A rock fall was discovered in the pillar inby spad 1041 in the #3 left entry outby the working section. The fall is approximately 5' high X 15' long X 20' wide. The fall was discovered at 1:40 pm. The area has been dangered and cribbed off. The fall did not have any affect on persons, ventilation nor equipment.

The 002 section had pillared 5 rows without an initial fall. The crew noticed that a fall was impending and moved and timbered the equipment to a safe location. The fall rode through the breaker timbers 1crosscut inby spad 1291 appx. 70' x 20'w x 10' high. MSHA was present at the time of the fall.

A roof fall was discovered outby the section in the #6 entry at spad 1281. The fall did not affect persons, ventilation nor equipment. The fall is approximately 20'x20' x 12 feet high. The area has been dangered and timbered off.

Employee was helping install track when something pulled in his shoulder. He went to the ER where the x-rays were negative. He continued to work until 11/15/07 when this accident became a loss time.

A ROCK FALL OCCURRED IN THE #2 ENTRY AT SPAD #1191 ON THE ACTIVE SECTION OF 001. DETERIORATING CONDITIONS HAD PROMPTED THE AREA TO BE DANGERED OFF PRIOR TO THE FALL. THE FALL AREA IS APPROXIMATELY 20'X20'X5'HIGH. THE AREA HAS BEEN CLEANED UP AND RESUPPORTED. NO PERSONS, EQUIPMENT NOR VENTILATION WERE AFFECTED.

A ROOF FALL WAS DISCOVERED IN THE #3 ENTRY AT SPAD 917. THE AREA IS APPROXIMATELY 20' X 80' LONG X 10' HIGH. THE FALL WAS LOCATED IN THE NEUTRAL OUTBY THE WORKING SECTION. THE FALL DID NOT AFFECT PERSONS, EQUIPMENT NOR VENTILATION. IT HAS BEEN DANGERED AND CRIBBED OFF.

Employee was operating a rail runner that was connected by chain to another rail runner when the chain broke and came back and struck him in the forehead. He was transported to the ER where he received 15 stitches. Employee returned to work on the next scheduled shift.

A roof fall was discovered in the # 5 entry 1 crosscut outby spad 399 outby the working sections. The fall was apx. 100'longx8'highx18'wide. The fall did not affect miners, ventilation nor equipment. The area has been dangered off and may be cleaned up at a later date.

EE WAS OPERATING THE RAIL RUNNER, WHEN A SHOVEL THAT HE HAD PLACED ON THE TOP OF THE RAIL RUNNER CAME INTO CONTACT WITH A SPILL BOARD AT A CONVEYOR BELT, CAUSING THE SHOVEL TO STRIKE HIM IN THE BACK OF THE HEAD. EE RECIEVED 5 STITCHES AND RETURNED TO WORK ON THE NEXT SCHEDULED SHIFT.

A rock fall was discovered in the #5 entry 1 crosscut out spad 923 at 2:15 p.m. The fall did not affect miners, equipment nor ventilation. The fall is approximately 9 feet high x 20 feet wide and 90 feet long. This area is well out by the active section. The area has been dangered and timbered off.

During the pre-shift examination prior to 2nd shift at 2:30 p.m. a rock fall was discovered in the #5 entry at spad 1040. The rock fall was approximately 20' X 19'x 5' high. This fall was reported immediately to MSHA supervisor and since the location was in the primary escapeway it was rerouted and all miners informed.

Upon examination of the neutral a rock fall was discovered in the #5 entry between spad 468 and 462. The fall was approximately 60' long x 19' wide x 12' high. The fall did not affect employees, ventilation nor equipment. The fall may be cleaned up at some point.

Employee was roof bolting when a piece of rock approximately 12"x12"x2" fell from the roof and struck his right little finger mashing it against the head of the roof bolter. He was transported to the hospital and received 20 stitches.

A roof fall was discovered by Mine Foreman and MSHA as they investigated 2 previous rock falls in an abandoned panel. The fall extended from spad 982 thru 980, 981 and from the right break in #3 across to #5 right in the belt line in #4. The area had been abandoned and did not have any affect on miners, ventilation nor equipment. The area has been dangered and cribbed off.

A rock fall occurred at 1:30p.m. as the section was being moved from the panel. The fall was above anchorage. The fall was inby spad 980. No persons, ventilation nor equipment were affected. the fall area was dangered and cribbed.

A rock fall was discovered in the #3 entry one crosscut inby spad #982. The fall was approximately 20'x20' x7' high. The area was cribbed and dangered off. Mining ceased and the equipment was moved from this panel. The fall did not affect ventilation nor equipment.

Found driver slumped over wheel, attempt to wake him up, apparently heart attack. Emt/ems onsite attempted care and transported to local hospital where he was pronounced dead. MSHA was notified immediately when ems, an investigation was completed by MSHA on truck and site, no violations were found.

EE stated that he was working on a belt set up, when he stepped on a piece of belt that was laying on the bottom as it was pulled by the scoop. EE saw a Dr. on that day and continued to work until 11/16/07, when this became a lost time injury.

Upon examination of the left return a roof rall was discovered. The fall is located at spad 401 on the 2nd south west mains in the left return. The fall is approximately 20 feet wide x 9 feet high x 90 feet long. The fall did not affect miners, ventilation nor equipment. The area has been cribbed and dangered off.

Employee stated that he was loading rock dust onto the scoop when he turned with a bag of dust in his arms and hurt his left knee. He continued to work his regular job and consulted a Dr. who performed surgery on 3-28-07 to repair cartlidge in his knee.

Employee was pulling cable off the reel of the shuttle car when he felt something pull in his lower back. He continued to work until 06/04/07 when he missed his first shift of work and this became a lost time accident.

As employee was moving a buggy, a piece of draw rock fell striking him on the neck and back, causing a small laceration on his neck requiring sutures and contusions on his back.

Employee was operating the roof bolter when his left knee started hurting. He continued to work on regular duty without lost time until 1/17/07 when he underwent surgery for a meniscus tear.

A roof fall has occurred in the number 2 entry one break inby spad #503. The fall measures 20' X 20' X 5 1/2' in thickness. The fall did not affect ventilation.

As employee was lifting timber, he felt a pull in his groin. Employee had hernia surgury on 8/01/2006 which was his first day missed.

While stepping off a flatcar, employee claimed to have pain running up back. He became restricted duty on 3/29/2006.

At 10:30 A.M. on this date a rockfall was discovered in the intake at spad # 487. The fall measured 20' X 20' by 4-5' in thickness. The fall was dangered off with cribs and timbers set. Ventilation was not affected.

On this date a roof fall was found in #1 entry three breaks inby spad 411. The fall is 20'x20' and 5' in thickness. Ventilatioin was not impaired.

As employee was operating a chain ratchet to change a tailpiece bearing, the ratchet slipped striking his right hand resulting in a broken bone.

As employee was reaching for something on the roof bolter, he cut his left arm on a strap. Recieved 9 staples in his arm.

SHOVELING BELT LINE. EE STEPPED ON BOTTOM BELT AND IT PULLED HIS LEFT FOOT FROM UNDER HIM (STRAINED BACK).

While installing a roof bolt, a piece of draw rock fell striking employee in the nose. His reaction to the falling material caused him to pull his head back, striking his head against the roof bolter canopy causing a neck strain. Employee started missing work on 7/25/2005.

Going up mine road to pick u pump and met a coal truck that was driving left of center of the road in a curve and collided head on.

EMPLOYEE WAS INSTALLING STRAPS ON A BROW WHEN ONE OF THE STRAPS TURNED STRIKING HIM IN THE FOREHEAD CASUING A LACERATION

As employee was loading roof bolting machine with supplies, he felt a sharp pain in his lower back. The employee began missing work on 6/29/2005 due to this injury.

A roof fall was found in the #3 main entry one crosscut inby spad #75. The fall was 20' by 20' by approximately 9 feet in thickness. The area was dangered off, and additional support was set in the area. The fall did not disrupt ventilation or the mining cycle.

Employee's left shoulder started hurting on this date. No specific injury noted. Went to Dr on 3-11-05. Doctor took him off work and said pain was work related. Started missing work on 3-11-05.

EMPLOYEE WAS MOVING A BUNDLE OF ROOF STRAPS WHEN HE FELT PAIN IN HIS LOWER BACK. EE BEGAN MISSING WORK ON 3/18/05.
